The concentrations of fibronectin in aqueous humor, measured by ELISA which was developed to detect fibronectin, ranged from 5 ng/ml to 100 ng/ml. Aqueous humor was aspirated from human eyes with cataracts and glaucomas using a 26 gauge needle through the peripheral cornea before making the limbal incision into the anterior chamber during surgery. The results of the study show that the average concentration and standard deviation of fibronectin was 0.136 +/- 0.192 microgram/ml in cataract eyes, and 0.962 +/- 0.918 microgram/ml in glaucoma eyes respectively. There was a statistically significant difference between both groups (p = 0.000). However, no significant differences according to age and sex were noted. There was no influence due to preoperative intravenous mannitol injection on fibronectin concentration. The source of aqueous fibronectin is still not clearly known and the mechanism of the higher concentration of fibronectin in glaucoma has not been clearly disclosed, however it is thought that normally present fibronectin is accumulated in the anterior chamber because it can not pass the aqueous outflow pathway, or that fibronectin production may be increased in glaucoma.

Avidin-biotin immunohistochemical staining for fibronectin was used to study 17 aphakic intraocular lens implanted globes and 12 explanted intraocular lenses. Fibronectin was identified as a component of acellular pseudophakic membranes and was also observed within lens epithelial cells and macrophages.

The content and localization of fibronectin, an extracellular glycoprotein, in the serial sections of lenses of normal human donors and cataractous patients of different ages were determined by the indirect immunoperoxidase staining technique. This was followed by the evaluation with quantitative morphometric analysis. It was shown that fibronectin was present in the area of cell contacts as single deposits of faint orange-brown stained material in the lens samples of young donors. The fibronectin level was raised in lens sections from aged donors. Its accumulation was detected mostly within the spaces of the lens fiber cells. At different stages of cataractogenesis a dramatic decrease of the fibronectin content was detected in the lens sections obtained from patients of different ages. A new linear spectrophotometric technique was developed for evaluation of the lens transparency, to correlate the lens opacity with corresponding histological data obtained from the immunostaining technique. Morphological studies performed further suggested that the lens fiber cell plasma membrane structures were deteriorated. This was observed as changes of fibronectin staining in the lens sections at different periods of human ageing and cataract development. It is concluded that a decrease of fibronectin staining in the human lens is an indication for the structural damage of the lens fiber cell plasma membranes during ageing and cataractogenesis.

The experiment was designed to illustrate the difference between the diamond knife and the excimer laser (ArF 193 nm) on keratotomy incisions. The histopathologic effect of fibronectin applied to these wounds was observed. Fibronectin demonstrated a chemotactic effect on fibroblasts and increased the amount of newly synthesized collagen deposited at the incision sites. The excimer laser incisions treated with fibronectin had 43% more fibroblasts per 2,500 microns 2 than the excimer laser incisions not treated with fibronectin. The adjustable diamond knife incisions treated with fibronectin had 42% more fibroblasts than the adjustable diamond knife incisions not treated with fibronectin. The excimer laser ArF 193 nm incisions had 18% more fibroblasts in the incision area than the adjustable diamond knife incisions independent of fibronectin usage. This indicates that the excimer laser incisions and the diamond knife incisions may have different healing mechanisms--perhaps because of the distinctive modes of action of these instruments.

The natural course of corneal epithelial erosions was studied retrospectively in patients who had undergone cataract extraction. Postsurgical epithelial defects occurred in 41 of 796 eyes (5.2%). No difference was noted in the incidence of corneal epithelial defects in eyes after intracapsular and extracapsular cataract extraction. Of the 41 eyes with postsurgical corneal erosion, 26 (63.4%) showed corneal epithelial defects by the fourth postoperative day. Two types of healing patterns were noticed: rapid healing, in which there was complete epithelial resurfacing within 4 days after the onset of corneal erosion, and prolonged healing, in which the period for complete epithelial resurfacing ranged from 6 days to 34 days. Since fibronectin has been reported to facilitate corneal epithelial migration and adhesion, we administered autologous fibronectin eyedrops in five cases in which corneal epithelial erosion developed after cataract surgery and epithelial defects persisted more than 7 days (except case 2). In all cases receiving fibronectin treatment, complete epithelial resurfacing occurred. In four of five eyes, fibronectin eye drops were effective in epithelial resurfacing within 3-9 days; in one eye, complete epithelial resurfacing took 23 days. The present results indicate that there are two types of epithelial erosion and that fibronectin eye drops might be one of the possible therapeutic approaches for the treatment of persistent epithelial defects. The weak adhesion of epithelial cells underlying basement membrane may be one of the causes of persistent epithelial defects after cataract surgery.

"Ball and socket"-like membrane processes interlock fiber cells in the sheep lens cortex, but appear reduced deeper in the lens. Wheat germ agglutinin (WGA) binds preferentially to these ball and socket structures, and more weakly to other membrane regions. On protein blots, 125I WGA binds to glycoproteins with 140,000 and 32,000 apparent molecular weight, the smaller protein also binding 125I fibronectin. In two animal cataract models, the intense WGA labeling of globular bodies replaces the spotty WGA staining pattern associated with the ball and sockets in the normal lens.

The concentration of the high molecular glycoprotein fibronectin (FN) was measured by an enzyme-linked immunoadsorbent technique (ELISA) in tears of 25 patients before and after cataract extraction. Both native and degraded FN was measured. The median concentration of FN in tears before surgery was 0.28 mg/l (range: 0.1-3.8 mg/l) and 0.18 mg/l (range: 0.1-1.95 mg/l for degraded and native FN, respectively. The time course of the median values of both types of FN during the post-operative observation period described a biphasic curve with peaks at the second and fifth day following operation. On the twelfth day the concentration of degraded and native FN had almost reached pre-operative levels. The variation in the post-operative FN concentration proved statistically significant. The biological significance of the changes in FN level in the post-operative period is not obvious; the first peak may be explained by a spill-over from plasma, and the second peak may reflect FN production from fibroblasts in the granulation tissue of the corneoscleral cicatrix.

The binding of plasma fibronectin to the collagens I, II, and III was greater in a cohort of glaucoma patients. In contrast, binding of plasma fibronectin to collagen type IV was less in 39 glaucoma patients than in any of 3 other diagnostic categories, including 92 patients that were normal, cataract, and glaucoma suspect patients. This observation may have significance in further understanding the control of aqueous outflow resistance in glaucoma and nonglaucoma patients.

Lipid peroxidation products, antioxidant system parameters, somatotrophic hormone, corticosteroids, fibronectin, and cyclic nucleotides were measured in the blood of 119 patients with open-angle glaucoma and mature senile cataract before and after a course of exposure of the eyeball to He-Ne laser. Statistically significant changes in the examined parameters were revealed, explaining to a great measure the antiinflammatory and vasodilating effects of laser and its ability to intensify the repair processes.

Effects of instillations of proteinase inhibitors and autologous fibronectin on the course of regeneration processes in corneal epithelial defects were studied in patients after perforating and lamellar keratoplasty, cataract extraction, trabecul- and vitrectomy. Proteinase inhibitors contrykal or gordox were administered to these patients to neutralize increased activity of lacrimal plasmin-like enzymes which was 899.3 +/- 80.8 rel. U on day 1 of observation vs. 50.72 +/- 7.73 in normal subjects. Contrykal (gordox) helped significantly reduce these enzymes activities in patients with corneal epithelial defects in comparison with the control group as soon as during the first days of the observation. Combined use of contrykal (gordox) and autologous fibronectin helped attain complete epithelialization in all the patients in 12.3 +/- 1.7 days (47.9 +/- 6.3 days in controls).
