In 1959 a patient then aged 66 underwent castration for a small nodule in the prostate. Polyestradiol phosphate (Estradurin), 20 ml every 2 weeks, was given until 1975 when urinary tract abstruction required transurethral resection. The resected tissue showed malignant change. At that time metastases were not demonstrable. Extrogen therapy (Tace) was again given. The nodule had considerably increased in size. After 6 months of this therapy the patient began using Estradurin, 40 ml every 2 weeks. The question was asked regarding increasing the dose or using irradiation. The replying urologist stated that, if the nodule 1st found in 1959 was shown by biopsy to be a carcinoma, a radical prostatectomy or radiotherapy was then indicated to attempt a cure as castration or estrogen therapy is only palliative. Now a change to diethylstilbestrol, 1 mg orrally 3 times a day, is advised. Irradiation is not recommended unless bone pain or urinary obstruction occur.
