The concentration of alpha 2-macroglobulin, alpha 1-antitrypsin, plasminogen, C3-complement, fibrinogen degradation products (FDP) and fibrinolytic activity, were studied in the aqueous humour and serum from nine patients with Fuchs' endothelial dystrophy, 17 patients with uncomplicated senile
cataract
and in the secondary aqueous from six
cataract
patients. Finally, the aqueous humour and serum from two patients
anterior uveitis
were studied. An increased concentration of alpha 2-macroglobulin, alpha 1-antitrypsin, plasminogen and C3-complement was found in both the aqueous and the serum from patients with Fuchs' dystrophy when compared with the primary aqueous and serum from patients with
cataract
but this was only significant for alpha 1-antitrypsin in aqueous humour. A significant increase in the amount of FDP was found in the serum of the Fuchs' patients compared with the
cataract
patients. Fibrinolytic activity could not be demonstrated in the serum in any of the patient groups. The concentrations of the various factors found in the secondary aqueous of the
cataract
patients differed only slightly from the content of the primary aqueous of the Fuchs' patients.

In 71 patients that underwent intracapsular extraction of the
cataract
it was possible to collect aqueous humor (a.h.) in sufficient quantities at the beginning and at the end of the operation. Using radial immunodiffusion albumin, IgG and alpha-1-antitrypsin were measured in the samples. In secondary a.h. huge variations in the concentrations were apparent. The primary permeability of the blood-aqueous barrier and the time between sampling had no influence on the relative increase of the proteins. No statistically significant difference was found between normotensives and hypertensives and patients with or without myopia of higher degree (over 6 diopters). The relative increase of proteins according to their difference in molecular weight followed the same pattern as described for
anterior uveitis
. In patients for whom an enzymatic zonulolysis with trypsin was used for the extraction, the relative increase of IgG was significantly smaller than in those that did not receive this treatment (P less than 0.005). The possible sources contributing to the formation of secondary a.h. are discussed.

The ocular and systemic characteristics of 160 patients with
anterior uveitis
and seronegative juvenile rheumatoid arthritis are reviewed. Chronic uveitis occurred in 131 patients, 76% of whom were girls. Both eyes were involved in 70% of the cases. Band keratopathy occurred in 41% of the eyes,
cataract
in 42%, and secondary glaucoma in 19%. Only 11 patients had uveitis before the onset of arthritis. Notable correlations included a pauciarticular onset of arthritis in 95% of the patients, and positive tests for antinuclear antibody in 82%. Of 29 patients with acute
anterior uveitis
, 27 were boys. The inflammation responded well to therapy, and serious complications did not occur. At follow-up 21 patients had typical ankylosing spondylitis, and five had sacroiliitis. The incidence of positive results of tests for HLA-B27 antigen was 94%.

Aqueous humour and blood from twenty cataractous patients with chronic
anterior uveitis
and the monopauciarticular form of juvenile rheumatoid arthritis were tested for immunoglobulins G, A, and M and for autoantibodies, particularly those against nuclear antigens. The aqueous immunoglobulins were raised in the majority of patients even when the eyes were normal biomicroscopically. Raised IgG antinuclear antibodies were found in 85% of the aqueous samples. Information obtained from the study of aqueous humour from fourteen patients with senile
cataract
was used for statistical analysis of these data. The presence of high molecular weight immunoglobulins in the aqueous humour from patients with Still's disease suggests an abnormal blood-ocular barrier which may be responsible for the recurrence which follows an ostensibly treated primary attack of uveitis. The presence of antinuclear antibody in a few aqueous samples without a concomitant rise in the blood levels is suggestive of local antibody synthesis.

Prenatal and early postnatal devpelopment of eyes of Wistar rats with X-ray-induced
cataract
mutation was assessed histologically, to determine the events leading to
cataract
formation. A significant phenomenon appeared to be retinal folding, which occurred regularly at 14 to 15 days of gestation and might have pushed the lens against the cornea. A corneal reaction with the lens was indicated by corneal adhesions to the lens, seen frequently during and shortly after the period of retinal folding, and could have stimulated the epithelial hyperplasia that leads invariably to anterior polar
cataract
in these animals. Changes in the lens fiber cells, which could have been generated by the epithelial hyperplasia, included the sporadic detachment of newly developing fibers from the lens epithelium and the eventual swelling of the anterior ends of fibers still attached to the epithelium. The detached fibers became fusiform and developed postnatally into posterior, suture-associated vacuoles.
Anterior uveitis
was noted postnatally in some eyes and probably contributed to the subsequent development of the
cataract
. The results are discussed in the light of congenital anterior polar
cataract
formation reported in humans and other animals.

During a 5-year period, phacolytic uveitis was diagnosed in 202 eyes of 151 dogs admitted to the veterinary teaching hospital. The diagnosis of phacolytic uveitis was based on the finding of a cataractous lens and

, unassociated with other identifiable causes of uveal inflammation. The most commonly affected breeds were the Toy and Miniature Poodle (35%) and the American Cocker Spaniel (19%). The mean age was 7.0 years for all breeds, 5.1 years for the Cocker Spaniel, and 9.0 years for the Poodle breeds. Evidence of

resorption was visible in 72% of the eyes. Fifty-one dogs were affected bilaterally and 100 dogs unilaterally. The mean interval between recognition of the

Lens extraction surgery was done on 50 LIU-affected eyes, and on 35 normal eyes in LIU-affected animals. A 51-year-old man with ankylosing spondylitis had intracapsular
cataract
extraction and anterior chamber lens implantation in the left eye in 1985. He had localized sclerokeratitis around the corneal section and a protracted
anterior uveitis
in the immediate postoperative period. He developed a spontaneous extrusion of the implant in 1991 (six years later). The extrusion was through a site 1.5 mm posterior to the previous surgical section. There was no corneal decompensation or corneal melt and no history of trauma. Signs of localized scleritis were present. It is suggested that anterior chamber lens implantation should be avoided under these circumstances.
