This is a case of an 85-year-old woman whom was admitted with otalgia and an abducens nerve palsy alongside a Pseudomonas otitis externa; she was presumed to have malignant otitis externa. However, despite optimum treatment and resolution of her otitis externa, she went on to develop an ipsilateral facial nerve palsy and sensorineural hearing loss. After further investigation, it was discovered that varicella-zoster
meningitis
was causing her
polyneuropathy
. She eventually responded to antivirals and steroids and, at follow-up, her sixth and seventh cranial nerve palsies had completely resolved, though a hearing deficit remained. This case highlights the importance of keeping a diagnosis under review, with the help of the multidisciplinary team, when the clinical course is not progressing as expected.

Angiostrongyliasis is the most common cause of eosinophilic
meningitis
worldwide and is primarily characterized by eosinophilic
meningitis
, meningoencephalitis, or myelitis. It is caused by ingestion of the nematode Angiostrongylus cantonensis, the rat lungworm (or apple snail). The most common route of infection is by ingestion of parts of the intermediate hosts like mollusks or food contaminated with infective third stage larvae. Following ingestion, the larvae migrate through the intestinal walls into the bloodstream and further into the central nervous system where they cause meningo-encephalo-myelitis and can have a variety of ocular manifestations. We present a case of a Caucasian United States Marine who suffered severe meningo-encephalo-myelitis with papilledema following ingestion of a raw Giant African Snail (Lissachatina lutica) while stationed in Japan. He developed eosinophilic meningoencephalitis,
polyneuropathy
, motor weakness, and papilledema. We describe the unique clinical features of this disease in our patient.

Tuberculosis being a global pandemic causes an array of neurological presentations ranging from tuberculoma,
meningitis
, radiculomyelitis, brain abscess, and so on.Association of Guillain-Barre syndrome with tuberculosis has been reported five to six times in the past. The authors report a case of a young female with tubercular
meningitis
on antitubercular therapy and steroids who went on to develop acute areflexic quadriparesis and diagnosed as a case of acute motor sensory axonal neuropathy variety of acute inflammatory demyelinating
polyneuropathy
(AIDP) who responded positively to plasmapheresis. The authors present the first case of an association between tubercular
meningitis
and subsequent development of AIDP.

Neurosarcoidosis when encountered by neurologists most commonly presents as cranial neuropathy, peripheral mononeuropathy,
polyneuropathy
, myopathy,
meningitis
or myelopathy. There are limited reports in the current literature on the cases of neurosarcoidosis patients presenting with ischaemic stroke. We discuss a 52-year-old patient with a known previous history of cutaneous sarcoidosis presenting with an acute third nerve palsy, facial weakness and ataxia. His magnetic resonance imaging (MRI) brain demonstrated focal signal changes in the midbrain consistent with an acute ischaemic event in the region of his third nucleus, suggesting a partial Claude syndrome presentation. Cerebrospinal fluid (CSF) examination demonstrated an elevated angiotensin-converting enzyme (ACE) level. We discuss the difficulties associated with confirming a diagnosis for his presentation and consider distinctions in stroke in neurosarcoid and its management in comparison to more common causes.

Guillain-Barre syndrome (GBS) is a life-threatening immune-mediated acute inflammatory
polyneuropathy
and is associated with various antecedent infections. Its association with tuberculosis is very uncommon with only a handful of cases being reported in the literature. It's association with tuberculous
meningitis
is even more scarce with only one case reported in literature till date. We report a 40-year-old lady with GBS associated with tuberculous
meningitis
. GBS was confirmed with clinical examination and nerve conduction studies.
