A Meckel's diverticulum containing gastric mucosa was causing recurrent intestinal bleeding in a 5-year old boy. 99mTc-pertechnetate scintigraphy was used to demonstrate the heterotopic gastric mucosa preoperatively. Compared to intestinal radiography and angiography, 99mTc-pertechnetate scintigraphy is a mild and uncomplicated investigation without any risk and with a minimum of radiation exposure. Evaluating its results critically this procedure can be of great help in the preoperative diagnosis of intestinal bleedings of unknown origin.

99mTc-pertechnetate scintigraphy of the digestive tract was performed on 4 children with intestinal bleeding. Two of them showed a focal uptake in the right lower abdominal quadrant and a diagnosis of bleeding Meckel's diverticulum was confirmed by surgery. This procedure being easy to perform, non-invasive and remunerative, should be systematically applied in paediatric intestinal bleeding.

This paper reviews the presentation and surgical pathology of 50 Meckel's diverticula encountered in one hospital in a 20-year period. It is concluded that in children with intestinal bleeding or a "raspberry tumour" of the umbilicus a Meckel's diverticulum is usually suspected. In adults with an acute surgical emergency Meckel's diverticulum is unlikely to be considered. However, a Meckel's diverticulum can give rise to surgical problems in many ways and at any age. In particular, the possiblity of an attached mesodiverticular band leading to obstruction must be stressed.

Meckel's diverticula have a prevalence of approx. 2% in the population. About 90% of them are clinically inapparent. Usual complications are a perforation of ectopic gastric mucosa (possibly with intestinal bleeding), an invagination and a volvulus. Etiology is normally revealed only by surgical exploration. Calculi are rarely found in Meckel's diverticula, males being afflicted much more often than females. The enterolith consists of calcified residues of food. Unexplained abdominal symptoms with an ileus situation should also be considered a Meckel's diverticulum containing enteroliths. This is especially so, if there are unidentified calcifications visible on the abdominal X-ray.

Heterotopic gastric mucosa (HGM) may be found anywhere in the gastrointestinal tract, most often in a Meckel's diverticulum. Approximately 30 cases of HGM, located in the small bowel beyond the ligament of Treitz and not associated with a Meckel's diverticulum, have been reported. They were most often revealed by intestinal intussusception, occasionally by perforation of an intestinal ulcer or intestinal bleeding. We report a 4-year-old boy who had three attacks of acute intestinal intussusception over a 5-month period resulting in surgery. Both physical examination and barium examination of the small bowel and large intestine were found to be normal between attacks. Peroperative palpation of the small bowel was normal during the three laparotomies. During the third operation, he underwent an intraoperative endoscopy (IOE), which revealed a polypoid mass 2 cm in diameter and 0.5 cm in height, 40 cm proximal to the ileocaecal valve. Histologic examination showed HGM with fundic glands, and chief and parietal cells. This case emphasizes the interest of IOE, the main indications of which are the localization of unknown sites of gastrointestinal bleeding and the search for hamartomatous polyps of the Peutz-Jeghers syndrome for polypectomy and/or segmental resection.

Some rare causes of intestinal bleeding, where diagnosing is difficult, have been shown. Cases of Meckel's diverticulum, leiomyoma and angiomas have been reported. The considerable help which can be expected from modern diagnostic procedures has been stressed, from screening using 99 m Tc Pertechnetate in diagnosing the Meckel's diverticulum and from selective angiography in diagnosing the other pathologic changes, mainly arteriovenous malformations.

A diagnosis of Meckel's diverticulum was made by small bowel enema in a patient suffering from occult bleeding. Meckel's diverticulum is rarely seen on the standard small bowel follow-through and is unreliably detected by isotope studies in the adult. In the investigation of intestinal bleeding the diagnosis may be overlooked unless a small bowel enema is performed. It is recommended that it be performed before arteriography if colonoscopy is normal.

In the United States, four diseases account for the vast majority of cases of lower intestinal bleeding: arteriovenous malformation, diverticulosis, neoplasms, and internal hemorrhoids. In this article the authors discuss less frequent causes of gastrointestinal bleeding. "Common" less frequent causes of gastrointestinal bleeding include solitary rectal ulcer syndrome, colonic varices, mesenteric vascular insufficiency, small bowel diverticula, Meckel's diverticulum, aortoenteric fistula, vasculitis, small intestinal ulceration, endometriosis, radiation-induced injury, and intussusception. Less frequent causes of gastrointestinal bleeding that have been recently described include portal colopathy, diversion colitis, and gastrointestinal bleeding in runners.

Digestive bleeding undiagnosed by conventional endoscopies occurs in about 5% of the patients with gastro-intestinal bleeding. Vascular ectasias are the most common cause. More than half of these tumors are benign. Meckel's diverticulum is the cause of bleeding in two thirds of men under the age of 30. Hemorrhage from the biliary tree or pancreatic duct occurs more often in patients with a suggestive history. According to the abundance and the cause of hemorrhage numerous investigations can be undertaken and the diagnostic value of new endoscopic methods (enteroscopy, endoscopic ultrasonography) must be assessed.
