That a normal spleen can rupture spontaneously has long been a point of debate. A definitive case is reported here in an attempt to resolve the issue in its favour. Spleen involved in tropical diseases like malaria, infectious mononucleosis and typhoid fever is prone to rupture spontaneously as well as in cases follow minor trauma. In contrast, normal spleen ruptures almost always following severe trauma, such as road accidents or fall from significant heights. Though cases of spontaneous rupture of normal spleens have appeared off and on in the past, doubt has always surrounded the very existence of such an entity. We are reporting a definitive case of spontaneous rupture of normal spleen. This is only the second case of its kind since 1958 and the first ever from the Indian subcontinent.

Spleen size in patients with infectious mononucleosis (IM) was measured using ultrasound, and the findings were compared with data obtained in cases of acute viral hepatitis (AVH). The size was expressed as the spleen index (SI) or the product of the longitudinal and transverse diameters, expressed in cm2, of the maximum cross-sectional area of the spleen. A normal value obtained from 28 healthy individuals was 15 +/- 7 cm2. The SI (mean +/- SD) of patients with AVH and IM were 38 +/- 7 cm2 and 88 +/- 26 cm2, respectively. When the SIs were divided into five groups--grade 0 (0-25), grade I (26-50), grade II (51-75), grade III (76-100), and grade IV (100+), 83.3% of the patients with AVH were graded as Grade 0 or I, whereas 88.9% patients with IM belonged to higher grades. Splenomegaly with a SI value over 75, grade III or IV, strongly suggests IM when IM and AVH are being considered in the absence of other potential causes of splenomegaly. Because the clinical features of IM sometimes resemble those of AVH, it is often difficult to differentiate the two entities. The measurement of spleen size could be a significant supplemental aid in the diagnosis of IM in primary medicine, before clinical data can be obtained.

The effects of experimental infection with guinea-pig herpes-like virus (GPHLV) on hematologic and histopathologic parameters were studied in Hartley strain guinea pigs inoculated intraperitoneally with 10(3) or 10(6) 50% tissue culture infectious doses of GPHLV. The total leukocyte count, the percentage of cells that were mononuclear, the number of spontaneous rosette-forming cells, and hematocrit values were determined after various intervals. Cells from spleens, cervical lymph nodes, and bone marrow of animals killed at various times were assayed for infectious virus, and tissues were prepared for histological examination. Spleen-to-body weight ratios were also determined. GPHLV infection resulted in a mild, asymptomatic, transient lymphoid hyperplasia. The infected animals experienced a relative and absolute peripheral-blood lymphocytosis lasting for seven to eight weeks. The spleens showed evidence of immune stimulation for four to six weeks. Despite the transient nature of the histological and peripheral-blood changes, virus was recovered by cocultivation from the spleen, bone marrow, and lymph nodes for up to eight months (the time of the last test). Infectivity titers were highest in the B-lymphocyte subpopulation during latent infection. These findings of GPHLV persistence after a mild, acute mononucleosis-type syndrome were compared with observations during human Epstein-Barr viral infection.

Spleen rupture has been a rare but feared complication in patients with infectious mononucleosis. There have been many cases reported during the last decade. However, it has rarely been mentioned among the young infant group. Here, we present a case of an infant, two months of age, who suffered from spleen rupture due to infectious mononucleosis, and was latter cured by himself under conservative treatment.
