Neoplasia is the least common complication of Meckel's diverticulum. A case of partial obstruction of the small intestine due to adenocarcinoma arising in a Meckel's diverticulum is presented. The noteworthy feature of this case is the presence of neoplastic Paneth cells in the tumor.

Pentagastrin, histalog, glucagon, secretin, and perchlorate were intravenously injected into mice in order to investigate the hormonal regulation of 99mTc pertechnetate uptake by gastric mucosa. Pentagastrin significantly increased uptake; the effect of histalog was weaker. Secretin did not alter the uptake. Glucagon produced some diminution, and perchlorate significantly inhibited gastric uptake. Hormones may play a role in improving the sensitivity and specificity of diagnostic tests for Meckel's diverticulum.

A case is reported of multiple calculi occurring in a Meckel's diverticulum which caused chronic abdominal pain in a 48-year-old man. The problem was diagnosed before operation by radiological studies. The features of this condition are well illustrated in this patient. The published literature is reviewed.

A case of Meckel's bleeding diverticulum diagnosed by emergent arteriography is presented. Arteriography revealed an extravasation of contrast medium into the distal ileum and an abnormal ileal artery which passed to the antimesenteric border of the ileum. The diverticulum was demonstrated by double contrast study of the small intestine. The merits of emergent arteriography and other examinations for diagnosing Meckel's diverticulum are discussed.

The noninvasive advantage of this method justifies its early use as a diagnostic measure when a Meckel's diverticulum is suspected in the differential diagnosis of lower gastrointestinal bleeding.

A 22-year-old man with burning pain in the lower abdomen and rectal bleeding had a sodium pertechnetate Tc 99m scan. The scan showed an area of uptake in the right lower quadrant which at operation was found to be a Meckel's diverticulum. The ability of ectopic gastric mucosa within a Meckel's diverticulum to concentrate sodium pertechnetate Tc 99 m allows for this noninvasive diagnosis of some types of rectal bleeding, especially when the cause has escaped detection by conventional endoscopic and barium contrast examinations.

The literature data concerning the incidence of Meckel's diverticulum in the adult and the frequency of its complications are examined and problems associated with the preoperative diagnosis of this form are discussed. Reference is made to the limitations of abdominal scintiscanning and the encouraging results obtained with superior mesenteric arteriography in diverticular bleeding. Five cases of Meckel's diverticulum in the adult are presented. Stress is placed on the need to explore the ileum during appendicectomy to enable diverticula to be detected and removed, since they are often the cause or the site of complications, even in the adult.

A case of vascular ectasia of the cecum is presented. The isotopic pattern did not differ from that of Meckel's diverticulum. The authors suggest that studies with 99mTc may be helpful in screening for areas of pathology in cases of lower gastrointestinal bleeding.

Complementary radiologic examinations are needed in the visualization of Meckel's diverticulum due to difficulties in making the correct diagnosis.

A total of 600 cases, comprising 4 of our cases and 596 others in Japan, was analyzed, and the diagnosis of Meckel's diverticulum was made in only 34 of them. The rate of diagnosis was 5.7 per cent, which is very low. The most common complication of this disease was intestinal obstruction, the second most common was intussusception, the third was inflammation, and the fourth was rectal bleeding. Ectopic tissue, present in ninety-three patients, consisted of ectopic gastric mucosa in fifty-eight. 99mTc-P scanning was performed on fifteen patients. In twelve of them ectopic gastric mucosa was observed. However, it was not found in three, all of whom had false-positive scans.
