A 16-month-old Charolais bull was examined because of acute onset of scrotal and hind limb edema, fever, tachycardia, tachypnea, anorexia, and lethargy. Scrotal circumference on initial examination was 48 cm. Clinicopathologic abnormalities included microcytic, normochromic anemia and numerous Eperythrozoon organisms in blood smears. Results of immunohistochemical staining of a skin biopsy specimen suggested that the edema was the result of an Arthus-type reaction. Semen quality deteriorated rapidly, and the bull was aspermic within 7 days. The bull was treated with oxytetracycline, and the anemia and edema gradually subsided. Eperythrozoon organisms were not detected in blood smears after 3 days. Six months after initial examination, results of physical examination and semen evaluation were normal. We hypothesize that scrotal edema caused failure of testicular thermoregulation, resulting in transient production of abnormal sperm and infertility.

A 33-year-old woman presented to the emergency department (ED) with tachypnea, tachycardia, and hypoxia, complaining of dyspnea and paroxysms of dry cough for 2 days. The patient had undergone an unsuccessful in vitro fertilization procedure 1 month earlier and had had another embryo implantation 6 days earlier. She had been receiving intramuscular injections of estradiol and progesterone in sesame oil to support implantation. A chest radiograph demonstrated extensive bilateral pulmonary consolidation; a leukocytosis count of 19.7 x 10(9) with 20% eosinophils was noted on a CBC count. A MEDLINE search yielded a case report in infertility literature describing an eosinophilic pneumonitis with similar clinical features after injections of progesterone in sesame oil. Treatment with intravenous corticosteroids was initiated in the ED, resulting in symptom and chest radiograph result improvement within 2 days and sparing the patient further imaging and antibiotic therapy.

In this case-control study, we examined infants delivered vaginally at 37 weeks or later to identify factors associated with transient tachypnea of the newborn (TTN). We reviewed the obstetric records of all vaginal deliveries at the Japanese Red Cross Katsushika Maternity Hospital from 2005 through 2007. Demographic information and the characteristics of labor were extracted from patient charts. Multivariate analysis identified that the incidence of TTN was significantly associated with nulliparity; a history of infertility therapy, such as in vitro fertilization; augmentation of labor; nonreassuring fetal status; vacuum/forceps delivery; and low Apgar score (<7) at 1 and 5 minutes. In addition, a low Apgar score at 1 minute was the factor most strongly associated with the incidence of TTN (adjusted odds ratio, 20; 95% confidence intervals, 12-34; p<0.001). The present results indicate that the improvement of obstetric surveillance to diminish the frequency of low Apgar scores is important for preventing TTN in infants delivered vaginally at 37 weeks or later.

Although hydrothorax may accompany abdominal ascites in women with severe ovarian hyperstimulation syndrome (OHSS), there are few cases reported with isolated pleural effusion. Herein, we report two patients with isolated hydrothorax without any significant abdominal fluid following infertility treatment, along with a systematic review of the literature to describe risk factors for this rare entity. Two women with isolated pleural effusion without significant abdominal ascites were reported. The available literature was screened from Ovid-SP and PubMed to review OHSS cases with isolated hydrothorax. Two women aged 28 and 31 years were admitted to hospital with chest pain, tachypnea, and tachycardia after infertility treatment. They had right pleural effusion without abdominal fluid and the symptoms relieved after thoracentesis. Similar to our cases, we identified 24 case reports (n=41 women) in the literature according to eligible criteria. On the day of triggering, estradiol (E2) level was <4000 pg/mL in 81% of reported cases and hematocrit (HCT) was <45% in 44% of cases at the time of diagnosis. Isolated hydrothorax is an unpredictable event, which may even complicate women with low E2 levels or HCT concentrations. Physicians should keep in mind the possibility of isolated hydrothorax when respiratory symptoms are significant but abdominal ascites is not evident.
