The oestrogen deficiency syndrome comprises all subjective and objective complaints and all organic and metabolic organ damage encountered after natural and induced menopause. In the development of these pathological conditions participates in a marked way endogenous oestrogen deficiency - oestrogen deficiency. The oestrogen deficiency syndrome affects some topographically remote organs and is manifested by an apparently clinically heterogeneous symptomatology. The clinical picture of the syndrome can therefore be very varied. It affects the mucosal membranes of the vagina, urethra, trigonum of the urinary bladder, oral cavity, nose, pharynx and larynx, eyes and large intestine, as well as the skin and its adnexa, the breasts, skeleton and cardiovascular system. The syndrome involves therefore all medical disciplines. The common origin of all pathological conditions ensuing from oestrogen deficiency calls for interdisciplinary collaboration in a new discipline - climacteric medicine. Exogenous oestrogens in different forms hold an important place in the prevention and treatment of oestrogen deficiency syndrome. In the first place it is prevention and treatment of arteriosclerosis, ischaemic heart disease, cerebrovascular attacks and osteoporosis. If we add undoubtedly favourable effects of hormonal substitution therapy on vasomotor complaints and symptoms of the organ syndrome, then the advantages of HRT markedly overrule its risks.

Hormonal replacement therapy becomes frequently used in peri- and postmenopausal women. It causally affects the climacteric syndrome, positively stimulates psychics, improves quality of the skin, decreases dryness of mucous membranes and frequency of recurrent inflammations of eyes and vagina. The positive influence on the bone metabolism and therefore on the incidence of osteoporosis highly dominates among its long-term effects. Long lasting hormonal replacement reduces also the incidence of Alzheimer disease, colorectal carcinoma and it has particularly favourable effect on the cardiovascular system. Estrogens positive affect the lipid spectrum, however, more than 50% of their beneficial influence comes from their direct vasodilatory effect. Estrogene replacement becomes in many countries indicated for the primary prevention of the ischemic heart disease. The question of its application for the secondary prevention remains still open.

Artificial vaginas are designed to imitate the female sex organ. This is the first reported case in the forensic literature of a man being pronounced dead with an artificial vagina in situ. A middle-aged man was found unconscious in a bathroom when the door was forced open and was pronounced dead on admission. Autopsy revealed that the penis was inside an artificial vagina. There were no injuries, but there were left ventricular hypertrophy, myocardial fibrosis, and narrow coronaries. The cause of death was ascertained as ischemic heart disease due to coronary atherosclerosis and the comments included were no evidence of violence, and ischemic heart disease could have been precipitated due to abnormal sexual activity. If removal of artificial vagina was done before the admission, this circumstance could not have ascertained. Removal of such devices before admission to hospitals could be the reason why such incidents do not come to light.
