There is as yet no adequate animal mode for human
myocardial ischemia
. The commonly utilized technique of coronary arterial ligation in large animals may induce regional ischemia but introduces variables that make it difficult to compare studies in different laboratories. A model of global ischemia in an isolated perfused rat heart that offers a rapid, inexpensive means for producing graded, controlled, stable state and reproducible ischemia is described. The technique has been utilized with success to study the hemodynamic and metabolic effects of ischemia and to evaluate pharmacologic interventions designed to protect the ischemic myocardium. Propranolol has been shown to improve bioenergetics and reduce anaerobic glycolysis by a depression of the hemodynamic response of ischemic myocardium.
Methylprednisolone
appears to exert its primary effect by direct coronary vasodilation, increasing resting or control flow and providing an enhanced reserve when ischemia is imposed. Mannitol improves cardiac performance by reducing the increased myocardial cell water content induced by hypoxia or anoxia.

Echoventriculography, a multiaxis M-mode echocardiographic technique, was developed to examine in detail the regional wall motions of the left ventricle. The basic technical aspects and limitations are described, and experience is reviewed on 263 healthy subjects or patients with
ischaemic heart disease
. The reliability in detecting site and size of asynergic segments was excellent as related to electrocardiographic and thallium scintigraphic sites of acute infarction, and with left ventricular cineangiograms in chronic coronary heart disease. The correlation with pathologic anatomic size of infarct in 24 consecutive patients was r = 0.88 (p less than 0.001) when expressed by a percentage of the left ventricular horizontal circumference. 94% of 111 infarcted segments were correctly detected by echo; only the posteroseptal and the most lateral regions remain out of the methodological range. The method separated old infarct scars from fresh necrosis. Decreasing echo contraction index correlated with increasing severity of coronary obstructions in 43 patients studied for coronary artery surgery. In 15 infarct patients the M-mode technique was more sensitive than two-dimensional echocardiography in recording asynergic segments or endocardial echoes. The multiple segmental echoventriculographic index decreased parallel with clinical severity of acute infarction (r = -0.79, p less than 0.001; 30 patients). There was a 88% (p less than 0.01) concordance between the reduction of the ST segments (-30%) and the recovery of the mechanical function in the ischaemic myocardial segments (+26%) after beta blockade with pindolol in 22 patients with acute infarction.
Methylprednisolone
showed no improvement. With dopamine the left ventricular size decreased markedly (p less than 0.0005). Echoventriculography thus seems to be very informative in evaluation of chronic or acute left ventricular dysfunction, despite the rather demanding nature of the technique in practice.

An in vitro model to study myocardial cell injury was developed with primary monolayer cultures of rat myocardial cells. Two important conditions associated with
myocardial ischemia
were simulated by depriving the cultures of oxygen and glucose for a specified period of time. Cellular injury caused by hypoxia and glucose deprivation resulted in significant leakage of lactate dehydrogenase (LDH) from the cells into the culture medium. The cells were not lethally injured by treatments as reflected by a lack of change in cell viability and protein content when compared to controls. Pretreatment of cultures with methylprednisolone for 24 hr provided protection to the cells when challenged by hypoxia and glucose deprivation.
Methylprednisolone
exhibited a dose-response effect in reducing LDH leakage in cultures, which were subsequently deprived of oxygen and glucose for 4 hr. Similar pretreatment with hydrocortisone had no effect in limiting cellular injury in hypoxic and glucose-deprived cultures.

. 58% of the patients survive after transplantation, the mortality rate is 42%. The highest mortality is in the early postoperative period (within two weeks after operation)--21 cases (50%). The longest survival period is nine years and six months. All patients have cyclosporin immunosuppressive treatment (in combination with another drug, later with another two drugs). In the authors' group in particular incipient acute rejections are encountered, "mild" rejections are less frequent and "moderate" rejections least frequent. The patients do not reach the stage of advanced acute rejection, i.e. "severe" rejection. Acute rejections are treated as a rule with 3 g

Degradation of troponin I (TnI) by calpain occurs with myocardial stunning in ischemia-reperfusion injury. Glucocorticoids attenuate
myocardial ischemia
-reperfusion injury, but their effect on TnI degradation is unknown. A piglet model was used to test the hypotheses that cardiopulmonary bypass (CPB) and deep hypothermic circulatory arrest (DHCA) are associated with TnI degradation and that TnI alterations could be prevented by glucocorticoid treatment. Piglets were cooled to 18 degrees C, subjected to 2 h of circulatory arrest, rewarmed to 37 degrees C, and allowed to recover for 2 h.
Methylprednisolone
was administered 6 h before surgery (3 0 mg/kg) and at initiation of CPB (30 mg/kg). The untreated group received saline. Left ventricular tissue was collected after recovery and analyzed by Western blot for TnI, calpain, and calpastatin (the natural inhibitor of calpain). CPB/DHCA animals had 27.4 +/- 0.2% of total detected TnI present in degraded form. Glucocorticoid treatment significantly decreased the percentage of degraded TnI (12.0 +/- 0.1%, p < 0.05). Calpain I and calpain II increased after CPB/DHCA compared with non-CPB/DHCA controls (p < 0.05), with or without glucocorticoid treatment. Calpastatin significantly decreased in untreated CPB/DHCA animals compared with non-CPB/DHCA controls (p < 0.05), but levels were preserved by glucocorticoids. Glucocorticoids were associated with preservation of maximum rate of increase of left ventricular pressure at 95 +/- 10% of baseline, whereas maximum rate of increase of left ventricular pressure decreased to 62 +/- 12% of baseline without steroids. TnI degradation occurs after CPB/DHCA in neonatal pigs. Reduction in reperfusion injury by glucocorticoids may depend partly on preservation of calpastatin activity and intact TnI.
