We present the case of a 62-year-old male patient who entered our center through the Emergency Department due to an acute pain in the right hypochondrium, dyspnea and deterioration of general condition. This clinical situation could be related to the rupture of a hydatid cyst and discharge of the content into the adjacent pleural cavity. Images are consistent with complex hydatid cyst with pleural fistulization and secondary pleural effusion.
