In patients with multiple
bone metastases
of malignant tumors, bundle-nailing according to Hackethal is a fast and non-traumatic way of stabilizing existing or impending pathological fractures. It is also a therapeutic alternative in benign lesions, provided it is genuinely indicated and is performed correctly. Either as primary treatment following resection (sometimes with spongioplasty) or as secondary treatment, i.e. stabilization, it can be a valuable technique. Between 1976 and 1986 we performed 14 operations in our hospital using this technique. The tumor was malignant in 11 cases, benign in 3 and unclear in 1 case. The incidence of unfavorable results due to corrosion has been reduced recently, in particular since
titanium
has been used.

Only a few plate systems are available for anterior fixation of thoracolumbar vertebrae because of the difficulty in fastening a screw and a plate together. If the fixation is inadequate, the screws will become loose. The Rigix plate system consists of screws made of
titanium
alloy and a plate made of pure
titanium
. All screws used for internal fixation are screwed into the plate. This system permits the use of anchor screws, which facilitate exertion of force to compress the vertebral bodies together or to distract them from each other. In this study, Rigix plates were used in 24 patients (20 with burst fractures and 4 with metastatic tumors). In the 20 patients with fracture, internal fixation with a graft and a Rigix plate was performed after anterior decompression. In the four patients with malignant tumors, total spondylectomy was performed anteriorly and posteriorly, followed by implantation of a vertebral prosthesis, and then internal fixation with a Rigix plate combined posterior instrumentation with Diapson (Stryker Co., Tokyo, Japan) pedicular screws. Bone union was achieved in all patients. Neither breakage of instruments nor loosening of connections occurred in any case. In patients treated for
bone metastases
, the reconstructed spinal structure was able to be maintained for a long period. Of the screws used, five (5%) were not able to remain fixed as intended because they were inserted at inappropriate angles into the plate, but the fixation itself was excellent. Because of the low profile, ease of manipulation (mean instrumentation time was 25 min), and compatibility with magnetic resonance imaging, the Rigix plate is useful for anterior fixation of thoracolumbar vertebrae.

Seventeen segmental resections of diaphyseal tumors and five knee resection arthrodeses were reconstructed using the Huckstep nail with intercalary bone grafts (fibula), cementation, and ceramic and
titanium
spacers. Although the bone defects were 0 to 25.5 cm in length (mean, 13.1 cm), all patients had relief of pain and were able to move alone with a wheelchair or a cane. Nail breakage occurred in four patients who had reconstruction surgery using a ceramic spacer (four patients without intercalary fibula graft and one patient with intercalary fibula graft). The nail survival rate was significantly better in the patients with reinforcement by vascularized fibular graft (100% at 5 years and 75% at 10 years; n = 8) than in the patients without reinforcement by vascularized fibular graft (87.5% at 3 years and 0% at 5 years; n = 14). Huckstep nailing is a useful option for reconstruction of large bone defects in diaphyseal tumors and knee resection arthrodesis. It should be used in combination with a vascularized fibula graft to prevent mechanical failure and to achieve durability of limbs with defects from primary bone tumors. Huckstep nailing with nonbiologic augmentation is good for palliative surgery for
bone metastases
in patients with a shorter expected survival rate.

A 27-year-old male presented with a very rare metastasis to the vertebral body from a cardiac pheochromocytoma manifesting as a pathological fracture of the C-4 vertebral body that occurred while playing golf. The patient was initially treated with hard collar fixation. Gallium scintigraphy demonstrated multiple hot spots in the mediastinum, the frontal bone, the vertebral column, and the rib. Magnetic resonance imaging of the chest delineated a cardiac tumor. The patient underwent biopsies of the cardiac and the frontal bone lesions. The diagnosis was malignant cardiac pheochromocytoma with multiple
bone metastases
. Initial irradiation of the cardiac and the vertebral lesions was followed by surgical intervention to the cervical spine to prevent aggravation of the kyphotic deformity and spinal cord compression. Preoperative embolization of the feeding arteries was followed by C-4 corpectomy, iliac bone grafting, and anterior
titanium
plating fixation. The patient was discharged and returned to work. However, 20 months later, he died of a metastatic brain lesion with systemic tumor progression.

Pathologic fractures of the femur because of
bone metastases
seriously affect the quality of life of cancer patients. Different surgical options are indicated to achieve a durable and solid fixation, depending on several clinical, prognostic and mechanical factors. Locked intramedullary nailing is currently used to treat pathologic femoral fractures in patients with multiple metastases when the trochanteric region or shaft is mainly involved. This study evaluates intramedullary nailing for treatment of patients with pathologic femoral fractures, focussing on complications, clinical and functional outcomes and survival rates. The series included 80 pathologic fractures treated with a
titanium
alloy Proximal Nail (Standard or Antirotation) or Lateral Anterograde Femoral Nail (PFN, PFNA or LAFN, Synthes) inserted in a locked static mode. Acrylic cement was used to fill the bone cavity after nail insertion in eight patients; percutaneous cementoplasty was simultaneously performed in 11 cases of severe ipsilateral acetabular osteolysis. Postoperative outcomes focussed on pain relief, postoperative mobility and quality of life and they were analysed retrospectively using QOL-ACD and ECOG. Eleven patients (13.75%) suffered from non-fatal DVT post-surgery, with no pulmonary embolism. Six patients (7.5%) developed superficial wound infections and two patients (2.5%) developed pnaeumonia. There was no loss of reduction, breakage, screw pull out, or hardware or implant failure that required component substitution or revision. Lung histotype and the contemporary presence of cerebral and visceral metastases appeared to be predisposing factors in reducing survival time. All patients attained satisfactory pain relief, early deambulation and a marked clinical improvement during the first 6-10 postoperative months, with gradual worsening thereafter from deterioration of their general condition due to cancer progression. The patients' survival rate was 40% at 1 year, 25% at 2 years and 15% at 3 years. Results confirm that multiple factors related to patients and primary cancer may affect survival rate after femoral fracture. Intramedullary nailing should be indicated for pathologic fractures at femoral diaphysis and metaphysis when cancer is in an advanced stage. This procedure offers good and durable stability, and enables pain relief, early postoperative mobilisation and weight-bearing, thus improving the quality of life of cancer patients.

Radiation therapy, in conjunction with surgical implant fixation, is a common combined treatment in cases of
bone metastases
. However, metal implants generally used in orthopedic implants perturb radiation dose distributions. Carbon-Fiber Reinforced Polyetheretherketone (CFR-PEEK) material has been recently introduced for production of intramedullary nails and plates. The purpose of this work was to investigate the perturbation effects of the new CFR-PEEK screws on radiotherapy dose distributions and to evaluate these effects in comparison with traditional
titanium
screws. The investigation was performed by means of Monte Carlo (MC) simulations for a 6 MV photon beam. The project consisted of two main stages. First, a comparison of measured and MC calculated doses was performed to verify the validity of the MC simulation results for different materials. For this purpose, stainless steel,
titanium
, and CFR-PEEK plates of various thicknesses were used for attenuation and backscatter measurements in a solid water phantom. For the same setup, MC dose calculations were performed. Next, MC dose calculations for
titanium
, CFR-PEEK screws, and CFR-PEEK screws with ultrathin
titanium
coating were performed. For the plates, the results of our MC calculations for all materials were found to be in good agreement with the measurements. This indicates that the MC model can be used for calculation of dose perturbation effects caused by the screws. For the CFR-PEEK screws, the maximum dose perturbation was less than 5%, compared to more than 30% perturbation for the
titanium
screws. Ultrathin
titanium
coating had a negligible effect on the dose distribution. CFR-PEEK implants have good prospects for use in radiotherapy because of minimal dose alteration and the potential for more accurate treatment planning. This could favorably influence treatment efficiency and decrease possible over- and underdose of adjacent tissues. The use of such implants has potential clinical advantages in the treatment of
