A 47-year-old factor VIII deficient haemophiliac successfully underwent bilateral cataract extraction. The use of cryoprecipitates to achieve haemostasis permitted retrobulbar anaesthesia and a peripheral iridectomy without complication. We consider that the use of cryoprecipitated factor VIII concentrate allows safe elective ocular surgery in patients with classic haemophilia.

Plasma exchange was performed on a patient with hemophilia A and inhibitor to factor VIIIC prior to bilateral cataract removal. Koate was used as replacement fluid with effective reduction of the inhibitor level. From the technical standpoint we found out that factor VIII is best reconstituted in water and directly infused through the venous line and not the centrifuge bowl.

We report the discovery of a circulating anticoagulant in a patient suffering from CREST syndrome. The patient was first seen with a microangiopathic haemolytic anaemia which led to the diagnosis of the CREST syndrome. Several months later, prior to a cataract operation, a routine coagulation screen (prothrombin time and partial thromboplastin time) was abnormal. Investigation showed the presence of a circulating anticoagulant as well as a decrease in several clotting factors, principally factor VIII C.

A 40-year-old patient with severe haemophilia A and an inhibitor against factor VIII underwent a cataract extraction under local anaesthesia. Recombinant activated factor VII was use to achieve haemostasis. The procedure was successful. Neither bleeding complications nor side effects occurred.
