Twenty two patients (mean age: 69 years) undergoing cataract surgery were perfused with imipenem (1g for 1 hour). The samples of serum and aqueous humor were taken simultaneously 0.5-1-2-5-7 hours after the end of perfusion and immediately stabilized by MES or MOPS buffers and ethylene glycol. The antibiotic concentrations were determined with Bacillus subtilis ATCC 6633. Aqueous humor levels increased from the end of perfusion to 2nd hour (peak 5.2 mg/l) and decreased after (mean level 2.8 mg/l). The concentrations were well above MIC-90 of the species commonly involved in bacterial endophthalmitis. Imipenem appears to have an exceptional penetration into ocular fluids.

Thirty five patients (mean age: 70 years) undergoing cataract surgery or vitrectomy were perfused with ciprofloxacin (400 mg IV/for 1 hour) or received either a single, 2, 6 or 8 preoperative orale doses (750 mg each). The samples of serum, aqueous humor or vitreous fluid were taken simultaneously at 1, 2, 3, 4, 6, 9 or 12 hours after the last administration of antibiotic. The antibiotic concentrations were determined by microbiological techniques (using Bacillus subtilis ATCC 6633). Aqueous humor levels, after perfusion of 400 mg, were stable during 3 hours (mean: 0.22 mg/l); after a single oral dose of 750 mg were stable during 12 hours (mean: 0.18 mg/l) but were significantly lower than after multiple doses (up to 0.8 mg/l) which suggest accumulation in anterior chamber. In vitreous fluid, levels were elevated from one to six hours after the end of the perfusion (up to 0.4 mg/l). The levels are above the MIC 90 of enterobacteriaceae, and around the MIC 50 of Staphylococcus or Pseudomonas.

In 53 patients undergoing cataract extraction, the authors measured pefloxacin and ofloxacin penetration in aqueous humour and lens. Cataract removal was performed at varying times after either the end of one-hour infusion of pefloxacin, ofloxacin per os in one time, or pefloxacin per os during 3 days. In the two first schemes, acme concentrations in aqueous humour are 1.45 mg/l for pefloxacin, 1.14 mg/l for ofloxacin. These concentrations reach the MIC90 of most bacteria usually involved in endophthalmia. The steady state concentrations of pefloxacin are between 6 and 7 mg/l during the first 12 h: they reach the MIC of 80% of Streptococci. There is no evidence of accumulation of pefloxacin in lens; lenticular concentrations of ofloxacin are very low and rapidly decrease.

Ciprofloxacin is a potentially useful antibiotic in eye infection. A pilot study was performed on 25 patients undergoing routine cataract surgery to assess the penetration of the drug into aqueous humor after oral administration. The mean peak level of 0.56 mg/l achieved in aqueous is above the MIC for most gram negative organisms, including Pseudomonas aeruginosa.

The aqueous penetration of orally administered Ciprofloxacin (CPX) was studied in twenty-two human volunteers undergoing cataract surgery. CPX in a dose of 250 mg, was given p.o. at 12 hourly intervals in 3 groups such that a maximum dose of 0.5 grams was delivered in one group, 1.0 grams in another and 1.5 grams in the last group. A fourth group consisted of 6 individuals who were controls and received lactose tablets. Twelve hours after the last dosing aqueous samples were collected and assayed by the Grove-Randall technique. The aqueous penetration of Ciprofloxacin achieved was 1.51 +/- 0.15 mgL-1 in group II and 2.49 +/- 0.52 mgL-1 in group III. In group I, the aqueous concentration was experimentally indeterminate and the control samples in group IV did not show any antibacterial activity. Divided oral doses of 1.5 grams of CPX were found to produce an aqueous titre up to 2.5 times the MIC of Ps. aeruginosa.

The penetration of fosfomycin in aqueous humour was studied in 21 patients who were to undergo cataract surgery. All patients received 4 grams of fosfomycin as an infusion lasting one hour. Concentrations of the drug in aqueous humour were measured 1, 2, 4, 6 and 12 hours after the start of infusion. Drug concentrations in aqueous humour and serum were measured by HPCE (High Performance Capillary Electrophoresis). The aqueous humour concentration at one hour was 11.46 mg/l +/- 2.12. Peak concentration was 14.63 mg/l +/- 5.54, reached two hours after the infusion. Concentrations were high until 6 hours and remained significant at 12 hours. These results confirm the excellent diffusion of fosfomycin in aqueous humour, with high levels at 12 hours. They justify its use in intraocular infections, by infusions repeated every eight hours, to maintain concentrations above the MIC 90 for organisms usually susceptible to the drug.

The purpose of this study was to investigate the penetration into the aqueous humor of cefuroxime after a single oral dose as cefuroxime axetil. Fourteen patients scheduled for cataract extraction received a single oral dose of cefuroxime axetil corresponding to 500 mg of cefuroxime 2-8 h preoperatively. Aqueous humor samples were obtained at the beginning of the cataract surgery and blood samples were drawn at the time of anesthesia. Cefuroxime levels were determined by high-performance liquid chromatography. The aqueous levels were (mean +/- SEM) 0.48 +/- 0.13 microgram/ml from 3 to 8 h after administration. Serum levels averaged 3.80 +/- 0.58 micrograms/ml. These data indicate that detectable levels of cefuroxime, exceeding the MIC of some bacterial species that frequently cause intraocular infections, may be achieved in uninflamed eyes after a low dose of cefuroxime axetil.

This study was designed to measure the concentration of ofloxacin in aqueous humor after topical, oral and intravenous administration in 50 patients undergoing cataract extraction. In Group 1, ofloxacin 0.3% eyedrops were topically instilled ten times and the aqueous humor concentration was 2.73 +/- 0.88 microg/ml. In Group 2, ofloxacin 0.3% eyedrops were topically instilled six times and the aqueous humor concentration was 0.84 +/- 0.61 microg/ml. Aqueous humor concentration 12 hours after 200 mg oral dose in Group 3, was 0.38 +/- 0.12 microg/ml. In Group 4, patients were given ofloxacin as a single intravenous 200 mg dose and the aqueous humor concentration 2 hours after the end of infusion was 0.45 +/- 0.11 microg/ml. Concentrations were determined by high performance liquid chromatography (HPLC) with fluorescence detection. There was a significant difference between Group 1 and the other groups, but not between Group 2 and Groups 3, 4. It was concluded that ofloxacin penetrates the corneal and the blood-aqueous barriers and can achieve good aqueous levels when given topically and systematically. Ofloxacin can be applied topically for external bacterial infections such as conjunctivitis and keratitis. Systematically administered ofloxacin reached higher levels than the MIC for some bacteria which cause endophthalmitis.

We determined the linezolid concentrations in serum samples and aqueous humors (AHs) from 21 patients undergoing cataract extraction. Cataract removal was performed at various times (from 60 to 270 min) after the end of a 30-min infusion of 600 mg of linezolid. Serum samples were obtained 1 h after the end of linezolid administration to determine the maximum concentration of linezolid (C(max)); AHs and a second serum sample were taken simultaneously during the operation, and the concentrations of linezolid in AH (C(AH)) and serum (C(S)) were determined. The mean C(AH) 1 h after linezolid administration was 4.94 micro g/ml, and the mean ratio of C(AH) to C(S) (R = C(AH)/C(S)) was 0.43. All patients had a C(AH) of >2 micro g/ml, which was higher than the MIC at which 90% of Staphylococcus epidermidis strains are inhibited.

In the study presented here, levofloxacin concentrations in serum samples and the aqueous humour (AH) of 16 patients undergoing cataract extraction were measured in order to determine the penetration characteristics of levofloxacin into the AH of the non-inflamed human eye. Cataract removal was performed at various times (from 90 to 270 min) after the end of a 30-min intravenous infusion of 500 mg of levofloxacin. Serum samples were obtained 1 h after the end of levofloxacin administration (Cmax); AH and a second serum sample were taken simultaneously during the operation, and the concentrations of levofloxacin in AH (C(AH)) and serum (C(S)) were determined using a rapid high-performance liquid chromatography assay. The mean Cmax was 6.07 microg/ml (range 3.75-9.53 microg/ml, SD 1.83). The mean C(AH) at the first hour following levofloxacin administration was 1.37 microg/ml (range 1.17-1.6 microg/ml, SD 0.22) and the mean ratio (R=C(AH)/C(S)) was 0.26 (range 0.24-0.3, SD 0.02). The mean C(AH) at 125-270 min following levofloxacin administration was 1.39 microg/ml (range 0.82-1.98 microg/ml, SD 0.33) and the mean R was 0.3 (range 0.15-0.53, SD 0.11). Of 16 patients, 15 had a C(AH) of >1 microg/ml 1 h after levofloxacin administration. In conclusion, 1 h after administration of 500 mg of levofloxacin, the levels obtained were higher than the MIC at which 90% of methicillin-susceptible Staphylococcus aureus and certain gram-negative bacteria strains are inhibited.
