The progression of a monocular nuclear cataract was observed for thirty-two months in an eye which was apparently otherwise normal. Three weeks after uncomplicated cataract surgery the patient presented with an apparent detachment of the choroid which enlarged dramatically within a relatively short time and which was further complicated by a retinal detachment. The subsequent development of rubeosis iridis and increased intraocular pressure suggested some other etiology. Ultrasonography revealed a malignant melanoma which was confirmed following enucleation. Transillumination of all suspected postoperative choroidal detachments is recommended with more definitive examination when indicated.

In the last four years we have treated with the mobile argon-laser beam 25 cases of aphakic pupillary block glaucoma (p. b. g.). According to the time of manifestation of the p. b. g. the patients were divided into an "early" and a "late" group. In the "early" group (17 eyes) the glaucoma manifested itself 1--4 days after cataract extraction. In the "late" group (8 eyes) p. b. g. appeared from 4 weeks to 3 years after cataract extraction. In 14 of the 17 cases of the first group we achieved a prompt reformation of the anterior chamber and a considerable decrease of intraocular pressure immediately after laser application. In 3 cases of the first group, where the pupil was under relative or absolute mydriasis before argon-laser treatment, the mobile laser beam failed to produce satisfactory results. In all of the 8 patients of the "late" group a reformation of the anterior chamber was achieved, but in only 4 of them did i. o. pressure normalize owing to extensive anterior synechiae. Laser application left either very slight or no traces at all on the iris. In order to ascertain the actual effect of the laser beam upon the iris, we applied laser under identical conditions in eyes which had no glaucoma but were to undergo enucleation because of choroidal melanoma. The histological examination revealed a slight degree of hyperaemia of the iris as an oedema of the connective tissue, while the iris sphincter did not appear to be affected by laser treatment.

The aqueous humor and serum of patients with various ocular diseases were analyzed for the occurrence of rheumatoid factor or immune complexes. Rheumatoid factor was detected by its direct agglutinating activity on IgG-coated particles (latex) and immune complexes were detected by their ability to inhibit exogenous rheumatoid factor or the subunit q of the first component of complement (Clq). Rheumatoid factor or inhibitory activity were found in the aqueous humor of nine of 11 patients with Fuchs' syndrome. In some patients who were examined two or three times, agglutination alternated with inhibition. Among 13 patients with endogeous uveitis, five had either agglutinating or inhibitory activities in their aqueous humor. The tests were also positive in the aqueous humor of three of seven patients with choroidal melanoma. A high titer of rheumatoid factor was found in the aqueous humor in an accidentally perforated eye that resulted in sympathetic ophthalmitis. No inhibitory or agglutinating activity was detected in the aqueous humor of 20 patients with senile immature cataract.

A case of multilobed malignant melanoma with a haemorrhagic retinal detachment which mimicked a postoperative choroidal detachment is described. Because the lesion showed no resolution six weeks after cataract surgery a tumour evaluation was undertaken. The diagnosis was established with a positive radioactive phosphorus uptake (32P) test and biopsy of the intraocular mass. A suggested approach to the diagnosis in such difficult cases is proposed.

Seventy-two cases of malignant melanoma of the iris treated surgically have been reviewed from Wilmer Institute records with respect to the age, sex, and race of the patients and the clinical and histologic results. Clinical features studied include location and extent of tumor, duration, presence of preexisting lesions, and associated features such as hyphema, glaucoma, cataract, pupillary distortion, and vascularity of the tumor. Surgical therapy included iridectomy, iridocyclectomy and enucleation. Complications of iridectomy included cases of hyphema, cataract, wound dehiscence and episcleral seeding of the tumor. Complications of iridocyclectomy included cases of vitreous loss, cataract, high astigmatism and incomplete excision. Histopathologic features were studied, and the relationship of cell type and tumor cohesiveness to tumor behavior was stressed. Tumor deaths were observed in two of the 72 cases. Clinicopathologic and statistical features in this series of 72 patients were compared with those previously reported series of iris melanomas, and the therapy of these lesions was discussed.

We examined the records of 53 patients treated for choroidal melanoma between 1985 and 1989. The aim of this study was to assess the safety and short-term results of iodine-125 episcleral plaque therapy. There were 28 males and 25 females, aged 20 to 77 years (median 61 years), treated for single tumours with a median diameter of 9 mm (range 5 to 15 mm) and with a median thickness of 4 mm (range 2 to 10 mm). The plaques containing iodine-125 seeds were chosen according to tumour size: 10 mm (16 patients); 15 mm (36 patients); 20 mm (one patient). All patients are alive at last follow-up (median 1.3 years, range 4 months to 3.3 years). Four patients underwent enucleation for melanoma progression. Thirty patients have developed some type of complication (more than one complication occurred in the same eye in 12 patients): retinitis (19), optic neuropathy (7); cataract (4), rubeosis iridis (2). Overall, visual acuity deteriorated in 32 patients, remained stable in 12 patients and improved in 9 patients. Iodine-125 plaque therapy appears to offer patients good prospects of tumour control and preservation of useful vision.

We evaluated visual outcome and risk of metastases in patients who underwent cataract extraction after protonbeam irradiation of a uveal melanoma. A total of 84 patients underwent cataract extraction between 2 months and 11 years after irradiation. One year after cataract extraction, approximately half of the patients had visual acuity of 20/100 or better, and approximately one third had an acuity of 20/40 or better. Larger tumor size was highly correlated with poor visual outcome 1 year after extraction. Six patients underwent enucleation after cataract removal, five due to blind, painful eyes and one due to continued growth of a previously undiagnosed ring melanoma. The rate of metastases was not higher among patients who underwent cataract extraction (adjusted rate ratio, 0.83). Results suggest that cataract extraction offers improvement of vision in selected eyes previously irradiated for a uveal melanoma, without adding to the risk of metastases among patients undergoing the procedure.

A previous observation of increased aqueous magnesium in eyes with senile cataract has been re-examined in a totally different material. The serum and aqueous level of calcium and magnesium from 34 patients with senile cataract were tested against comparable values from 4 patients with choroidal melanoma. In contrast to calcium, the magnesium aqueous/serum ratio was significantly higher in the cataract group. Possible explanations are suggested.

Ten patients with large melanomas and one patient with recurrent retinoblastoma were treated with combined localized current field (LCF) hyperthermia and iodine 125 irradiation delivered by episcleral plaque. Tumors were heated to 43 degrees to 45 degrees C for 28 to 45 minutes. Localized current field hyperthermia when combined with irradiation appeared to induce rapid tumor necrosis. One eye enucleated 17 hours after treatment showed only focal necrosis of the melanoma, while another eye demonstrated extensive necrosis 60 hours after treatment. In all remaining eyes, tumor regression occurred within the first month of treatment. Complications included cataract formation in six eyes, hemorrhagic retinal detachment in five eyes, and phthisis in two eyes. Complications from combined therapy of large intraocular tumors in this series appeared to result from the rapid necrosis of the tumor and secondary intraocular inflammation. Intraocular temperature dosimetry measurements demonstrated a temperature gradient of not more than -0.23 degrees C/mm-1 per axial millimeter from the episcleral plaque surface to the apex of the tumor. The authors believe that LCF hyperthermia could be a suitable means of application of hyperthermia in patients with intraocular tumors if further modifications were performed to reduce ocular complications.

An 82 year old patient was lost to follow-up having been diagnosed both clinically and on CT scan as having a right choroidal malignant melanoma. Twenty months later she presented with a painful right proptosis, dense cataract and raised intraocular pressure. A second CT scan showed orbital cellulitis with no evidence of the previous mass. Following enucleation histopathological examination revealed that the tumour had undergone total spontaneous necrosis. The implications of the CT scan findings are discussed. Five cases of necrosed choroidal malignant melanoma producing proptosis in the absence of extrascleral tumour extension have now been described.
