Plummer-Vinson syndrome is a clinical entity associated with dysphagia,
sideropenic anemia
and atrophic
glossitis
. Using rabbits with
iron deficiency anemia
, the author investigated the cause of dysphagia in this syndrome. Iron deficient animals were produced by bolus feeding without iron and intramuscular injection of an iron-chelating agent. The fibers of swallowing muscles (the thyro-pharyngeal, cricopharyngeal and cervicoesophageal muscles) were classified into three types (Type 1, 2A and 2B fibers) by actomyosin ATPase staining. No significant difference between the muscles of sideropenic rabbits and those of normal rabbits were found in the composition and distribution of their muscle fibers. By NADH-TR staining, however, the disturbance of the intermyofibrillar network and/or a "Moth Eaton" appearance, known to be caused by leakage of mitochondria, were observed in Type 1 fibers of the swallowing muscles of sideropenic rabbits. These morphological changes are similar to those observed in progressive muscular dystrophy. The quantity of iron in the swallowing muscles of sideropenic rabbits was significantly reduced in comparison with that in the sternothyroid and femoral muscles. This finding suggests that a selective decrease in myoglobin occurs in the swallowing muscles of iron deficient animals. From these observations, it might be concluded that dysphagia in

is caused by a myogenic disorder.

Plummer-Vinson syndrome is known as the association of postcricoid dysphagia, upper esophageal web, and
iron deficiency anemia
. Although correction of iron deficiency may result in resolution of dysphagia and sometimes disappearance of the webs, dilation therapy is usually necessary to remove webs and relieve dysphagia. We report two cases of Plummer-Vinson syndrome. Both patients presented with significant and longstanding dysphagia, sideropenia,
glossitis
and koilonychia. Our two patients had occasional choking and aspiration episodes at eating and endoscope did not pass through at the level of the upper esophagus. Patients' esophagograms revealed the presence of webs in part of the post-cricoid region. Both patients were treated with esophageal bougienage or balloon dilation, and iron supplementation. The patients were examined periodically for two years after the initial treatment and found to be in good general condition.

A significant positive correlation of blood hemoglobin status and the endurance capacity as the function of nutritional status of a cross-sectional population of adolescent rural girls (13-15 years) of Thrissur district, Kerala State, India has been reported. Anthropometric measurements revealed that mean body weight and height of adolescent girls were low compared to Indian standards and BMI indicated a high prevalence (44 per cent) of chronic energy deficiency-Grade III. Very high portion (88.7 per cent) of the rural population of adolescent girls had energy intakes below normal standards. The prevalence of major deficiency diseases like dental caries, pigmented skins,
glossitis
and anaemia were also studied as functional responses. More than half of the population was suffering from either or more of the deficiencies and among the clinically identified symptoms,
iron deficiency anaemia
was most prevalent (30.0 per cent). Only 40 per cent had acceptable levels of haemoglobin (> 12g/dl). Though none of the adolescent girls had excellent or even good endurance capacity, two-way frequency analysis revealed a consistent increase in the same with acceptable levels of haemoglobin (Y = 3.33X + 26.11) and the nutritional status. A wide array of anemic and hemostatic disorders encountered in internal medicine has manifestations in the oral cavity and the facial region. Most of these manifestations are non-specific, but should alert the hematologist and the dental surgeon to the possibilities of a concurrent disease of hemopoiesis or hemostasis or a latent one that may subsequently manifest itself.

. Although it is relatively uncommon, this condition is important to recognize because it is a source of dysphagia and it confers an increased risk for hypopharyngeal cancer. Cases of PVS associated with gastrointestinal conditions such as celiac disease and gastric cancer have been previously reported in the literature, but as far as we know, no case of PVS associated with bariatric surgery has been previously reported. We describe the case of a 39-year-old woman who developed PVS following gastric bypass surgery, and we briefly discuss the current knowledge of this syndrome.

A classic case report of PVS with clinical features, oral manifestations, malignant potential, differential diagnosis, investigation, dental implication, and treatment is discussed here with the literature review from the dentist's point of view. The article carries a message that dental surgeons have to be familiar with the oral manifestations of anemia and be able to suspect PVS to aid in early diagnosis and prompt treatment.

. The patient was a 63-year-old woman with a clinical history of
iron deficiency anemia

glossitis
in her 20s to 40s and who had experienced swallowing difficulties for the past 20 years. A membranous stricture was found in the cervical esophagus during a fluoroscopic examination. An endoscopic examination conducted under general anesthesia revealed an oblique linear scar on the proximal surface of the stricture. Sequential balloon dilation was performed successfully. We suggest that the esophageal web formation might have been related to the healing of an esophageal ulcer.
