In 35 of 316 patients suffering from severe cancer pain, an epidural catheter was placed for continuous morphine application. Indications for epidural opiates included failure of pain relief with oral morphine, severe side effects with oral administration, and contraindications for oral morphine, e.g., ileus. METHODS. The epidural catheter was inserted in the lumbar, thoracic or cervical region, according to the main localization of pain. A

catheter with a Dacron cuff (4.2 F Broviac Davol, Cranston, R.I.) was connected and tunneled subcutaneously to a distant exit on the lateral chest wall (Fig. 2). A portable morphine pump (CADD-

Pharmacia Deltec) was connected to the externalized catheter. The morphine was infused continuously at a basic rate. It could be increased to a programmed limit by additional boli determined by the patients themselves. Thirty patients were treated as outpatients. RESULTS. The mean duration of treatment was 101 (10-333) days. The daily dose of morphine ranged from 9 to 200 (33) mg at the beginning of therapy, and from 20 to 288 (88) mg at the end of treatment. In 27 patients (77%) epidural morphine administration proved to be a valuable method of pain control (Fig. 3). Even in most cases of tolerance to oral morphine, especially in patients suffering from pain of neuropathic origin, pain control was adequate. There were no cases of continuous loss of effectiveness of continuous loss of effectiveness or development of tolerance (Fig. 5). The epidural morphine dosage depended on the character and intensity of pain and its responsiveness to epidural opiates. Technical complications were noticed in 6 patients (17%), and fairly mild side effects of epidural morphine occurred in 20% of the patients for a limited time. CONCLUSIONS. The technique described is a simple and convenient method for long-term treatment of cancer patients with epidural morphine. There was no need for more invasive procedures, such as intrathecal or intraventricular morphine administration, in this group of patients in whom no pain relief had been achieved with oral morphine administration.

We demonstrate the use of spots taken with magnetorheological finishing (MRF) for estimating subsurface damage (SSD) depth from deterministic microgrinding for three hard ceramics: aluminum oxynitride (Al(23)O(27)N(5)/ALON), polycrystalline alumina (Al(2)O(3)/

), and chemical vapor deposited (CVD)
silicon
carbide (Si(4)C/SiC). Using various microscopy techniques to characterize the surfaces, we find that the evolution of surface microroughness with the amount of material removed shows two stages. In the first, the damaged layer and SSD induced by microgrinding are removed, and the surface microroughness reaches a low value. Peak-to-valley (p-v) surface microroughness induced from grinding gives a measure of the SSD depth in the first stage. With the removal of additional material, a second stage develops, wherein the interaction of MRF and the material's microstructure is revealed. We study the development of this texture for these hard ceramics with the use of power spectral density to characterize surface features.

Concerns over the toxic by-products produced by traditional ammunition have led to an increase in popularity of nontoxic ammunition. In this work, the chemical composition of six brands of nontoxic ammunition was investigated and compared to that of a road flare, which served as an environmental source with similar composition. Five rounds of each brand were fired while a further five were disassembled and the primer alone was fired. Particles collected from all samples, including the road flare, were analyzed by scanning electron microscopy with energy dispersive X-ray analysis. Common elements among the different ammunition brands included aluminum, potassium,

, calcium, and strontium. Spectra were then subjected to principal components analysis in which association of the primer to the intact ammunition sample was generally possible, with distinction among brands and from the road flare sample. Further,

An accurate description of muscular activity plays an important role in the clinical diagnosis and rehabilitation research. The electromyography (EMG) is the most used technique to make accurate descriptions of muscular activity. The EMG is associated with the electrical changes generated by the activity of the motor neurons. Typically, to decode the muscular activation during different movements, a large number of individual motor neurons are monitored simultaneously, producing large amounts of data to be transferred and processed by the computing devices. In this paper, we follow an alternative approach that can be deployed locally on the sensor side. We propose a neuromorphic implementation of a spiking neural network (SNN) to extract spatio-temporal information of EMG signals locally and classify hand gestures with very low power consumption. We present experimental results on the input data stream using a mixed-signal analog/digital neuromorphic processor. We performed a thorough investigation on the performance of the SNN implemented on the chip, by: first, calculating

neurons at the input and the hidden layers to show how the network helps in separating the samples of different classes; second, performing classification of the data using state-of-the-art SVM and logistic regression methods and a hardware-friendly spike-based read-out. The first systematic study on high fluoride contaminated fractured rock aquifer in South Africa using geochemical and chemometric approach is presented. Groundwater samples were collected from 49 boreholes and analysed for EC, pH, major and minor ions. The groundwater with high fluoride is associated with NaCl water types. The groundwater chemistry is governed by mineral weathering, evaporation and wastewater infiltration. PHREEQC modelling indicates that carbonate mineral saturation is decreasing with increasing salinity due to evaporation whereas fluorite saturation increases with increasing F being one of the prime controlling factors.

minerals namely chalcedony, quartz and talc express saturation and over-saturation while chrysotile and sepiolite are undersaturated in most of the samples.

provided four factors and justified the role of mineral weathering, evaporation, ion exchange, longer residence time and anthropogenic impacts on water chemistry. R-mode and Q-mode cluster analysis resulted in four clusters. In cluster 1, (29%) of groundwater is less mineralized due to recent recharge. In cluster 2 (45%), groundwater chemistry is governed by weathering of silicates and fluorite minerals. Cluster 3 (20%) reveals the impact of anthropogenic activities and induced mineral weathering. implying that groundwater is affected by evaporation and longer residence time promoting mineral-water interaction and precipitation of carbonate minerals. More than 50% of the study area is degraded due to fluoride contamination which requires proper remedial actions and further investigations on human health risk due to impact of fluoride contamination in groundwater is recommended.
