There is a common belief that administration of anticoagulants to patients with brain tumors is contraindicated. Between 1982 and 1986, 50 patients with
deep venous thrombosis
and pulmonary emboli and brain tumors were examined and treated. Twenty-four patients received an inferior vena cava Greenfield filter and 25 patients were treated with anticoagulants. One patient was terminal and received no therapy. Patients in each group were similar with regard to age, sex, primary tumor, computed tomographic findings, and ultimate outcome. At the time of diagnosis, all patients had
residual tumor
and most had significant cerebral edema and midline shift. There were no complications in the group receiving Greenfield filters. One patient had a pulmonary embolus after the filter was placed and later required anticoagulant therapy. In the group receiving anticoagulants, one patient had focal intraventricular bleeding observed incidentally on computed tomographic scan one month after beginning anticoagulant therapy and was totally asymptomatic. One patient had gastrointestinal tract bleeding five days after beginning anticoagulant therapy with heparin sodium, and the therapy was therefore discontinued. No other patient had significant bleeding. In view of these findings, a reevaluation of anticoagulant therapy in patients with central nervous system tumors is warranted.

Duplex sonography was used to measure diameters of the common femoral, superficial femoral, and popliteal veins in 56 patients followed for more than 6 months after
DVT
and in 17 normal subjects. Diameter changes with Valsalva's maneuver were also measured as an index of venous compliance. Among patients with unilateral thrombosis, segments with
residual disease
were 0.07 to 0.28 cm smaller than the contralateral disease-free side (p < 0.05 for CFV and SFV) with a diameter index (ipsilateral/contralateral diameter) significantly less than that of normal subjects. In contrast, completely recanalized segments were not significantly different from the contralateral side and had diameter indices indistinguishable from normal subjects. Distensibility with Valsalva's maneuver was not significantly different from normal in
DVT
patients with either resolved or
residual disease
. Venous diameter does decrease following
DVT
, but returns to normal following complete recanalization and is not associated with chronic venous compliance changes.

Duplex ultrasound (US) has become the new gold standard in the assessment of acute
deep vein thrombosis
. In view of the large number of cases with persistent changes, all cases should be reassessed at 6 months to document the extent of
residual disease
. The role of duplex US in chronic venous disease is less well established but it is evolving as the single most useful examination and is likely to supplant other noninvasive methods in the near future.

Dienogest is a novel synthesized progestin used for treatment of endometriosis. This is the first case report describing a therapeutic effect of dienogest on a gynecologic malignancy. The patient was a 44-year-old woman with advanced adenosarcoma arising from the endometriosis in the rectovaginal space and infiltrating the left pelvic wall, left ureter, rectum and vagina. The
residual tumor
after tumor debulking surgery was resistant to both chemotherapy and radiotherapy. Dienogest was used as a substitute for medroxyprogesterone acetate because of the presence of
deep vein thrombosis
. Based on the RECIST criteria, partial response was obtained with oral dienogest therapy at six months and the serum CA125 level also decreased from 70 U/ml to 16 U/ml. The tumor remained stable up to 21 months. Thromboembolism or other adverse effects did not occur during the dienogest therapy. Dienogest may be useful for the treatment of adenosarcoma arising from endometriosis.
