In an attempt to assess the effect of posterior chamber intraocular lens (IOL) implantation on the outcome of combined extracapsular cataract extraction (ECCE) and trabeculectomy we compared 23 eyes subjected to ECCE and trabeculectomy with 23 eyes subjected to a triple procedure-that is, with the addition of IOL implantation. The results showed that IOL implantation did not have a detrimental effect on postoperative IOP reduction, gain in visual acuity, or needs for antiglaucoma medication. The incidence of anterior chamber reactions consisting of the development of posterior synechiae and fibrin formation was significantly higher when IOL had been implanted. However, the fibrin was generally absorbed within 14 days and the posterior synechiae did not occlude the visual axis. We conclude that IOL implantation should be included in these combined operations with the object of rehabilitating visual function.

An extracapsular cataract extraction (ECCE) with posterior-chamber-IOL was performed in 21 eyes of 20 patients with primary angle closure glaucoma (PACG; 2 suspects, 5 acute PACG, 14 chronic PACG). The ECCE was performed to improve the IOP. In 14 cases the ECCE was done in lieu of a filtering procedure. After the ECCE the anterior chamber deepened. The mean IOP was reduced from 31 mmHg preoperative to 16 mmHg postoperative. Only 5 eyes needed additional medication after the ECCE. The IOP was reduced even if extensive peripheral anterior synechiae were diagnosed or after failed filtering procedures. The only temporary complication was the IOP-peak in the immediate postoperative period. It is concluded that an ECCE with PC-IOL should be seriously considered as the procedure of choice in PACG instead of a filtering procedure (or instead of a combined procedure).

Thirty eyes of 29 patients (12 male and 17 female) with an age range 50-86 years (mean 76 years) were operated by combined technique for cataract and glaucoma with a follow-up ranging between 6-18 months (mean 6.8 months). Seventeen eyes had capsular glaucoma and 13 eyes simple glaucoma. The surgical technique of trabeculectomy, extracapsular cataract extraction and posterior chamber lens implantation is described. The pre-operative IOP was 31.5 mmHg +/- 7.8 on maximum tolerated medical therapy and the post-operative IOP 17.8 mmHg +/- 3.9. Fourteen eyes had to be treated with topical timolol X 2 daily after the operation. Twenty-eight eyes had a pre-operative visual acuity of less than or equal to 0.3. Of these, 17 had a visual acuity of less than or equal to 0.1, and were severely visually handicapped. Visual acuity improved markedly in 24 eyes and remained unchanged in 6 eyes. Every patient but one (with vitrectomy and no lens implantation) experienced considerable subjective visual increase. The main complications consisted of vitreous loss in one eye and fibrinous effusion into the anterior chamber in 7 capsular glaucoma eyes. Pre-operative precautions and post-operative care are important. The combined triple procedure is recommended for the management of patients with uncontrolled glaucoma and cataract.

Combined trabeculectomy and phacoemulsification was performed on 21 eyes with poorly controlled glaucoma (15 open-angle glaucomas, 5 capsular glaucomas, and one secondary glaucoma) and coexisting cataract. In 20 of these eyes there was severe glaucomatous damage of the optic nerve with visual field loss. With the procedure used by the present authors there was a higher incidence of choroidal detachments, whereas other complications occurred at a lower rate than reported by other authors combining cataract extraction and trabeculectomy. Mean IOP was lowered from 25.7 +/- 6.1 to 17.1 +/- 3.2 mmHg after 18 months. Thirteen patients (62%) required no medication, while 5 (24%) required additional therapy for sufficient control of IOP (less than or equal to 21 mmHg). The results indicate that the combined phacoemulsification and trabeculectomy procedure may be a useful approach in eyes with severe glaucomatous damage and cataract.

Two cases of ocular hypotony in myopic eyes following extracapsular surgery are described. A trampoline-like shrinkage of the posterior capsule and severe hypotony developed four and eight weeks after cataract extraction without IOL implantation. After discussion of the capsule by YAG-laser an IOP overshoot to more than 50 mmHg occurred, but three weeks later the IOP had dropped to normal values without further medical treatment. All transition stages to hypotony seem to be possible, thus explaining the well-known pressure-lowering effect of extracapsular cataract surgery can be understood.

Visualized cyclodialysis was carried out in 16 consecutive cases of operation-demanding glaucoma, where trabeculectomy was considered inexpedient. In 9 cases simultaneous cataract extraction was carried out. At follow-up 2-34 months (median = 15) post-operatively, the IOP had decreased from a median value of 32 mmHg (range 21-44) to a median value of 17 mmHg (range 7-28) in 15 out of 16 eyes. The 16th eye had to be eviscerated because of pain before and after an inefficient cyclodialysis. Five eyes were reoperated upon. Essential complications were 2 cases of central visual field impairment and 1 case of arteriovenous occlusion after cyclodialysis revision. We find visualized cyclodialysis a useful procedure for aphakic glaucoma as well as for re-operations. Frequent post-operative examinations are necessary for 1-2 months post-operatively.

Aqueous humor catecholamine levels were measured in 14 patients admitted to hospital for cataract or glaucoma surgery. Norepinephrine was detected in all patients. Epinephrine was detected in one patient who had received a preoperative retrobulbar injection of epinephrine. Dopamine was not detected in any patients. The highest level of norepinephrine was detected in cataract patients with normal intraocular pressures (mean = 7.18 nmol/l).

Argon laser iridotomy was performed in a total of 140 eyes of 104 patients with primary angle-closure glaucoma. A total of 69 eyes were treated by the long-burn technique and followed for an average period of 2.7 years; 71 eyes were treated by the short-burn technique and followed for an average period of 1.7 years. The effects on the IOP control and various factors of glaucoma were similar between both techniques, and the results of iridotomy were analyzed for all eyes. The IOP was controlled at or lower than 20 mmHg in 29% without medication, in 52% with reduced or the same medication as the preoperative treatments, and in 13% with increased medication. However, in 6% the IOP could not be controlled. The incidence of IOP control by iridotomy alone was significantly higher in eyes with peripheral anterior synechia (PAS) less than half the angle circumference than in eyes with more extensive PAS. Visual acuity loss occurred in 51%, and the loss by more than three lines of the acuity chart was seen in 19%; the visual loss was thought to be due to cataract progression or development. No significant differences were found in the visual field, cup-disk ratio, the extent of PAS or tonographic C-value before and after iridotomy. The IOP control and visual acuity loss were comparable with those seen after surgical iridectomy. Because of the absence of significant complications and ease of performance, iridotomy may replace surgical iridectomy in the treatment of angle-closure glaucoma.

Timolol of 0.5% was applied in 6 eyes prior to cataract extraction, while 7 eyes were not treated. Aqueous humor was drawn after retrobulbar analgesia with 2% lidocaine and analyzed for epinephrine and norepinephrine using a radioenzymatic assay. Epinephrine levels averaged 15.8 +/- 16.2 and 19.3 +/- 27.5 pg/ml in the timolol treated group and controls respectively. The corresponding values for norepinephrine were 1.59 +/- 0.55 and 1.87 +/- 0.90 ng/ml. These differences were not statistically significant. Norepinephrine levels in this study were somewhat higher than in previous reports of catecholamine levels in human aqueous humor and considerably higher than plasma levels of norepinephrine. There was no significant correlation between aqueous humor norepinephrine levels and age in the two groups combined. The mean norepinephrine levels for men and women in the two groups combined were 1.73 +/- 0.89 and 1.76 +/- 0.56 ng/ml. The difference was not statistically significant. Timolol resulted in a decrease in the intraocular pressure.

In 78 eyes treated by different fistulizing procedures (57 by goniotrepanation or trabeculectomy, 13 by Elliot trepanation and 8 by iridencleisis) extracapsular cataract extraction with implantation of posterior chamber lenses was performed. A sector iridectomy had to be performed in about 40% of the eyes because of difficulties in dilatation of the pupils and posterior synechiae. Postoperatively the IOP was regulated in 83% of all eyes without additional topical medication; a second fistulizing procedure was only necessary in one eye. Complications such as keratopathies, posterior capsule fibrosis, and dislocations of the posterior chamber lenses were not found to be more frequent compared with nonglaucomatous eyes treated by extracapsular cataract extraction with implantation of posterior chamber lenses.
