The presentation of a rare case of leiomyoma of
Meckel's diverticulum
in a boy of 5 yr forms the starting point for a discussion of such cancers. In adult age, the complication is not so rare as the sporadic publications would suggest, incidence of tumoral forms being around 1 : 50 M.D. with a high
malignancy
frequency. In paediatric age, on the other hand, the tumoral degeneration of a M.D. is an exceptional occurrence: a review of more than 1,200 reported cases and personal experience (48 cases of M.D. in 15 yrs) confirm the rarity of the observation. The need to seek M.D. on each occasion and remove it to prevent the risk of tumoral degeneration in adult age is stressed. M.D. surgical removal tecniques are commented on and emphasis laid on the desirability of ample intestinal resection in cases of
cancer
of the diverticulum.

Examination of the records of 378 children with intussusception at our institution revealed that 29 cases were caused by an identifiable intestinal lesion. A
Meckel's diverticulum
was the causative agent in 21 children, all of whom were under 2 yr of age. A previously undiagnosed ileal lymphosarcoma produced the intussusception in six other children, all between 6 1/2 and 9 yr of age. Our experience indicates that any child over 6 yr of age with the clinical findings of colicky abdominal pain, bloody stools, and a palpable mass plus the radiographic evidence of intussusception must be considered to have ileal lymphosarcoma until proven otherwise. Hydrostatic reduction of the intussusception must be accompanied by extensive small bowel reflux of barium in order to effectively rule out a small intestinal lesion. If this is not accomplished, surgery should be planned with the suspicion that a
malignancy
may be present. If this suspicion is confirmed by frozen section, the operation procedure should include wide surgical excision of the lesion along with the regional lymph nodes.

Thirty-one patients presented with simultaneous lymph-node metastases and 27 with distant metastases. Based on these data, a metastases staging system (four stages) has been defined for study stratification. This work also aims at evaluating the correlation of tumour diameter and tumour tissue invasion with primary metastases occurrence; primary tumour diameter remains the most important prognosis-related factor for appendiceal location. For other locations, tumour tissue invasion can be markedly better correlated with regional lymphatic and distant spread than is the case with tumour diameter. Tumour tissue invasion has, therefore, been selected to define a local staging (four stages) for further study stratification. Tumour location, metastases stage and local stage combined, best describe the current state of the disease and will serve to evaluate further pathological stratification and to correlate survival data.

Sixteen cases of
Meckel's diverticulum
are reported, 14 of them detected during emergency operative intervention for acute surgical abdomen and 2 during planned operation. The complicated forms of
Meckel's diverticulum
were 10: acute diverticulitis 4 (25 per cent), intestinal obstruction 4 (25 per cent), neoplastic process 1 (6.25 per cent), intestinal hemorrhage 1 (6.25 per cent). Symptom-free were 43.75 per cent of the diverticuli. The clinical symptoms of acute diverticulitis resembled much those of acute appendicitis. The method of choice for its surgical management is cuneiform resection of the intestinal wall at the site of the diverticulum and transverse reconstruction of the intestinal lumen.
Cancer
of the diverticulum requires extended intestinal resection.

Carcinoid tumor arising in a
Meckel's diverticulum
is a rare lesion with only 52 cases described in the literature. Most of these tumors are found incidentally at autopsy or during an operation performed for an unrelated reason. The lesion can, however, in about one-third of cases, be the cause of symptoms necessitating surgical intervention; the diagnosis is nearly never apparent till the specimen has been removed. When the signs do not match the symptoms, consideration of uncommon conditions such as this may help in making a correct preoperative diagnosis. Resection of the intestinal segment containing the diverticulum along with its mesentery to the level of the superior mesenteric vessels usually results in cure; however, metastatic disease and even carcinoid syndrome may still occur. The authors report a case of a
Meckel's diverticulum
mesenchymal tumor incidentally discovered in a 69-year-old woman undergoing laparotomy for an ovarian cyst. Histological examination of the tumor revealed a fasciculated proliferation of spindle cells that proved to be immunoreactive to vimentin only; pleomorphism was mild, necrosis extensive and mitotic count high. Because these features were consistent with those of fibrosarcoma, the tumor was classified in the category of gastrointestinal stromal tumors; more specifically, it was considered malignant for the presence of spread beyond the primary site in the form of peritoneal nodules and for the high mitotic count. We have been able to find only one previously reported case of mesenchymal
