In serums of patients with congestive cardiomyopathy (CCM), patients with
ischemic heart disease
(
IHD
) and those of healthy subjects, presence of antibodies to capsid and early antigens of Epstein-Barr's virus (EBV) and to various structural components of myocardium was determined in order to study seroepidemiologic relations between the infection and CCM as well as immunopathogenic reactions against myocardium in this disease. Seroepidemiologic relation between EBV and CCM consisted in significant (compared to healthy subjects and patients with
IHD
) increase in average geometrical titres of antibodies to virus-associated antigens. Half of the patients with CCM had active EBV infection proved by presence of antibodies to early EBV antigen. Antibodies to various structural components of myocardium were found, including those to myocardial nervous fibers (for the first time) in 1/3 of the patients. Etiological role of active EBV infection in development of lymphomas during the post heart transplantation period in CCM as well as in myocardial nervous fibers damage in this disease is also discussed.

27 patients with IIB stage of essential hypertension (EH) and left ventricular hyperthropy (LVH), 14 of them with
IHD
, were studied. In all of them coronaro- and ventriculography was performed. In 14 cases atherosclerotic alterations of coronary arteries were present. Nobody had abnormal pump function or contractility of myocardium. End-systolic, end-diastolic and stroke volume indexes in EH patients with pronounced coronary atherosclerosis were significantly increased. The examined patients demonstrated significant alteration of the left ventricular myocardium relaxation function. The conclusion is drawn that on a certain stage LVH is of compensatory character and plays an important role in supporting adequate myocardial pump function.

The study was performed in 22 patients with
ischemic heart disease
who underwent direct myocardial revascularization. The functional and structural state of mammary arteries was estimated by the noninvasive Duplex scanning before surgical operation to evaluate the possibility of using them as mammary-coronary bypasses (MCB). Direct blood velocity measurements in the bypasses during aortocoronary bypass (ACB) surgery and MCB were made by ultrasound pulsed Doppler system with a 10 MHz transducer. A method was developed for noninvasive Duplex scanning and evaluation of coronary bypass flow in the postoperative period by using the ultrasound system with 5 a MHz phased array linear transducer, which enabled one to obtain the bypass image in B-mode and the Doppler velocity spectrum inside the bypass.

Lovastatin was investigated in a single-blind placebo-controlled trial in 150 patients with coronary atherosclerosis confirmed by coronary angiographic studies and those with nonfamilial hyperlipoproteinemia. After 3 months of treatment total cholesterol (TC) level was reduced by 36% (p less than 0.001), LDL cholesterol level by 48% (p less than 0.001), triglycerides level by 19% (p less than 0.001), VLDL cholesterol by 24% (p less than 0.01), whereas the HDL-cholesterol level was increased by 36% (p less than 0.001). Besides, concentration of apolipoprotein A-I increased by 19% (p less than 0.05), apolipoprotein B decreased by 22% (p less than 0.05) and the ratios of LDL cholesterol/HDL cholesterol and TC/HDL cholesterol decreased by 64% and 56%, respectively (p less than 0.001). The side effects of lovastatin were negligible. Thus, lovastatin is a highly effective and well tolerated hypolipidemic drug for the treatment of patients with
IHD
and hyperlipoproteinemia.

In 22 patients with spontaneous angina pectoris, diagnostic value of 24-hour ECG monitoring for cardiac rhythm disturbances registration was compared to that of cold test (CT) and hyperventilation test (HVT). Ventricular extrasystoles or paroxysmal tachycardia during anginal attack (either spontaneous or induced) was registered in 40.9% of patients. 24-hour ECG monitoring proved to be the most valuable method for determination of the anginal attack character and connected with it's rhythm disturbances. Combination with CT and HVT increases occurrence of angiospastic angina pectoris and cardiac rhythm disturbances determination during
myocardial ischemia
. Repeated HVT in 14 patients using calcium antagonists (diltiazem, nifedipine) induced no coronary spasm.

Late potentials (high-frequency oscillation of the initial part of ST-segment) reflect decelerated ventricular depolarization and predispose onset of reentry arrhythmias. Investigators are keenly interested in their possible exact registration. The article describes the experience of realization of late potentials registration method based on the method of Simson et al, with the use of a USSR-produced electronic apparatus. The first experience of clinical application of the late potentials registration method in patients with
IHD
is analysed. Presence of late potentials is correlated with spontaneous ventricular arrhythmias and possible disturbances of left ventricular contractility. The obtained data as well as analysis of technical possibilities of the developed system allow evaluation of the proposed technique as acceptable for late potentials registration.

The structure of consumption and need in cardiovascular drugs were studied in a hospital and in the Kirghiz Scientific Research Institute of Cardiology according to a scheme developed in the USSR Cardiology Research Center. Use of cardiovascular drugs was studied with respect to optimal results of therapy in 361 patients with main cardiac pathology (
ischemic heart disease
, essential hypertension and their combination) who had received 1125 courses of drug treatment. The prescriptions were analysed according to their intensity, regularity and optimal effect. The authors assessed provision of cardiological patients of Frunze with drugs and gave recommendations on its improvement, in particular, satisfaction of need in some active drugs (prolonged nitrates, calcium antagonists, cardiac glycosides).

The article deals with the effect of permanent, temporal and total
myocardial ischemia
on the oxidation of acetate, hexanoate, palmitoylcarnitine and palmitoyl-CoA in isolated rabbit heart mitochondria. All the three models of ischemia in different experimental situations demonstrated a similar degree of fatty acid oxidation suppression independent of the length of acyl residue, the suppression being the greatest during the first hours and in total ischemia. Both in the control and in ischemia, respiratory activity was at its highest level with acetate, decreasing in the order above. Thus, the rate of Krebs' cycle reactions and of respiratory chain does not limit medium- and long-chain fatty acid oxidation. It is nevertheless established that suppression of their oxidation in ischemia is completely determined by the decrease of cytochrome S and of endogenous substrate intermediates of Krebs' cycle in the mitochondria; decreased adenine nucleotide and carnitine-palmitoyl-transferase transport (other authors' data) is not critical, at least in early ischemia (0.5 h). Ischemic mitochondria are characterized by incomplete palmitoylcarnitine oxidation.
