43 women with spontaneous bloody
nipple discharge
were examined by galactography performed with
water
soluble contrast medium. The purpose of this study was to evaluate the use of galactography in these patients, to localize the cause of this particular type of discharge and to register the therapeutic consequences of the findings. Surgical resection was carried out in all cases where galactography showed intraductal pathology. The remaining patients (except for four who also underwent surgery) were observed without treatment. No sign of carcinoma was found in a follow-up examination of the non-operated patients after two to four years. The results show that galactography can be suitably used to demonstrate and localize intraductal pathology, and may therefore be of importance in the preoperative investigation of spontaneous blood-stained secretion.

The results of real-time hand-held sonographic breast examinations on 86 patients are presented. The technique is described in detail. Sonography was found to be a useful adjunct to the x-ray mammogram in three groups of patients: (1) patients with dense breasts and localized symptomatology or a suspicious area on x-ray mammogram; (2) patients with nonpalpable abnormalities discovered on x-ray mammogram; and (3) patients with palpable masses considered indeterminate on x-ray mammogram. The examination was also found useful for guiding needle aspiration biopsies, in patients with persistent

, and in those with breast prostheses. Advantages of the hand-held real-time technique include supine positioning, the ability to flexibly orient the probe, improved resolution due to higher frequency transducers and the lack of compounding, and the ability to vary the amount of compression to assess tissue compliance and fixation. The disadvantage of decreased resolution in the near field can be overcome by attaching a detachable
water
-path step-off device to the transducer. Geometric distortion from the pressure of the transducer was not believed to be a problem. Hand-held sonographic technique is useful when attention can be directed to a specific area of the breast. The additional information provided results in fewer equivocal interpretations of the x-ray mammogram; however, it should not be used as a substitute for routine mammography.

To describe the appearance of isolated intraductal papilloma on contrast-enhanced
water
-specific, high spatial-resolution and rapid dynamic breast MRI, a retrospective review of unilateral breast images of 15 pathologically proven papilloma was performed. MRI revealed three patterns: Four papillomas were small, smooth, enhancing masses at the posterior end of an enlarged duct, corresponding to the "small lumenal mass" appearance of papilloma known from galactography. MRI detected two of these "small lumenal mass" papillomas in patients with abnormal
nipple discharge
even when galactography was unsuccessful. Seven papillomas were irregular enhancing masses detected in patients without
nipple discharge
. None of these papillomas had specifically benign findings. All seven demonstrated rapid enhancement and three showed rim enhancement or spiculation. These "tumor-like" papillomas mimicked invasive breast cancer on MRI. Four papillomas were occult on MRI, not revealed by either contrast-enhanced MRI or fat-suppressed T(2)-weighted MRI. Intraductal papillomas present with a variable appearance on MRI ranging from occult to "small lumenal mass" papillomas to irregular rapidly enhancing lesions that cannot be distinguished from invasive cancers.

between August 1989 and September 1991 underwent mammary ductography (contrast mammography) using

soluble contrast media. The ages ranged from 29 to 75 with a mean of 41.7 years. Two patient had bilateral duct injections giving a total of 28 ductograms. The examination was simple and well tolerated by patients. The underlying causes of

were clearly demonstrated. There were five cases of papilloma, one hematoma, one intraduct carcinoma, 11 duct ectasia, six fibrocystic disease of which four were normal. Ductography was found to be useful in the preoperative evaluation of patients with
