The physical-disease charcteristics of 125 skid row and 736 non-skid row male alcoholics were compared in detail to determine whether skid row alcoholism is characterized by a distinct medical, as well as a social, profile. Trauma, tuberculosis,
venereal disease
, and malnutrition were more common in the skid row alcoholics. Epilepsy, peripheral neuritis, acute brain syndromes, chronic brain disease, and lifetime recordings of all nervous system illnesses also occurred more frequently in the skid row group, as did gastritis, gastrointestinal hemorrhage, ulcer surgery, and postgastrectomy syndrome. Fatty liver, hypertension,
ischemic heart disease
, cardiomyopathy, and cardiovascular illnesses of all kinds, however, were less common. The skid row medical profile is, in part, the product of a unique sociologic environment. Thus, skid row alcoholism may be viewed as a distinct sociomedical entity.

Forty patients (34 males and 6 females) with neurological complaints/manifestations and with a past history of multiple sexual partners attending the Government Rajaji Hospital, Madurai, India between April 1992 and October 1992 were investigated for neurosyphilis. Metabolic disorders, hypertension,
ischaemic heart disease
, arrhythmias and trauma were excluded. Seven males (17.5%) were found to have neurosyphilis. The youngest was 26 years old and the oldest was 47. All were married and of low socioeconomic background. Meningovascular syphilis was the predominant presentation (6:1). Associated cardiovascular involvement was noticed in one of the cases. There was no associated HIV infection in these cases. The incidence is higher than previous reports from this centre.

The aim of our investigation was to assess the efficacy of enhanced external counterpulsation (EECP) in
ischemic heart disease
(
IHD
) patients depending on previous by-pass surgery. 30
IHD
patients (pts) were treated with EECP using standard protocol (35 hours, 1 hour each procedure). All patients were divided into 2 groups. 1-st group (n=15) - pts after by-pass surgery, 2-nd group (n=15) - pts without previous coronary revascularisation. Pts were examined by ECG, echocardiography, bicycle test, radionuclide perfusion scintigraphy (SPECT) before and after EECP treatment. There was significant decreasing of perfusion defect severity for rest SPECT after EECP treatment (64.93+/-13.21
std
and 58.53+/-17.28
std
, respectively, p<0.01) and decreasing of size of perfusion defects - 26.10+/-15.76% before and 22.63+/-18.54% after EECP (p<0.05). Size and severity of perfusion defects estimated by SPECT at stress conditions didn't change. After EECP treatment the decreasing of perfusion defect severity (p<0.01) was registered in both groups, the decreasing of size of perfusion (p<0.05) - only in the 1-st group. In the 1-st group decreasing of perfusion defect severity and size of perfusion was more intensive. The efficacy of EECP depends on previous coronary revascularization.

Rhabdomyolysis is a rare, but possible, complication of combination antiretroviral therapy (cART). We report a unique case of an HIV-positive patient on cART who came to our attention for suspected
ischaemic heart disease
. Coronary angiography was carried out and complicated in the following days by rhabdomyolysis. We discuss the possible links between rhabdomyolysis, iodinated contrast media and HAART.
