This is the first case report of a ruptured aortic aneurysm presenting with acute right buttock pain. The patient was an 80 year old man. A literature search revealed one report of ruptured internal iliac artery aneurysm presenting with acute
hip pain
and another of an unruptured aortic aneurysm presenting with chronic
hip pain
. Thus the present case is another unusual presentation of ruptured
abdominal aortic aneurysm
and highlights the importance of careful history taking and clinical examination. A high index of clinical suspicion of aneurysm rupture should be maintained in elderly patients presenting with a history of collapse.

The rupture of an
abdominal aortic aneurysm
(
AAA
) is a catastrophic event. Misdiagnosis by first-contact emergency physicians remains a serious concern. Varied and frequently nonspecific presentations lead to erroneous diagnostic impressions and cause significant delays in definitive intervention. We report the case of a 73-year-old man with a ruptured
AAA
presenting with isolated acute right
hip pain
without any classical features such as truncal pain or hypotension. Despite major advances in imaging and definitive treatment, a heightened awareness among emergency physicians remains the only effective means of improving detection and thereby survival.

The presentation of an
abdominal aortic aneurysm
(
AAA
) with isolated
hip pain
is a rare phenomenon. We present an atypical case of a 58-year-old previously fit man who presented with a 6-month history of progressively worsening left
hip pain
associated with unintentional weight loss, tender bilateral testicular swellings and a large non-tender palpable mass on abdominal examination. Urgent abdominal CT scan findings revealed a 15 cm infrarenal
abdominal aortic aneurysm
extending to the aortic bifurcation associated with an extensive left hydronephrosis. In theatre, the diagnosis of inflammatory
AAA
(IAAA) was confirmed following the presence of pyuria and a successful repair with an open approach using a bifurcated dacron graft was performed.

This is the case report of a contained ruptured aortic aneurysm presenting with acute cauda equina syndrome. The patient was a 79-year-old man. A literature search revealed various unusual presentations of
abdominal aortic aneurysm
(
AAA
), including femoral neuropathy,
hip pain
and others; however, there are no other reports of cauda equina-like syndrome. The present case is therefore another unusual presentation of ruptured
abdominal aortic aneurysm
and reiterates the utmost importance of careful history taking and clinical examination.
Abdominal aortic aneurysm
rupture should always be considered in the differential diagnosis of sudden onset isolated lower limb neuropathies and or pain.
