Many potential complications associated with retrobulbar anesthesia have been reported. Of these, sudden loss of vision in the contralateral eye is the least expected. We report a case of immediate loss of vision in the fellow eye after retrobulbar anesthesia for
cataract
extraction. Possible etiology such as emboli thrown during
ventricular fibrillation
, direct optic nerve anesthetic injection with reflux to the chiasm, cortical stroke, amaurosis fugax, arterial injection of anesthetic material, and hysteric reaction are discussed.

An 82-year-old ASA 2 patient underwent routine sub-Tenon's block for
cataract
surgery. One minute after injection of the local anaesthetic, the patient had a generalised tonic-clonic seizure and developed refractory
ventricular fibrillation
; subsequent resuscitation was unsuccessful. With no evidence for intravascular injection, the lack of structural brain abnormalities, and the most striking feature on post mortem examination being severe triple vessel coronary artery disease, it was concluded that this was primarily cardiac in origin; however, the possibility of brainstem anaesthesia should also be considered.

Type 1 myotonic dystrophy (DM1) is a hereditary neuromuscular disease affecting multiple organs in human adults. Here we report a 42-year-old man diagnosed with DM1. Having a history of progressive muscular weakness and gradual loss of visual acuity, he was referred to us by his ophthalmologist for risk assessment of undergoing
cataract
surgery. Cardiology workup revealed type 1 Brugada ECG pattern, positive late potentials and inducible
ventricular fibrillation
in an electrophysiology study. Literature review revealed that those ECG changes may be observed in DM1, suggesting that DM1 and Brugada syndrome may share a common pathophysiologic pathway.
