A study of visual habits consisting of a questionaire and an examination was carried out on 70 unilaterally aphakic patients. These patients wore correcting glasses and had better vision in the cataract operated eye. It was observed that the patients, aided by senile convergence weakness on one side and senile divergence weakness on the other, were able to develop a form of monocular vision without complications and in particular without double vision. These patients who wore glasses developed a type of "multiple choice" vision, which was dependent on the intelligence and the determination of the individual.

The examination of visual acuity against the light may help the decision, whether or not a senile cataract should be operated on. We examined 84 eyes with different lens opacities, the visual acuity was analysed in relation to the axial, cortical and capsular cataract. We found in all cases a marked reduction of the visual acuity against the light. This has been general ophthalmological experience especially in the capsular cataract. This additional examination, we propose, is easily performed, and was given helpful information for all kinds of cataracts, where the indication for surgery was not clearcut.

Various surgical procedures were performed on albino rabbits, which impaired anterior segment blood circulation. Our early and late clinical observations showed a consequent development of anterior-segment ischaemia into anterior-segment necrosis. The significance of different surgical procedures for resulting complications is discussed. Histopathological observations showed ischaemic changes in the anterior segment: iris and ciliary body atrophy, cataract formation, rubeosis iridis. Even though changes in the anterior segment were not observed on biomicroscopic examination in the cases with ligation of one LPCA, histological studies revealed different degrees of sectoral iris atrophy with minor rubeosis.

The eyes of an 81-year-old man were obtained three and ten days after intracapsular extractions had been made through limbal incisions under limbal-based flaps of conjunctiva and Tenon's capsule. The wounds were closed with 7-0 chromic collagen. Before death from bronchopneumonia, the patient developed bilateral filtrations in association with violent coughing spells. In both specimens, the healing processes of the conjunctiva and Tenon's capsule were advanced, confining the escaping aqueous humor to the subepithelial tissues of the flap. In contrast, the stromal wounds were still inert in accordance with our earlier observations that these wounds are precariously weak during the first two weeks after surgery. In cataract surgery, flaps including both conjunctiva and Tenon's capsule should be prepared and closed to support the slower healing stromal wound. Prevention of filtrations is not a simple matter of using many sutures. This patient's eyes demonstrated that improperly placed or too tightly tied sutures may be directly responsible for leaking wounds.

A 69-year-old man with bilateral open-angle glaucoma and nonfiltering iridencleisis scars underwent combined cataract extraction and glaucoma filtering operations in each eye. Later, he again required intensive medical therapy including topically applied echothiophate iodide, epinephrine, and acetazolamide to control his glaucoma. Topical instillation of epinephrine repeatedly resulted in reversible cystoid macular edema in each eye. The macular changes were documented by fluorescein angiography. The clinical and angiographic features of the macular edema in this patient did not differ from cystoid macular edema seen in other ocular conditions.

A human cataract, removed in its capsule, and implanted in either the anterior or posterior aqueous chamber of a rabbit eye, provided a model system that simulated a clinical case of human senile cataract. Such a model can be used for practice surgery or for evaluation of an instrument designed to remove a cataract through a small incision.

In a study of 45 patients, those with monarticular and pauciarticular juvenile rheumatoid arthritis, and with antinuclear antibodies, were likely also to have chronic, persistent iridocyclitis. In cases where iridocyclitis was present, topical mydriatics were used to prevent formation of posterior synechiae. Topical corticosteroids were used but discontinued if the uveitis failed to respond. Despite the generally poor prognosis for the vision of these patients, surgery for complicated cataract was often successful.

This paper describes a 7-year-old girl with trichodysplasia, normal teeth, onychogryposis, hypohidrosis, psychomotor and growth retardation, dry and warm skin with follicular hyperkeratosis, pigmentary disturbances (hyper- and hypochromic spots), bilateral nuclear cataract, dermatoglyphic anomalies, and other signs. This condition is considered a new form of ectodermal dysplasia.

Retinal detachment after cataract extraction occurred in nine out of 136 eyes with myopia of 6 or more dioptress (6-7 per cent), during a follow-up period of 1 1/2-9 1/2 years. Five of the nine detachments occurred within 3 months of cataract extraction. All patients with retinal detachment were under the age of 63 years. Relatively young patients with high myopia bear a special risk of developing retinal detachment after lens extraction. The possible reason for this is discussed.
