Algesimetry was used to study the pain sensitivity in 143 subjects (21 healthy individuals, 20 cases with neurocirculatory dystonia, 62 with chronic ischaemic heart disease and 40 with subacute myocardial infarction). The tests were performed for the tactile threshold, pain threshold, threshold of pain endurance and the interval of pain endurance. In a part of patients indices of pain sensitivity were correlated with tolerance to physical exertion. In neurocirculatory dystonia high pain sensitivity was due to the lowering of the pain threshold. In patients with ischaemic heart disease there were three types of pain threshold (low, medium, high) and two variants as concerns the interval of pain endurance (low and high). Low interval of pain endurance corresponded to low tolerance of physical exertion, hence algesimetry can serve as an additional method of objective interpretation of the bicycle ergometry tests.

As many as 20 patients with neurocirculatory dystonia (NCD) and 10 IHD patients presenting with stable exertional angina were evaluated for an effectiveness of antianginal action of validol tablets commercially- and noncommercially produced, the above tablets being of the changed composition in the latter case. Validol of both changed and unchanged composition had a similar transient antianginal effect which was higher in NCD than it was in angina pectoris. Economical as well as clinical effects of validol of the changed make up warrant it to be commercially produced.

Erythrocyte phospholipids (FL) composition and their elements polyunsaturated fatty acids of omega-6 and omega-3 type were studied in the course of combined balneotherapy of neurocirculatory dystonia and ischemic heart disease (IHD) patients. The balneological factors promoted FL concentration normalization under positive dynamics in clinical symptoms in 62% of patients with neurocirculatory dystonia. The modulating effect occurred in IHD only in those patients who, in addition to balneotherapy, took alimentary omega-3 fatty acids made of sea products as food additives. The intensity of the FL and fatty acid modification to the end of the treatment was related to the severity of the baseline metabolic abnormalities.

Patients with vitamin B12 deficiency have protean neurological manifestations that are often insidious. Acute onset of cerebellar dysfunction and extrapyramidal manifestations like dystonia and chorea are rather uncommon in adults. We describe a patient who manifested with acute onset of language dysfunction, chorea and ataxia. There was no history of hypertension, diabetes or ischemic heart disease. He had low serum vitamin B12 and elevated serum homocystine levels. He improved dramatically following B12 replacement therapy. Our patient provides insight into the pathophysiological mechanism of this rare manifestation. Further the importance of considering vitamin B12 deficiency, in country like India, where vegetarian food practice is quite common, is being emphasized.

Comparative study of pathomorphology of myocardial circulation under conditions of increased afterload of the left or right ventricles showed similar changes. All compartments of the coronary bed were plethoric, capillary blood stasis and perivascular edema, more pronounced in arterial vessels, were detected in both cases. These changes equally involved both ventricles and the ventricular septum. Significant differences consisted in local increase in the density of functioning capillaries. The increase was the maximum in hemodynamically overloaded ventricle and ventricular septum, presumably due to increase of their contractile activity. The density of functioning capillaries in the intact (vs. pressure overloaded) ventricle also increased, but to a lesser degree, which could be due to systemic neurohumoral effects. If increased afterload was complicated by the development of heart failure, circulatory disorders in the myocardium progressed. Significant increase in the density of functioning capillaries in all cardiac compartments indicated decreased vascular tone and exhaustion of coronary reserve. This was paralleled by a sharp arterial plethora in case of increased afterload of the left ventricle and sharp blood stasis in the microcirculatory bed in case of increased right ventricle afterload. Reduction of effective perfusion pressure in the presence of coronary dystonia can cause coronary insufficiency and myocardial ischemia in case of increased right ventricle afterload.

This review presents the data of assessing antihypertensive efficacy and tolerability vasoselective high-lipophilic the 3d generations calcium channel blocker lercanidpine. The inhibition of the calcium ions flow through the membranes of smooth muscle cells of blood vessels causes peripheral, cerebral, renal and coronary vasodilation decreasing total peripheral vascular resistance and, consequently, blood pressure (BP) lowering and improve regional circulation. During reception of lercanidipine the level of norepinephrine remains the same even when using high doses of the drug. Negative inotropic effect does not occur therefore, lercanidipine can be used in the treatment of myocardial ischemia. Renal protection properties slow down the development and progression ofchronic renalfailure (CRF). The drug can be successfully used in patients with arterial hypertension, chronic renalfailure, diabetic and non-diabetic nephropathy. Lercanidpine also may be effectively used in the treatment of hypertension with associated clinical conditions: bronchial asthma, chronic obstructive pulmonary disease, bradiarrythmias, atrioventricular blockade 2-3 degree, sinus node dysfunction, peripheral arteries deseases with symptoms of the extremities ischemia, sleep disturbance, depression, dystonia, asthenic and cephalgic syndme in the frame of the cerebrovascular insufficiency manifestations. Therapy with lercanidpine, in addition to lowering blood pressure, can help to nephroprotection, neuroprotection, antianginal effect, the regression of left ventricular hypertrophy, improvement of lipid metabolism and glucose tolerance. With over 30 years experience in the application and modification of the molecular structure, slow the onset of action and superior long-lasting effect reception of letranidipine well-tolerated and provides a high adherence ofpatients to the treatment of hypertension.
