A patient with a relatively localized occult carcinoma of the lung and hemorrhage secondary to chronic disseminated intravascular coagulation (DIC) which continued for eight months is described. Despite continuing DIC two major operations were performed without excessive blood loss. Preoperative heparinization, in vitro clotting of the arterial graft, and temporary postoperative reversal of systemic heparin were utilized for abdominal aortic aneurysm resection. Blood product replacement therapy facilitated an emergency laparotomy. The patient was also managed successfully for several months without anticoagulant therapy. A hypothesis that vascular thrombi are not a necessary prerequisite for DIC is proposed. This hypothesis is based on the absence of thrombi or evidence of ischemia in this patient and in others and is supported by experimental observations made by other investigators.

The cell substrate adhesion of the Lewis lung carcinoma in vitro maintained sublines (LL2 basic line, WGA-resistant LL2-8, and Aleuria aurantia-resistant LL2-AAA) has been studied by a hydrodynamic method using various shearing forces generated by the medium flow. The force of adhesion of the LL2 cells is about 10(-12) N to 10(-11) N with statistically significant differences between various sublines: it is the highest for LL2-AAA cells, intermediate for LL2 cells and the smallest for LL2-8 cells. In another series of experiments the distribution of the number of single cells and multicellular aggregates in the suspensions of LL2 cells together with the effect of gravitational sedimentation were examined. When the LL2 cells are incubated in 37 degrees C they display active motile behaviour which consists in forming cell-surface extensions of various shapes and duration (from a few seconds to several minutes and more). The displacement of the whole cells (locomotion) has not been observed. The results of the study on adhesion and motility of LL2 cells are discussed from the point of view of their metastatic properties, cell-membrane structure and mechanisms of malignant invasion.

A man with small-cell carcinoma of the lung underwent a Tc-99m red blood cell gastrointestinal bleeding study because he had a tarry stool. In his first-pass study, there was early filling of the radiotracer in the middle and lower portion of the inferior cava; in addition, the study revealed a distorted and distended inferior vena cava along with an abdominal aortic aneurysm. Collateral circulation of the trunk was noted in the subsequent bloodpool images. A subsequent radionuclide superior vena cava study confirmed superior vena cava obstruction just above the entrance to the right atrium. Contrast-enhanced computed tomographic scans of the chest showed that the superior vena cava was compressed completely by the mediastinal tumor mass and that collateral circulation was present. Development of collateral circulation after superior vena cava obstruction in this patient allowed venous blood to return to the right atrium through the inferior vena cava. Thus, early filling of the inferior vena cava on radionuclide first-pass studies of the abdomen may serve as a sign of superior vena cava obstruction.

The protein coding segment of the TP53 genes from the glioma-derived cell lines M059J and M059K was sequenced. The sequences from both cell lines were identical over 5039 bp, including the gene segment containing exons 2 through 9, exon 10, and the proximal segment of exon 11. In both cells, the first nucleotide of codon 286 (GAA, Glu) is changed to an A (AAA, Lys). Comparison with the same TP53 segment from the A549 human lung carcinoma cell line revealed several differences in intron sequence.

The adrenal gland is the most common site of extranodal spread from primary lung cancer. We report a case of adrenal hemorrhage secondary to metastasis in a patient with primary large cell carcinoma of the lung presenting as a suspected ruptured abdominal aortic aneurysm.
