Cardiac rhythm was monitored in 219 patients who had ocular operations under general anaesthesia and in 15 patients who had cataract surgery under local analgesia. Of these, 140 were children having squint surgery. The high incidence of the oculo-cardiac reflex and the adequate protective effect of atropine 0-01 mg/kg given intravenously was confirmed. Isolated extrasystoles were common, particularly in older patients, and sustained ectopic cardiac arrhythmias were observed in 4-6 per cent of patients of all ages. The significance of these arrhythmias in relation to surgical stimulation and the anaesthetic drugs used is discussed.

Anterior segment ischemia changes can occur without detachment of any muscles. The most common cause of such ischemic changes of the anterior segment is the removal of too many rectus muscles in one operation. Twenty dog eyes and eight monkey eyes were subjected to the disinsertion and detachment of various combinations of extraocular muscles. They were sacrificed at intervals from 30 to 90 days. During the observation period, they were observed for gross and slit lamp changes. The enucleated eyes were studied microscopically for signs of ischemic and necrotic changes. Two patients who were studied, observed, and treated for anterior segment ischemia following muscle surgery are described. The changes which occur after muscle surgery are extensive and include corneal edema, cataract, chemosis, corneal changes, decreases in intraocular pressure, decreases in outflow or glaucoma and frank necrosis. The variables which lead to this reaction is described in detail. Also, some unanswered queries, such as the duration of the reaction and the time interval of the reaction after multiple muscle surgeries, are discussed.

Among 200 children who had received routine ophthalmic and orthoptic examinations as part of their evaluation by a multidisciplinary assessment team, it was found that there was a high incidence of ocular defects. These defects included refractive errors (98 children), squint (74 children), nystagmus (15 children), cataract, retinopathy and optic atrophy. This high incidence of ocular defects emphasized the need for the routine ophthalmic examination of developmentally handicapped children in order to detect and treat ocular conditions which might otherwise have gone undetected, and enabled the visual sense of these children to be evaluated in relationship to their general development.

A review of 473 patients operated upon as day cases indicates that such surgery is safe, effective, and practicable. The surgical procedures included strabismus and ptosis correction in 265 children; major oculoplastic surgery for subtotal full-thickness eyelid reconstruction; and intraocular surgery in adults for trabeculectomy, cataract extractions with or without simultaneous intraocular implant introduction, and cataract extractions combined with trabeculectomy. No modification in surgical technique was required for strabismus and ptosis surgery. Slight modification was necessary for cataract surgery, and certain innovations were made in oculoplastic surgery.

Observation of fifty patients submitted to primary Binkhorst intraocular lens insertion for uniocular cataract has shown that 88% regained 6/12 or better visual acuity in the operated eye. All but three patients with healthy fellow eyes and a good visual result from surgery regained orthotropia and some binocular functions promptly without squint surgery, but formal tests of stereoacuity were poorly performed. The best clinical guide to binocular performance for near vision was found to be Lang's pen test.

The problems of optical correction of unilateral traumatic aphakia are discussed. The treatment of choice is a combined Cataract-operation with intraocular lens implants. The operation must be done as soon as the eye is free of inflammation. Prescription of bifocal glasses in addition to pseudophakoi is necessary. With this method a "malignant" Strabismus can be avoided and binocular vision, stereopsis as well as minimal grade of aniseikonia can be achieved.

A study on the eyes of 39 embryos, which were removed from women who had contracted clinical or subclinical rubella during the first and beginning of the second trimester of gestation is presented. A virological and histological study was performed on the eyes. In four cases rubella virus was isolated from the eye tissues (10.3%) and in nine cases, histological evidence of rubella cataract was found (27.3%). These results suggest that the rubella infection, even in cases of subclinical infections in pregnant women in the first trimester, are highly dangerous (in our study--37.6%) to the eye of the fetus, and, therefore, early diagnosis is necessary in order to avoid the risk of the consequences of congenital rubella infections in the newborn.
