Scintigraphy with pyrophosphate 99mTc was performed in 230 patients with various forms of ischemic heart disease and in 15 persons with pain in the region of the heart caused by osteochondrosis of the cervical and thoracic spinal segments or vegetovascular dystonia (control group). It was found that labelled purophosphate accumulated in the myocardium in necrosis of the heart muscle or when coronary insufficiency takes a course in which necrosis of the myocardial cells is quite possible. Positive results of scintigraphy with pyrophosphate 99mTc are not a strict criterion of acute myocardial infarction because they are also encountered in a chronic course of ischemic heart disease and are evidence in this case that "a state of risk" has occurred during the disease.

The results of tests with physical load on a bicycle ergometer and treadmill in 50 patients with ischemic heart disease were compared. The control group was formed of 20 patients with neurocirculatory dystonia. Agreement between the results of both tests was encountered in 90% of patients with ischemic heart disease. The authors conclude that the tests do not differ essentially in the main indices (rate of cardiac contractions, index of cardiac activity at the peak of the load, time of uninterrupted work until the criteria of load cessation). It is suggested that tests on the treadmill and those on the bicycle ergometer in patients with ischemic heart disease are equivalent.

The rheologic properties of blood were studied in 40 patients with ischemic heart disease both with affected and with intact cardiac coronary arteries (according to the findings of angiography) and in 13 persons with cardialgia due to vegetovascular dystonia. Significant hemorheologic pathology according to all values was revealed in patients with ischemic heart disease. It was noted that the growth of the fluidity threshold depends on the developing pathologic erythrocyte aggregation which is not associated with changes in the concentration of plasma fibrinogen. The importance of disorders in blood rheology in the origin of angina pectoris and myocardial infarction is discussed.

The study was undertaken to examine 35 patients with chronic coronary heart disease and 20 patients with neurocirculatory dystonia concurrent with the cardialgic syndrome, the latter were enrolled into a control group. Detection of sigma R in the standard leads of resting ECG and at the peak of atrial pacing made it possible to single out groups of chronic coronary heart disease patients, which differed in clinical manifestations of the disease, myocardial contractility, hemodynamics, and myocardial lactate extraction. In patients with chronic coronary heart disease ti is recommended to use ECG measurements (sigma R and nST) in the standard leads during loading tests to give a comprehensive characterization of the pump function and the severity of myocardial ischemia, the prognosis of the natural history of the disease and risk for myocardial infarction. To enhance the sensitivity and specificity of loading tests, it is recommended to take into account higher sigma R in combination with ST segment depression when the results of the tests are assessed.

The authors studied the functional activity of thrombocytes, levels of plasma and thrombocytic cyclic AMP (cAMP) in the dynamics of a veloergometric test in patients with IHD and neurocirculatory dystonia. The revealed signs of hyperactivation of thrombocytes and absence of compensatory response of cAMP in IHD patients against the background of thresholds loads allows to improve the quality of prediction of thrombotic complications and control of treatment efficacy with agents possessing antithrombocytic activity.

A seasonal character was established for some of these diseases--gastric and duodenal ulcer, chronic gastritis, ischemic heart disease, arterial hypertension, neurocirculatory dystonia of the hypertensive type. It is recommended to use these results for chronoprophylaxis of the mentioned pathological conditions.

Variations in arterial and venous blood prostacyclin and thromboxane were examined in 32 patients with coronary heart disease, neurocirculatory dystonia and the cardialgic syndrome, exposed to cardioselective stress as a result of the atrial stimulation test. Prostacyclin and thromboxane measurement in blood specimens obtained from the left ventricle and the coronary venous network may provide additional markers of myocardial ischemia in cases where diagnosis is otherwise difficult to make.

In patients with ischemic heart disease (IHD), neurocirculatory dystonia (NCD) and acute myocardial infarction (AMI), we observed significant, positive, linear correlations between total concentration of non-etherified fatty acids (NEFA) as well as between the percentage of individual fatty acids in arterial and coronary sinus blood. Correlation analysis of the results demonstrated that in patients with IHD and NCD there is significant myocardial NEFA extraction which is highly reduced by electrically induced myocardial ischemia in patients with IHD and by increased myocardial load in patients with NCD. These changes are more evident in patients with IHD. Patients with AMI exhibited minor myocardial NEFA extraction, similar to that in patients with IHD after stimulation, but much higher NEFA concentration in arterial and coronary sinus blood. Determination of NEFA levels in arterial and coronary sinus blood in patients with AMI and their temporal dynamics may be helpful for evaluation of the degree of the disease severity and can also be of prognostic value.

The authors examined 29 patients with ischemic heart disease and 3 patients with neurocirculatory dystonia. According to the results of echocardiography it was found that dilatation of the left ventricle and decrease in its contractility were more pronounced when the anginous attack was attended by a rise of the ST segment on the ECG than when it was attended by depression of the ST segment. During an ergometry-induced anginose attack with no changes on the ECG the diastolic filling of the left ventricle was disturbed but its contractility did not change, which evidently points to early manifestations of ischemic changes in the myocardium.
