At clinically relevant concentrations, volatile anaesthetic agents influence neutrophil function. Our hypothesis was that sevoflurane would inhibit neutrophil apoptosis and consequently influence the postoperative pro-inflammatory state. In order to identify selectively the effect of the anaesthetic agent sevoflurane, we studied patients undergoing minimally stimulating (cataract) surgery randomly allocated to receive either sevoflurane (n = 11) or local anaesthesia (n = 12). Venous blood samples were taken immediately prior to anaesthesia and at 1, 8 and 24 h thereafter. The rate of neutrophil apoptosis, plasma concentration of cytokines and differential white cell count were measured. The rates of neutrophil apoptosis and plasma concentrations of IL-1beta, TNF-alpha and IL-8 at each time point were similar in the two groups. IL-6 concentrations increased significantly and to a similar extent compared to preanaesthetic levels at 8 and 24 h. This study demonstrates that sevoflurane does not influence the rate of neutrophil apoptosis, cytokine concentrations and neutrophil count following cataract surgery.

A 70-year-old woman previously exposed to hyaluronidase during ophthalmic surgery had excision of hypertrophic bulbar conjunctival tissue in the right eye. Hyaluronidase was added to the peribulbar anesthetic agent. Five days postoperatively, the patient presented with proptosis in the right eye, extraocular muscle restriction, and decreased visual acuity. She was apyrexial with a normal white cell count; computed tomography of the orbits showed a diffuse increase in soft tissue density. Clinical features were unchanged after 24 hours of intravenous antibiotics, but subsequent administration of high-dose oral steroids led to clinical improvement.

We report 2 cases of Cogan syndrome that developed after uneventful laser in situ keratomileusis. In the first case, an 8-month postoperative biomicroscopy revealed bilateral interface neovascularization, white intrastromal deposits, and anterior chamber cells and flare. In the second case, white cell infiltration and neovascularization were observed in the deep corneal stroma of the patient's right eye 18 months postoperatively. Based on these observations, which are consistent with typical interstitial keratitis, and the patients' history of Meniere-like disease, such as vertigo and mild hearing loss, Cogan syndrome was diagnosed in both patients. Topical steroids were prescribed. Intensive treatments with corneal irrigation and topical steroids showed effective outcomes in both cases.
