The differential diagnosis of left lower quadrant abdominal pain in an adult man includes, among others, sigmoid diverticulitis; leaking
abdominal aortic aneurysm
; renal colic;
epididymitis
; incarcerated hernia; bowel obstruction; regional enteritis; psoas abscess; and in this rare instance, situs inversus with acute appendicitis. We report a case of situs inversus totalis with left-sided appendicitis and a brief review of the literature. There were several subtle indicators of total situs inversus present that were missed by the physicians and surgeons who initially evaluated the patient prior to surgery. Computed tomography scan with contrast, however, revealed the diagnosis immediately, and treatment was successfully initiated.

We report the case of a 73-year-old male patient who was a candidate for correction of an
abdominal aortic aneurysm
without abnormalities in his first cardiological evaluation. The surgery was postponed because of the need for treatment of
epididymitis
. Two weeks later, the patient returned to the hospital with thoracic pain, when the angiography showed obstructions in 2 coronary arteries, which were successfully treated with percutaneous transluminal angioplasty and stent implantation. After 45 days, the patient underwent surgery for correction of the
abdominal aortic aneurysm
under peridural and general anesthesia. The patient evolved without complications.

A 76-year-old man was admitted to our hospital for investigation of an apparent
abdominal aortic aneurysm
detected during treatment for
epididymitis
. A chest X-ray showed miliary shadows in the bilateral lung fields strongly suggestive of tuberculosis. The diameter of the aneurysm increased, and examinations showed impending rupture of a pseudoaneurysm. However, a definitive disease pathogenesis was not obtained before surgery. We performed a subemergency operation, which revealed an infrarenal abdominal pseudoaneurysm caused by tuberculosis. The pseudoaneurysm appeared to have resulted from direct extension of tuberculous lymphadenitis to the aortic wall, which ruptured. We review 24 other cases of tuberculous aortic aneurysms surgically treated in Japan before 2004.

. The patient underwent surgical repair with an aortic-bifemoral prosthesis and was ultimately discharged without further complications. This is the fourth report in the literature of orchidodynia as referred pain from an aneurysm of the abdominal aorta.
