Qatar is one of the rapidly developing Gulf States. Its life style is rapidly changing from a seminomadic to a more settled urban one. This is the first study of the causes of economic blindness (6/60) in Qatar. It embraces 3,072 patients (70% males and 30% females). The most common causes in those under 40 years of age were trachoma, infectious diseases, retinal diseases, trauma and congenital malformations. After 40 years, cataract and glucoma occupy the second and third commonest causes after trachoma. Thus, a proper antitrachoma campaign will cut down the incidence of blidness by 50%. Control of individual behavior and antenatal caution in prescribing medicines can reduce the risk of trauma and congenital malformations. Cataract is not preventable, but early detection of glaucoma delays the damage of ocular tissues. This is the same with systemic disease.

A 71-year-old woman complained of "smoky" vision, which was found to be caused by a hyphema with blood dripping from a vascular tuft located in the 12 o'clock meridian of the iris. Fluorescein angiography delineated vascular tufts and argon laser photocoagulation eradicated one of the tufts that bled. Histopathologic studies of iris obtained at the time of cataract extraction showed an aggregate of small vessels at the pupillary margin. Most patients with vascular tufts of the pupillary margin have no systemic disease but they are also observed in diabetes mellitus and myotonic dystrophy.

In 499 patients operated on for cataract, the clinical postoperative signs of extraocular inflammation (conjunctival hyperaemia, chemosis, discharge and oedema of the lids), the number of infiltrates around the corneoscleral sutures, and the severity of intraocular inflammation in the anterior chamber (aqueous-flare) were assessed on the fourth postoperative day and correlated with the bacterial conjunctival flora examined both qualitatively and quantitatively on the same day. Patients with potential pathogens (Staphylococcus aureus, gram-negative bacilli and streptococci) on the conjunctiva following operation did not show any increased inflammatory reactions when compared with those without such pathogen. The quantity of bacteria, i.e. number of colonies, did not appear to play a role. The reasons are discussed. The clinical postoperative inflammatory signs were further correlated with the following factors: surgical complications, quality of suturing technique, use of alpha chymotrypsin, systemic disease, sex and age. A positive correlation was found between the severity of extraocular inflammation and retained lens material and hyphaema. Furthermore, extraocular reactions were more severe in males than in females. The incidence of infiltrates around corneoscleral sutures was found to vary with age, i.e. occurred more frequently in patients less than 60 years. No relationship was found between the severity of aqueous-flare and the above mentioned factors.

The crystalline keratopathy of multiple myeloma may involve the corneal epithelium, but has not previously been described in a vortex epithelial distribution. Endocapsular hematomas have been described in the period immediately after extracapsular cataract extraction, but not later on or in association with systemic disease. We report a pseudophakic patient who developed a vortex epithelial crystalline keratopathy as a presenting sign of multiple myeloma, and who subsequently developed a spontaneous endocapsular hematoma.

Exfoliation syndrome (EXS) is a disorder which affects some structures of the eye. We studied the changes of the cornea in patients with EXS and compared with those in normal persons. A prospective study of 96 consecutive patients more than 70 years of age was set up. 48 of them (70 eyes, group A) had exfoliation in one or both eyes and 48 (group B) had no ocular disease other than senile cataract. None of the patients had any systemic disease. The endothelium and thickness of the central cornea were studied. Endothelium of the eyes with EXS showed significantly (p < 0.05) lower cell density than those of group B. Cornea in group A was significantly thicker (p < 0.05) than in group B. The morphology of the endothelium in group A showed a decrease of hexagonal cells and a higher rate of polymegethism compared to group B. Corneal thickness and endothelium showed no significant differences between the eyes with EXS and normal fellow eyes. These results add another risk factor, the fragile cornea, in eyes with EXS, in cataract surgery.

The clinical features, treatment, and visual outcome of 52 eyes from 43 patients who developed scleritis following surgery were reviewed. In all patients the scleral inflammation developed adjacent to a surgical wound. Ninety six per cent had necrotising disease and 23% also had evidence of secondary posterior scleritis. Many different types of ocular surgery were implicated and the majority (75%) of the patients had two or more surgical procedures before the onset of the scleritis. Although cataract extraction through a limbal incision resulted in the largest subgroup, scleritis also followed glaucoma, strabismus, and retinal detachment surgery. The latent period between surgery and the appearance of inflammation was short (mean 9 months) except for a small group in whom scleritis occurred many years after squint surgery. Sixty three per cent of patients had evidence of a systemic disease. Early diagnosis and aggressive medical treatment significantly improved the visual outcome. The precipitating factors, pathogenesis, and course of this condition are discussed.

Episcleritis and scleritis are two common inflammatory disorders encountered by primary care practitioners. Although episcleritis is a self-limited disease that usually resolves within 2 to 3 weeks, severe symptoms may require the use of topical steroids or, in recalcitrant cases, nonsteroidal anti-inflammatory agents (NSAIAs). Scleritis is a painful, generalized inflammation of the eye that is often associated with systemic disease. The necrotizing form of scleritis is the most serious, requiring aggressive treatment with NSAIAs and systemic steroids. Significant sequelae of scleritis include uveitis, glaucoma, cataract, and retinal detachment.

The activity of red blood cells galactose-1-p-uridyl transferase (GPUT) was determined by the spectrophotofluorometric method. The average GPUT activity in 102 control subjects was 13. 34 +/- 2. 03u. There were no differences in the sexes and ages. The average GPUT activity in 108 patients with congenital cataract was 11.58 +/- 4. 03u and was lower than that of the controls (P < 0.01). The GPUT activity in 14 of the 108(12.96%)patients was all-3SD below the mean value of the controls (< 7.24u) and the range was from 4.02u to 7.13u with the mean value of 5. 93u being about 44.45% of the normal. The difference between the two groups was statistically significant (P < 0.001). The results indicated that some of the patients with congenital cataract were due to decrease in the GPUT activity. The patients with decrease GPUT activity were mainly suffering from cataracts and their systemic disease were mild.

Lymphoma of the conjunctiva is rare. It presents in older patients as a mass lesion and usually remains localized. Surgery is limited to biopsy, and radiation therapy is the definitive treatment of choice. The entire conjunctiva is treated. Relatively high doses (approximately 30 Gy) are required for local control, which may lead to cataract formation. Twelve patients with conjunctival lymphoma were treated at the Massachusetts General Hospital between 1979 and 1988. Ten of 12 patients presented with a unilateral lesion; 2 of 12 with bilateral lesions. Two of 12 patients were found to have systemic disease at the time of presentation. One patient developed conjunctival lymphoma 5 years after the diagnosis of generalized disease. Using electron beam, all patients were treated with a single anterior circular field to total doses ranging from 24 Gy to 30 Gy delivered in 8 to 16 fractions over 9 to 20 days. In all cases, the lens was shielded by a specially designed plastic contact lens bearing a 12 mm diameter lead shield. The lens dose was determined at varying depths beneath the shield for 6 MeV and 9 MeV electron beams and ranged from a minimum of 5% to an absolute maximum of 18% of the total dose delivered to the tumor. Local control was maintained in all patients with follow-up to 9 1/2 years. One patient relapsed distantly 3 years after treatment. One of 12 patients died of systemic disease 4 years after treatment of the ocular lesion. Two patients developed cataracts 4 and 5 years after treatment; one had bilateral cataract, although only one eye had been treated. Both patients were over 75 years old. In both cases, the cataracts were felt to be senile cataracts which are ophthalmologically and radiographically distinguishable from radiation induced lesions.
