Gallbladders of 12 cases with chronic cholecystitis showing pseudopyloric glands (PPG) and of 18 cases with
acute cholecystitis
or chronic cholecystitis but without PPG were examined by the
peroxidase
- antiperoxidase (PAP) method using rabbit antibody against human lysozyme (LM). LM-immunoreactivity was detected in the cytoplasm of PPG and, to a lesser extent, in the pits of epithelial crypts that gave rise to PPG. No LM was found in normal gallbladders; in cases of cholecystitis without PPG, LM-immunoreactivity was restricted to infiltrating inflammatory cells. The presence of LM in PPG suggests that PPG represent functional metaplastic areas, involved in the non-specific defence mechanisms through participation of LM.

The authors analyse the results of surgical treatment in 105 elderly and old-aged patients who were admitted to the clinic for
acute cholecystitis
and concomitant diabetes mellitus of various severity. The results of clinical, biochemical, morphological, and cytochemical (salkaline phosphatase of neutrophilic leukocytes, HCT test,
myeloperoxidase
) studies revealed "the syndrome of mutual aggravation" of the pathological process. The authors conclude that an early (during the first 3 days after the onset of the disease and the severity of the "syndrome of mutual aggravation") operative intervention must be undertaken on this category of patients.

In this prospective study the free radical mediated reactions, the changes of endogenous antioxidant defense mechanism and activation of leukocytes were measured from the blood of patients undergoing elective cholecystectomy because of symptomatic cholecystolithiasis. The patients were randomised into two groups. Group one contained 21 patients treated by open cholecystectomy(OC). Group two consisted of 21 patients treated by laparoscopic cholecystectomy (LC). Both groups had similar patient characteristics. Patients with
acute cholecystitis
, pancreatitis, choledocholithiasis or other disease were excluded. Values from patients in both groups were compared. The measured biochemical parameters are the following: malondialdehyde (MDA) as a marker of the free radical induced lipid peroxidations, reduced and oxidised glutathione (GSH-GSSG), as endogenous scavengers as well as markers of oxidative stress and
myeloperoxidase
activity (MPO) of leukocytes. The results showed significantly lower values of postoperatively measured MDA, GSH-GSSG, and MPO activity of leukocytes in patients with laparoscopic cholecystectomy, indicating a lesser stress response and tissue trauma in this group of patients. The results correspond to the favourable results of most other trials evaluating clinical aspect of LC.

Functional condition of basic components of the microbicidal system (cation protein
myeloperoxidase
, acid phosphatase) and spontaneous NB-test of neutrophylic leukocytes' activity values were studied with the aid of cytochemical methods in 175 patients: with
acute cholecystitis
(133) and exacerbation of chronic one (42). As a result of performed examinations various changes of intraleukocytic components and activity of spontaneous HB-test, of neutrophils were established, which depended on phase, severity of the disease and morphological changes and manifestations of pathological process. The determined changes of intracellular components and activity of spontaneous HB-test values of leukocytes in patients with acute and chronic cholecystitis demonstrate the tension of nonspecific reactivity of the organism.

A 78-year-old man with chief complaints of cough and lower limb numbness was admitted to our hospital. Serum
myeloperoxidase
-antineutrophil cytoplasmic antibody titer (MPO-ANCA) was elevated (48.8U/ml), and a diagnosis of microscopic polyangiitis (MPA) was made. After admission, the patient developed a fever and right upper quadrant pain. Ultrasonography and computed tomography revealed an
acute cholecystitis
of unknown cause, and laparoscopic cholecystectomy was performed. Histopathological examination of the resected gallbladder revealed necrotizing vasculitis along with the infiltration of eosinophils, lymphocytes, and plasma cells around the small arteries in the muscular layer of the gallbladder, which are characteristics of MPA.
