In a series of 3,222
cataract
operations where enzymatic zonulolysis with Alphachymotrypsin was employed, the authors describe two complications which are thought to be due to the effects of the enzyme. They are the persistent iridoplegia in 18 cases (0.05) and the filamentary keratitis in 34 patients (1.05%). The clinical picture of an acute enzymatic intraocular pressure increase and the appearance of a persistent iridoplegia are discussed. The postoperative course of 141 eyes in 115 patients undergoing uncomplicated intracapsular
cataract
extraction was reviewed.
Alpha-chymotrypsin
as used in our institution did not increase the percentage of eyes developing significant postoperative ocular hypertension. Smaller amounts of enzyme and a shorter waiting period than have heretofore been advocated are adequate for clinically effective zonulysis and appear to be protective against "enzyme glaucoma." Indeed, we found that among non-diabetics, those receiving enzyme experienced a lower incidence of postoperative ocular hypertension than did those not receiving enzyme. The one factor in our series related to an increased percentage of postoperative hypertensive responses was diabetes mellitus.

Trypsin inhibition (reduction in benzoyl arginine p-nitroanilide hydrolysis), elastase inhibition (reduction in succinyl trialanyl p-nitroanilide hydrolysis), and chymotrypsin inhibition (reduction in acetyl tyrosine ethyl ester hydrolysis) by neutral extracts of mammalian lenses were estimated. compared to 1.46 +/- 0.57 in normals; antitryptic, 0.54 +/- 0.38 and 0.12 +/- 0.04; antichymotryptic, 1.03 +/- 0.61 and 0.297 +/- 0.055). Antielastase activity was distinctly higher in adult normal human lenses compared to infant lenses (0.159 +/- 0.068). Elastase- and trypsin-like activities were detected at low levels in all mammalian lenses.
Chymotrypsin
-like activity could not be observed in the lenses. The cataractous lenses had lower trypsin- and elastase-like activities compared to normal human lenses (elastase 1.20 +/- 0.643 in normal compared to 0.062 +/- 0.035 in
cataract
; trypsin, 0.367 +/- 0.154 and 0.069 +/- 0.038). The role of protease: inhibitor complexes in the expression of the individual activities and their role in cataractogenesis are discussed.

In a series of 1,000 consecutive intracapsular
cataract
extractions, the most important peroperative complications were rupture of the capsule in 6.5% of the cases and vitreous loss in 3.0%. The most important immediate postoperative complications were delay in wound closure in 2.7% of the cases, iritis in 2.9%, ocular hypertension in 3.4%, and some other complications. The late complications were an Irvine-Gass syndrome in 2.8% of the cases, an epithelial downgrowth in the anterior chamber in 0.6%, and a retinal detachment in 2%. alpha-
Chymotrypsin
often causes a temporary ocular hypertension (in at least 25.2% of the cases). The functional results are very good, when there is no pre-existing ocular pathology: in 90% of the cases the vision was 7/10 or more.
