By means of 30 pneumoperitoneograms performed "postmortem" and five performed for diagnosis, the exploratory technique, radiologic anatomy of umbilical region, and the usefulness of the process in the diagnosis in children are discussed. Pneumoperitoneum may be indicated in diagnosis of bleeding Meckel's diverticulum, in exclusion or confirmation of remnants of the omphalomesenteric duct in cronically moist lesions of the umbilicus resistent to symptomatic treatment, in cases of non-communicating urachal cysts which can not be diagnosed by cystogram, and in the differential diagnosis of abdominal tumors related to the umbilical region.

The presentation of a rare case of leiomyoma of Meckel's diverticulum in a boy of 5 yr forms the starting point for a discussion of such cancers. In adult age, the complication is not so rare as the sporadic publications would suggest, incidence of tumoral forms being around 1 : 50 M.D. with a high malignancy frequency. In paediatric age, on the other hand, the tumoral degeneration of a M.D. is an exceptional occurrence: a review of more than 1,200 reported cases and personal experience (48 cases of M.D. in 15 yrs) confirm the rarity of the observation. The need to seek M.D. on each occasion and remove it to prevent the risk of tumoral degeneration in adult age is stressed. M.D. surgical removal tecniques are commented on and emphasis laid on the desirability of ample intestinal resection in cases of cancer of the diverticulum.

A case is described in which the preoperative diagnosis of Meckel's diverticulum was made by scintigraphy after the administration of pentagastrin. Pentagastrin premedication may be helpful in demonstrating ectopic gastric mucosa in patients with gastrointestinal bleeding in whom initial radiopertechnetate scintigraphy is equivocal or normal.

99mTc-pertechnetate scanning is now recognized as a useful means of detecting Meckel's diverticula. When these contain gastric mucosa and present with ulceration and bleeding, the ectopic gastric mucosa can be identified by its secretion of 99mTc-pertechnetate. However, the secretions of normal gastric and salivary glands can pool in the gut and simulate a Meckel's diverticulum. We believe the diagnostic procedure of choice to include both continuous nasogastric suction and continuous sequential gamma camera images. To support this view, we present cases illustrating 1) that 99mTc-pertechnetate can appear in the bowel within the first few minutes of administering 99mTc-pertechnetate, even in the absence of a Meckel's diverticulum; 2) that rapid sequential camera views are helpful but not definitive in distinguishing such bowel activity from a Meckel's diverticulum; and 3) that continuous nasogastric suction most successfully eliminates this interference.

In twenty years, 66 infants and children with remnants of vitelline duct requiring surgery have been admitted. The patients were classified into three groups: patient vitelline duct (20 cases); Meckel's diverticulum as the primary surgical diseases (19 patients); and Meckel's diverticulum found incidentally at surgery (27 patients). The male preponderance in the groups of patent vitelline duct and symptomatic Meckel's diverticulum was 9:1. In contrast with other data, Meckel's diverticulum requiring surgery occurred with nearly equal frequency up to fourteen years. The gravest complication in the cases of patent vitelline duct were a T-shaped protrusion of ileum and a small bowel volvulus around the fibrous cord or the patent duct; and in the cases of Meckel's diverticulum causing symptoms, intestinal obstruction, bleeding peptic ulceration or inflammation. Three deaths occurred in newborn age in connection with patent vitelline duct, and one patient died who belonged to the group of asymptomatic Meckel's diverticulum.

Administration of cimetidine prior to pertechnetate imaging in two patients with Meckel's diverticulum resulted in demonstrating the abnormality with unusual clarity. This is presumed due to the cimetidine-mediated retention of pertechnetate in the gastric mucosa. Pertechnetate imaging in two normal subjects with and without cimetidine administration disclosed a marked difference in retention of the radionuclide in the gastric mucosa and its subsequent transit into the intestinal tract. Pertechnetate imaging in conjunction with cimetidine administration appears to result in markedly improved identification of Meckel's diverticulum.

Two cases of axial volvulus of a Meckel's diverticulum associated with a diverticular band are described. In both cases, the pre-operative diagnosis was of acute appendicitis. The importance of further exploration if the appendix is insufficiently inflamed to acount for the symptoms and signs is emphasized.

We present a 14 year old girl with a two years' history of colicky abdominal pain associated with the palpation of a tumor in the left upper quadrant of the abdomen. During these two years, the pain and the tumor appeared and disappeared spontaneously several times. In the operation we found a jejunojejunal intussusception, the head being a sessile polyp placed 20 cm from the ligament of Treitz. The pathological examination showed a polyp formed by mucosa similar to the gastric one with chief and parietal cells. We discuss the clinical pictures that can be associated with this pathological entity in this uncommon localization in opposition to the more common settling in Meckel's diverticulum.

The authors underline the value of selective superior mesenteric arteriography in the diagnosis of Meckel's diverticulum. In the ten cases reported, seven were found to have a Meckel's diverticulum on operation, while it was absent in the other three. Arteriography was able to diagnose its presence correctly in sex patients, gave a false positive diagnosis in three cases, and was considered to be normal in the last case in which a Meckel's diverticulum was present. Valid radiological signs are extravasation of the contrast medium into the lumen of the digestive tract and the presence of a vitellin artery dividing into a vascular network. This paper describes our experience with pre-surgical diagnosis of Meckel's diverticulum. We used a naso-jejunal-ileal tube, through which, we injected small amounts of radio-opaque fluid and air, thus utilizing double contrast. In seven children diverticulum was shown to be present by radioscopic and radiographic techniques; this was later confirmed during laparotomy. X-ray findings were compared with Tc99 scanning results.
