The validity of individual radiological signs for the differentiation of osteoplastic bone metastases from osteitis deformans has been investigated quantitatively (103 skeletal metastases from carcinoma of the prostate, 45 cases of Paget's disease, two osteoplastic bone metastases from a carcinoma of rectum and bronchus). The similarities were demonstrated by three cases observed by us. Problems in the quantitative evaluation of the radiographs are discussed.

The intravenous application of 89-strontium for the relief of pain in 43 patients with breast cancer, bronchogenic cancer, carcinoma of the prostate, hypernephroma and lymphoma with generalized bone metastases is reported. A remarkable clinical improvement was achieved in 33 (76.7%) patients. In four patients a transient analgesic effect was observed. In six cases no response could be achieved. The therapeutic effect usually was long-lasting. At the same time, an increase of alkaline phosphatase was observed, which was interpreted as an indication for the stimulation of osteoblasts and osteoid peripheral zones owing to beta-emission of the radioisotope in the affected areas. There was a significant correlation between the concentration of 85Sr in the bone scan and the therapeutic result of 89Sr-therapy. The indication for such therapy and possible late adverse effects of bone-seeking isotopes are discussed.

Seventy-four patients with carcinoma of the prostate were studied annually by combined radiological and 18F scintigraphy over a 5-year period. Of the patients who have no radiological evidence of bone metastases, 25% have a positive 18F bone scan. Follow-up of these patients has shown that scan abnormalities perceded x-ray changes from between 1 to 4 years. False negative scans were not seen with 18F which allows for greater accuracy in the detection of skeletal metastases. Teh accurate staging of carcinoma of the prostate cannot be made without bone scanning. Preliminary results with 111In bleomycin as an adjunct to 18F have shown this to be a useful radio-pharmaceutical to distinguish metastases from benign lesions, and further studies are warranted.

24-hour urinary hydroxyproline excretion (THP), a marker of bone collagen metabolism, has been measured in 35 patients with carcinoma of the prostate. 21 patients had bone metastases diagnosed by bone scanning (99mTc MDP). All 9 patients with metastases studied before hormonal treatment and the majority of those on treatment had elevated levels. Patients with negative bone scans invariably had normal THP levels. Furthermore, THP reflected the presence of bone metastases more accurately than plasma alkaline or acid phosphatase. Serial THP levels altered predictably with symptomatic response to treatment. These results suggest that THP is more reliable than other markers of the presence and activity of bone metastases in response to treatment and may have been neglected in favour of more elaborate and costly X-ray and isotope investigations.

We have observed creatine kinase isoenzyme BB in the sera of nine of 19 patients with Stage D carcinoma of the prostate. Its presence does not seem to correlate with acid phosphatase activity in serum or the presence of bone metastases as indicated by increased alkaline phosphatase activity in serum. Only three of the nine patients with BB isoenzyme activity detectable in their serum had abnormal values for total creatine kinase activity, but all had abnormal values for alkaline and acid phosphatase activity. All 10 patients with no BB isoenzyme detectable had abnormal acid phosphatase values; however, only seven had increased alkaline phosphatase values and only one had increased creatine kinase activity.

155 patients with a newly discovered carcinoma of the prostate were investigated for the presence of bone metastases. Straight X-ray (conventional X-ray) revealed such secondary in 38 cases and bone marrow examination in 20. Three patients with a normal X-ray picture nevertheless showed cancer cells on bone marrow examination. The authors consider bone marrow examination as a valuable complement to the routine invedtigation of the patients with carcinoma of the prostate.

Three types of involvement of the rectum and recto-sigmoid by carcinoma of the prostate are reviewed through an analysis of eight cases. A fourth type with subserosal metastatic implant of the proximal sigmoid may occasionally be encountered. The roentgenographic findings are not pathognomonic, but are characteristic of extrinsic involvement of the bowel wall. When clinical symptoms are predominantly related to the bowel, carcinoma of the prostate is usually advanced. All patients presented with bone metastases, uretero-hydronephorsis, lack of function of one kidney, or both bone metastases and urinary tract obstruction. Rectoscopy and biopsy are helpful. However, biopsy specimens often show non-diagnostic features in secondary malignancy. Correct diagnosis is important, since there is a difference in treatment of primary carcinoma and of secondary involvement of the rectum by prostatic carcinoma. A diagnostic challenge exists if the patient is evaluated by barium enema examination for primary bowel symptoms, in particular, large bowel obstruction. At this time intravenous pyelography and bone survey for metastases may not be available to suggest the correct diagnosis. More widespread use of barium enema examinations in the evaluation of advanced carcinoma of the prostate is suggested, since the type of rectal disease shown on barium enema study was not clinically suspected in five of eight patients. The prognosis is usually unfavorable because of advanced carcinoma. Survival often does not exceed several months to one year. However, one of our patients is still well after three years of hormonal therapy.

The discovery of the new markers for the diagnosis and therapy of the carcinoma of the prostate, the prostatic acid phosphatase (PAP) and the prostatic specific antigen (PSA), and their correlation has opened new horizons in the field of the neoplastic prostatic pathology. The PAP fails have a rule in the diagnosis of a prostatic tumour in the initial phase, and therefore as a test of screening. It increases considerably only in the bone metastases. The PSA is highly specific, and together with the PAP is very useful in the check of the patients in treatment and in the surveillance of the appearance of metastases. We have considered over a period of 10 months, 70 patients with prostatic pathology. 22 were affected with carcinoma of the prostate. 48 were affected with benign prostatic hyperplasia. In both groups the simultaneous dosage of the PAP and PSA has been carried out with the RIA method. In the patients with BPH the simultaneous dosage of the 2 markers was elevated only in two cases. On the contrary, in the patients with carcinoma of the prostate we have noticed univocal data of increasing of both the markers. In the patients with metastases, the 2 markers behave in a very similar way, and progressively increase with the progressively increase with the evolution of the clinical stage of the tumour. The control after 3 months of treatment with analogues of the LH-RH and flutamide has showed a reduction of the serum levels of the 2 markers varying between 65 and 85% and over.

Prostate cancer is extremely rare in men under 35 years of age. The tumour is invariably poorly differentiated and aggressive, with rapidly growing bulky soft tissue metastases and negative tumour markers. Bone metastases develop late and are usually osteolytic. The disease responds poorly to radiation or hormonal therapy and is too advanced at presentation for radical surgery. Chemotherapy appears to be of some benefit, though in the majority of cases death occurs within a year. We describe a 31-year-old man with carcinoma of the prostate. A review of the literature is presented. This is the first patient in whom the epithelial origin of the prostate cancer was confirmed by immunoperoxidase staining with prostatic specific antigen. Plasma prostatic specific antigen was normal despite a large tumour burden and widespread metastases. He did not respond to conventional treatment. The phenotypic expression and biological behaviour of these tumours are distinct from those occurring in men beyond the fourth decade.

At approximately 11%, carcinoma of the prostate is the third leading cause of tumor-related death. The etiology of the disease remains unknown. In 1989, 9,074 deaths from this disease were recorded in the Federal Republic of Germany. With a five-year survival rate of 75%, carcinoma of the prostate is one of the cancers with a favorable prognosis. The number of new cases occurring annually in the Federal Republic of Germany (formerly West Germany) is probably between 17,000 and 19,000, or roughly 60 per 100,000 men. The (true) number of prostatic cancer patients is about fivefold, approximately 90,000. The first population-related figures for such clinical parameters as initial symptomatology, distribution of stage and the pattern of progression are now available from the Munich Tumor Registry. Another such parameter is the figure of 15% describing patients in whom bone metastases presented at the time of the diagnosis.
