The disposition of morphine when administered by i.m. injection was studied in 36 patients receiving morphine as part of premedication before general
anaesthesia
, and in five patients who received morphine as a postoperative analgesic after median sternotomy for coronary artery surgery (
PCA
group). Maximum plasma concentration of morphine (CP max) was 75.3 +/- 6.0 (mean elimination rate constant (k) 4.85 X 10(-3) min-1 and half-life (T1/2) = 143 min for the preanaesthetic group. The corresponding values for
PCA
group were CPmax = 58.0 +/- 18.0 ng ml-1 (range 30--130 ng ml-1), k = 5.63 X 10(-3) min-1 and T 1/2 = 123 min. Analysis of variance showed no differences between the groups. Within the preanaesthetic group, there was a significant difference in k between males (k = 4.01 X 10(-3) min-1) and females (6.30 X 10(-3) min-1, P less than 0.01). The corresponding T 1/2 for males was 173 min; and 110 min for females. The variation in the disposition of morphine is thought to be the result of variations in resting muscle blood flow and inadvertent injection into adipose tissue. There were no significant differences between males and females in the preanaesthetic group with respect to age, Cpmax or time from injection to Cpmax.

and diaphragm discharge; but EMG activity returned to the

muscles before the diaphragm. Further, reactivation of the diaphragm was not always accompanied by a synchronized burst of

activity. Therefore, it is possible under some conditions to dissociate the motor outputs from the

muscles and diaphragm in this immature mammal.

In this prospective study, the postoperative analgesic effects of intraoperative iv clonidine were evaluated. Two hundred consecutive patients undergoing major abdominal surgery were randomly assigned to either balanced
anaesthesia
with iv clonidine (Group 1) or balanced
anaesthesia
alone (Group 2). A
PCA
infuser was connected immediately after tracheal extubation. It was programmed to deliver morphine "on demand" iv boluses at doses of 1 mg for patients greater than 65 yr and 1.5 mg for women or 2 mg for men less than 65 yr old. A blinded observer assessed postoperative analgesia by recording the analgesic demands (both met and unmet), patient pain scores, sedation scores, and any side effects during the first 36 hr after surgery. Intraoperative clonidine reduced the number of analgesic demands during the observation period (45 +/- 27 demands in Group 1 vs 81 +/- 60 in Group 2, P = 0.0001). This resulted in a reduction in morphine delivered (55.4 +/- 30.6 mg vs 67.1 +/- 45.1 mg, P less than 0.05), mainly during the first 12 hr (19.7 +/- 11.1 mg vs 27.6 +/- 18.1, mg P less than 0.001) and the unmet demand rate was also reduced at all time intervals (P less than 0.01). Clonidine did not exacerbate sedation or side effects. However, clonidine provided better analgesia in men and in patients less than 65 yr of age. Transdermal fentanyl 75 micrograms/h (Fentanyl-TTS) was compared with placebo in a randomized double-blind study in the early postoperative period, using 50 patients recovering from major urological operations. Analgesic efficacy was individually titrated with intravenous fentanyl by means of a

pump (demand dose 34 micrograms, lockout time 5 min). The test systems were applied 8 h before
anaesthesia
and were left in situ for 24 h. During the
PCA
period (18.2 h) patients with Fentanyl-TTS required significantly less additional fentanyl (0.48 vs 0.93 micrograms.kg-1.h-1) and reported less pain than patients in the placebo-group. Patient acceptance was high in both groups. Side-effects were of only minor intensity and did not differ between the two groups. We compared the postoperative epidural analgesia provided by the continuous epidural infusion of bupivacaine supplemented with patient-controlled injection (
PCA
) of epidural fentanyl with that provided by a continuous infusion of bupivacaine supplemented with a continuous epidural infusion of fentanyl. Our patient population comprised 16 ASA physical status I or II patients undergoing laparotomy with a midline incision under general
anesthesia
combined with bupivacaine epidural analgesia. Post-operatively, a continuous epidural infusion of bupivacaine (0.1 mg.kg-1.h-1) was combined with epidural fentanyl given by either (a)
PCA
(15-micrograms bolus with a lockout interval of 12 min, n = 8) or (b) continuous infusion (1 microgram.kg-1.h-1, n = 8). In the case of inadequate pain relief in the latter group, the fentanyl infusion rate was increased by 10 micrograms/h. Analgesia evaluated by a visual analogue pain score and by a verbal pain score was similarly effective in both groups. The sedation score was also similar in both groups. The total dose of epidural fentanyl administered during the first 24 h was significantly lower in the
PCA
group than in the continuous infusion group (405 +/- 110 micrograms vs 1600 +/- 245 micrograms, P less than 0.001). The dose of fentanyl given during each 4-h interval ranged between 40 and 160 micrograms in the
PCA
group and 251 and 292 micrograms in the continuous infusion group. Clinically detectable respiratory depression was not observed in either group. In conclusion, epidural administration of 0.1 mg.kg-1.h-1 bupivacaine combined with fentanyl provides effective postoperative analgesia with a total dose of fentanyl required that is lower when fentanyl is administered by epidural

The study was designed to compare five opioid analgesic regimens administered after cesarean delivery in a routine hospital setting with respect to patients' perceptions of their pain relief and the impact of analgesic technique on recovery and hospital costs. After cesarean delivery, 684 patients received one of the following: epidural morphine, alone (EM,n = 128), or with fentanyl (EM + F,n = 245); subarachnoid morphine (n = 48); intramuscular meperidine (n = 165), or patient-controlled analgesia using meperidine (

On the first three postoperative days (Days 1-3; day of operation is Day 1) patients were surveyed regarding their impressions of their analgesia, the incidence of side effects, times to resume normal activities and satisfaction with their technique. . Forty patients were divided at random into two groups with 20 patients in each group. Patients in group 1 received 0.5-1.0 mg intravenous midazolam and 25-50 micrograms intravenous fentanyl in increments administered by the anesthesiologist to achieve intraoperative sedation; patients in group 2 self-administered a mixture of midazolam (0.5 mg) and fentanyl (25 micrograms) in increments using an Abbott Lifecare
PCA
infuser to achieve sedation. Demographics of the patients, the types of surgery performed, doses of midazolam and fentanyl administered in a given period of time, and the level of sedation maintained during epidural
anesthesia
and surgery were similar in both groups. Patients in the self-administered group, however, rated their level of comfort during
anesthesia
and surgery higher than did those in the anesthesiologist-controlled sedation group. This could have been due to a positive psychological effect produced by allowing patient to feel that they have some control over their situation. The findings of this study indicate that patient-controlled sedation using a combination of midazolam and fentanyl is a safe and effective technique that provides intraoperative sedation ranked better by patients than that provided by anesthesiologists using the same drugs. More studies are, however, needed to determine the best choice of drug(s), the doses, the lock-out intervals, and the possible use of continuous infusion with patient-controlled sedation.

A bolus of epidural morphine was administered through an indwelling thoracic (Th8.9) catheter at 3 hrs prior to the expected end of surgery, which was followed with continuous epidural infusion of morphine at a rate of 0.167-0.042 mg.hr-1 with the pump during and after
anesthesia
and surgery with gradually decreasing dose until the third postoperative day. The remaining eight patients (systemic analgesics group) repeatedly received intravenous or intramuscular pentazocine and buprenorphine when needed. Plasma GH levels increased significantly only on the first postoperative day in both groups. Plasma insulin levels increased significantly on the first postoperative day in both groups. Blood glucose levels increased significantly at the end of surgery and during the following three postoperative days in both groups. There are no statistical differences in plasma GH, insulin and blood glucose levels between the two groups. Plasma prolactin concentrations increased significantly at the end of surgery and they were significantly higher in the systemic analgesic group than in the epidural morphine group. They, however, returned to the previous day's levels on the first postoperative day in both groups. Our study suggests that continuous epidural infusion of morphine has no suppressing effect on postoperative changes in plasma GH, insulin, prolactin and blood glucose levels as compared with systemic analgesic regimen.

Plasma dopamine levels were unchanged in the epidural morphine group during the surgical procedures, but they increased significantly on the first postoperative day and thereafter in the systemic analgesics group. Our study suggests that continuous epidural infusion of morphine is adequate for postoperative pain relief and exerts a suppressing effect on plasma catecholamine levels as compared with systemic analgesics regimen.
