In patients with
ischemic heart disease
(
IHD
), neurocirculatory dystonia (NCD) and acute myocardial infarction (AMI), we observed significant, positive, linear correlations between total concentration of non-etherified fatty acids (NEFA) as well as between the percentage of individual fatty acids in arterial and coronary sinus blood. Correlation analysis of the results demonstrated that in patients with
IHD
and NCD there is significant myocardial NEFA extraction which is highly reduced by electrically induced
myocardial ischemia
in patients with
IHD
and by increased myocardial load in patients with NCD. These changes are more evident in patients with
IHD
. Patients with AMI exhibited minor myocardial NEFA extraction, similar to that in patients with
IHD
after stimulation, but much higher NEFA concentration in arterial and coronary sinus blood. Determination of NEFA levels in arterial and coronary sinus blood in patients with AMI and their temporal dynamics may be helpful for evaluation of the degree of the disease severity and can also be of prognostic value.

Results of 408 cases of anesthesia in patients with
IHD
, arterial hypertension, disturbances of heart rate and conduction are analysed. Medical treatment up to the day of operation, use of anesthetics which have moderate depressive effect on the myocardial contractility and work, administration of vasodilators and beta-blockers made it possible to avoid severe disturbances of hemodynamics during the perioperative period. Perioperative myocardial infarction was observed in 5% cases of myocardial revascularization and in 0.3% of all the other operations.

Ten patients with
ischemic heart disease
and stable angina of effort have been examined for the possibility of tolerance development to verapamil antianginal effect, which was assessed using bicycle ergometry. Antianginal effect after the course of treatment with 80 mg of verapamil 3-4 times per day, and 120 mg of verapamil 3-4 times per day, was significantly more pronounced than after single administration of 80 and 120 mg of the drug. Thus, no tolerance to verapamil antianginal effect was observed during the course of treatment with verapamil, moreover the effect was enhanced.

A total of 27 patients were examined: 12 of them had non-coronarogenic cardialgias and 15 had
ischemic heart disease
. Prostaglandin E2 intravenous infusion followed the coronary angiography and ventriculography against the background of stabilized pressure in the left ventricle. The infusion lasted 10 minutes, then ventriculography was repeated. The total dose of prostaglandin E2 was 0.5-1.5 mg. Prostaglandin E2 had positive chronotropic and inotropic effects, as well as hypotensive and venodilatative ones. Stroke volume was decreased in most observations. Prostaglandin E2 eliminated the attacks of cardiac asthma and angina pectoris, caused by the increase of the left ventricle cavity.
