Seven patients were investigated 8--22 months after duodenectomy and pancreatectomy for
chronic pancreatitis
or
cancer
. Seven other patients, admitted for minor surgery, served as controls. 40 ml 100 mmol/l HCl was infused over 5 min into the jejunum 10 cm beyond the gastrojejunal stoma in the duodenectomized patients and 10 cm beyond the pylorus in the controls. Plasma immunoreactive secretin (IRS) increased from 3.0 +/- 0.8 pmol/l (mean +/- S.E.M.) to 9.8 +/- 0.5 pmol/l (p less than 0.01) in the duodenectomized patients. In the controls the increase was significantly higher than in the patients (p less than 0.01)--from 1.5 +/- 0.3 pmol/l to 19.5 +/- 3.3 pmol/l (p less than 0.01). The study shows that the jejunum is able to release IRS after acid infusion in duodenectomized patients.

The concentration of beta2-microglobulin (beta2m) and carcinoembryonic antigen (CEA) was measured radioimmunologically in the sera of 79 patients with malignant disorders and 15 patients with
chronic pancreatitis
. Elevated levels of beta2m and CEA were found in 11 out of 22 patients with carcinoma of the pancreas, which sets off this
malignancy
from
chronic pancreatitis
and other malignant tumors. Only 3 patients with carcinoma of the pancreas exhibited serum levels within the normal range for both parameters and none of the patients with
chronic pancreatitis
was shown to have elevated levels of beta2m. The simultaneous determination of beta2m and CEA suggests itself for the diagnosis of pancreatic
malignancy
especially in the case of a tentative diagnosis of a pancreatic tumor.

The ratio of renal clearance of immunoreactive trypsin relative to renal clearance of creatinine was measured in 71 subjects including 27 controls and patients with

, and acute pancreatitis. The upper limit of the control range was 4.1 x 10(-5) (mean + 2SD). 6 of 9 patients (67%) with acute pancreatitis had raised values. All 18 patients with
chronic pancreatitis
had values within the control range. In contrast, all 17 patients with carcinoma of pancreas had raised clearance ratios. The test may therefore prove valuable in distinguishing between
chronic pancreatitis
and
cancer
of pancreas.

Acid hydrolases (lysosomal enzymes) were analyzed and compared with trypsin in duodenal juice obtained after a test meal (Lundh test). The possible diagnostic role of acid hydrolases in pancreatic disease was investigated. In all patients with
chronic pancreatitis
normal values of acid hydrolases but subnormal trypsin activities were found. In pancreatic cancer normal values of acid hydrolases and normal trypsin values were seen in three patients with small tumors, whereas five patients with more advanced
cancer
of the pancreas had decreased trypsin activity and three of them high activities of acid hydrolases in duodenal juice. In five patients operated on with a gastroenteroanastomosis acid hydrolases were markedly increased. Five patients had no activity of acid hydrolases in the aspirate, probably reflecting technical failure with dislodgement of the catheter from the duodenum to the stomach. In conclusion the assay of acid hydrolases does not seem to increase the diagnostic value of the conventional Lundh test (trypsin).

Of 623 cases of malignant pancreatic tumor seen at University of Chicago hospitals over the last 30 years, 23 cases of malignant islet cell tumor were found (8 additional cases were found in the literature). Calcification, found in 2 of these cases, was characteristically discrete and nodular (calcifications found in
chronic pancreatitis
are typically diffuse, multiple, and punctuate). At least 7 of the 10 cases of islet cell tumor with calcification were malignant. Slow growth of the tumor, with calcified metastases, strongly suggests the diagnosis of
malignancy
; this combination of findings has been established in 3 cases. In 73 cases of benign islet cell tumor, no calcifications were found.

Serum Ribonuclease (RNase, EC. 3. 1. 4. 22) of normal persons and of patients with
chronic pancreatitis
, or pancreatic cancer was determined with poly (C) as substrate. Strikingly abnormal elevations occured in the serum RNase of patients with pancreatic cancer (p less than 0.001). Average serum RNase values of 18 normal persons, 10 patients with
chronic pancreatitis
and 26 patients with pancreatic cancer were 92, 118, and 249 units, respectively. In patients with pancreatic cancer, we compared the RNase level with four histologic types (ductar cell adenocarcinoma, anaplastic cell carcinoma, acinar cell carcinoma, and islet cell carcinoma). Adenocarcinoma showed higher activity than the other histologic types (p less than 0.005). When we compared the serum of pancreatic cancer and pancreatic cancer tumor extract with normal serum and normal pancreas extract, strikingly different phosphocellulose chromatographic pattern were evident. The correlation of increased serum RNase levels with tumor histology and different chromatographic pattern may explain the new enzyme production in
cancer
patients, and have biological significance in the development of pancreatic cancer.

We studied serum carcinoembryonic antigen (CEA) levels in 82 patients. Thirty-four of these had benign diseases while 48 had malignant diseases. Highest incidence and levels of CEA occurred in the sera of patients with pancreatic cancer and stomach cancer. In this paper we focused our particular attention on the serum CEA of 25 pancreatic cancer patients, and examined differences in serum CEA levels in relation to histologic differentiation and sites of pancreatric
cancer
. No statistical difference in serum CEA level was found among various histologic types and sites of the pancreatic cancer. Clinical courses of two patients with pancreatic cancer were also studied. Serial determinations of CEA levels are most useful in assessing the effect of operation or chemotherapies and are a useful indicator for differentiating pancreatic cancer from
chronic pancreatitis
but cannot be a conclusive factor for the diagnosis. Finally, we correlated serum CEA levels with those of RNase and confirmed a positive correlation.

By the use of [11C]methionine and positron computed tomography (PCT), images of the pancreas were obtained in 32 patients. The injection of between 10 and 20 mCi of this product enables four to six transverse sections to be obtained. Seventeen of the patients studied had no exocrine pancreatic disease, and in all these cases the pancreas was clearly visible. In four cases of pancreatic carcinoma and one of retroperitoneal tumor, there were abnormalities visible. In five cases of
chronic pancreatitis
, no pancreatic uptake was observed. In a sixth case, concentration was visible, but only in the head of the pancreas. One case of acute pancreatitis, which showed no concentration during the acute phase, returned to normal after recovery. When visible, the pancreas was easily located and distinguishable from the intestinal image, except in two cases that were uninterpretable for technical reasons. No false positive or negative was observed, but a differential diagnosis between
cancer
and pancreatitis was impossible.

The computed tomography gives direct visualisation of the pancreas in a transverse section. Form, size, and changed consistency of the organ can be diagnosed. Being a non-invasive technique it does not stress the patient, and can be applied to the severly ill with acute haemorrhagic pancreatitis. Other indications are

. The differential diagnosis of

, especially from
chronic pancreatitis
, may be difficult. Further methods of investigation such as arteriography or endoscopic retrograde pancreatography may also have to be used.

(25), hypersecretory disorders (10), renal transplanted (4) and with pancreatic cancer (9), were determined by a single radial immunodiffusion method. Although a trend of immunoglobulin hypersecretion in benign diseases and hyposecretion in
malignancies
was present, individual levels of Ig-A and Ig-M differed statistically only between
cancer
and other groups. Calculating the ratios of Ig-A to Ig-M (A/M) and Ig-A to total immunoglobulin contents (A/T) in the PDF of patients with
chronic pancreatitis
or
cancer
, however, a significantly higher or lower ratio, respectively, was found compared to control group. Differences in A/M ratios could not be detected between patients with benign diseases, but were constantly present when these patients were compared to pancreatic cancer. The underlying mechanism(s) of the disturbed immunosecretory system in pancreatic diseases remains speculative. Nevertheless, immunoglobulin measurements in the PDF of patients with pancreatic diseases offer a simple diagnostic aid in clinical differentiation of pancreatic pathology.
