A four year old girl presented with
varicella
gangrenosa, and haematological investigations showed a disseminated intravascular coagulation. The child subsequently developed a unilateral
deep venous thrombosis
. She was treated with oral steroids and intravenous heparin and made a full recovery.

We report six cases of protein S deficiency secondary to
varicella
. Five cases were complicated by thrombotic and vascular events, namely purpura fulminans and necrotic vasculitis,
deep vein thrombosis
and stroke. Two cases were associated with protein C deficiency and one case revealed a heterozygous factor XII deficiency. The underlying mechanism of this acquired protein S deficiency is unclear but could be related to a direct effect of zoster virus.

We report a 13-year-old boy with hereditary deficiency of protein S, who developed a
deep vein thrombosis
of the lower limb after a
varicella
with severe cutaneous lesions. Hereditary protein S deficiency is an established cause of thrombophilia; however thrombotic events are seldom described in pediatric patients. A review of previous literature revealed 35 cases, 16 girls and 19 boys, with a first episode below of the age of 18 years old (x = 10y). The 57% of the patients had venous thrombosis, 20% arterial thrombosis, and 14% both and in 9% the type of thrombosis was not reported. Predisposing factors were referred in only 12 cases. The deficiencies can be classified as type I in 25 patients and type III in 8.

Three cases of
deep venous thrombosis
following
varicella
infection are described which were successfully treated with bed rest and anticoagulants. Two of these patients had severe pulmonary manifestations of
varicella
and the third was complicated by pulmonary embolism.
Deep vein thrombosis
is an uncommon systemic manifestation of
varicella
, possibly associated with vascular endothelium wall damage caused by
varicella
zoster virus infection.

We report a case of Hughes syndrome 1 week after the occurrence of
chickenpox
. The patient presented with the clinical picture of acute right iliofemoropopliteal
deep vein thrombosis
. IgM and IgG anticardiolipin (aCL) were both positive upon admission. The IgM aCL antibodies were still positive 6 weeks later, which suggests their role as a predisposing factor of
deep vein thrombosis
.

An 11-year-old girl developed proximal
deep venous thrombosis
and bilateral pulmonary embolism associated with antiphospholipid syndrome following
chickenpox
. She responded to prolonged anticoagulation therapy.

A 34-year-old immunocompetent man with
varicella
zoster (VZ) infection developed
deep vein thrombosis
and pulmonary embolism after suffering severe pneumonitis. He recovered after treatment with acyclovir, high-dose steroids, and ventilatory support. The endothelial damage could be a direct link between VZ pneumonitis and pulmonary emboli.

Chickenpox
(
Varicella
) representing the primary infection by Varicella zoster virus is a common benign and self-limited infectious disease of childhood. Although the disease can be associated with complications, they are generally mild and tend to occur in adults and immunocompromised children. Severe and life-threatening complications are extremely rare, particularly those involving the cardiovascular system. We report a malnourished 5-year-old girl with
chicken pox
complicated by hemorrhagic pericarditis and
deep vein thrombosis
leading to fatal pulmonary thromboembolism. Though
varicella
infection runs a benign self-limiting course, it continues to cause significant morbidity and mortality when associated with complications, particularly in malnourished children. Hence, the importance of vaccination and early recognition of complications is emphasized.

Purpura fulminans (PF) and
deep vein thrombosis
are rare complications secondary to
chicken pox
disease. The presence of antibodies reflects an ongoing immunological process and requires specialized management. The present study reports a 4-year-old boy with no medical history who presented with purpura on the legs 10 days after
chicken pox
eruption. Laboratory tests showed a disseminated intravascular coagulation associated with low plasma protein C and S activities, and the presence of anti-protein S antibodies. A replacement therapy with protein C infusions and fresh frozen plasma was prescribed. The patient also underwent regular sessions of hyperbaric oxygen followed by the surgery. Fourteen days after the beginning of the purpuric lesions, he presented
deep vein thrombosis
(
DVT
) of the lower limbs and was treated with unfractionated heparin. This case report illustrates the pathophysiology of
DVT
occurring in a patient with
chicken pox
disease (i.e., acquired protein C and S deficiencies and anti-protein S autoantibodies) and emphasizes the utility of thrombophilia testing in order to better adapt treatment.

Varicella
(
chickenpox
) is a common disease of childhood, caused by
varicella
-zoster virus. Postviral thromboembolism is a rare complication of
varicella
-zoster virus in childhood. We describe 2 children who developed lower limb
deep venous thrombosis
shortly after
varicella
infection, along with a review of 130 previously reported cases.
