The present paper reports on experience gathered with the first 140 implantations of the Kelman II anterior chamber lens following intracapsular cataract extraction (ICCE). The technique of ICCE combined with lens implantation is described; the possible complications and the advantages of this new lens type are pointed out. With smooth implantation there have so far been no serious complications due to the lens, such as keratopathy, lens dislocation, secondary glaucoma, chronic iritis or membrane inflammation. The IC technique excludes the possibility of secondary cataract.

A retrospective study of 36 second eyes with cataract, who have undergone prophylactic retinocryocoagulation prior to cataract extraction, is presented. All 36 patients suffered an aphakic retinal detachment in the first eye in the past. Postoperative complications included iritis, transient cycloplegia and retinal hemorrhages; two patients developed retinal tears following cataract surgery. During the average followup period of 8.4 years, no retinal detachment occurred among this group of patients. This study strongly supports the value of preventive treatment (retinocryocoagulation) of the second eyes prior to cataract extraction, which have a high risk of aphakic retinal detachment.

From a clinico-anatomical point of view the implantation of IOLs with reversed optics approximates natural conditions and has many advantages. The distance between the endothelium or the pigment epithelium of the iris and the front surface of the lens is greater. The refracting surface of the lens is nearer to the nodal point and aniseikonia is reduced. However, it is still not known whether YAG laser treatment to open the posterior capsule is possible under these conditions. The present authors used the pulsed OPL3 neodymium-YAG laser ( Meditec ) to treat 45 eyes for secondary opacification of an intact posterior capsule after planned extracapsular cataract extraction. (A: 15 eyes with reversed-optic IOLs, B: 15 eyes with other types of IOL, C: 15 eyes with extracapsular aphakia). After performing a central capsulotomy with a mean of 44 shots, visual acuity increased immediately (mean improvement in visual acuity determined before and one week after YAG laser treatment - A:6 Snellen lines, B: 3 Snellen lines, C: 5 Snellen lines). In no eye was the post-laser visual acuity lower than before laser treatment. Even though the eyes were treated prophylactically with timolol 0.25% for 3 days, intraocular pressure - measured on the first postoperative day - increased insignificantly (mean: A: 1 mmHg, B and C: 2 mmHg). In 9 cases a mild iritis was observed, while 2 eyes had a hemorrhage of the iris vessels. Symptoms resolved within five days after laser treatment in all cases. The study demonstrates that Nd-YAG laser capsulotomy can be performed in patients with reversed optic IOLs without any disadvantage as compared to other types of IOL.

Nine hundred and thirty-eight consecutive primary intraocular lens (IOL) implant cases performed at The Wilmer Institute during the past six years by two surgeons were reviewed. Progressive corneal endothelial cell loss and an increased rate of late-onset corneal oedema were detected in the group with the Morcher (European) four-loop iris-clip Binkhorst IOLs. A higher rate of clinically-significant cystoid macular oedema developed in eyes with the American-manufactured four-loop Binkhorst lens, presumably from chronic iritis caused by contaminants on the IOLs. Our short-term results with the American-manufactured Shearing-type IOLs implanted at the time of extracapsular cataract extraction are encouraging, with 96% of cases achieving 20/40 vision or better, excluding those with known macular degeneration or amblyopia. Analysis of data submitted to the United States Food and Drug Administration (FDA) indicates that most of the four classes of intraocular lenses (anterior chamber, pupillary-supported, iridocapsular, and posterior chamber) are safe and effective at one-year follow-up. During the past three years there has been a gradual shift of preference in the type of IOLs being implanted--away from pupillary-supported and iridocapsular IOLs and, in part, from anterior chamber IOLs, to posterior chamber IOLs. A continued conservative approach to IOL implantation is recommended.

Progressive corneal endothelial-cell loss and an increased rate of late-onset corneal edema were detected in the group with the Morcher (European) four-loop iris-clip Binkhorst IOLs. Clinically significant cystoid macular edema was more frequent in eyes with the American four-loop Binkhorst lens, presumably due to chronic iritis caused by contaminants on the IOLs. Our short-term results with the American Shearing-type IOLs implanted at the time of extracapsular cataract extraction are encouraging, with 98% of the cases achieving 20/40 vision or better, excluding those with known macular degeneration or amblyopia. A continued cautious approach to IOL implantation is recommended.

On the basis of a screening of 6226 patients in western Nepal the most common causes of disease were established. Refraction anomalies and cataract were both found in over 20 percent of the patients, conjunctivitis/dacryocystitis and corneal opacities each in more than 10 percent. Lid conditions, trachoma, pterygium, vitamin A deficiency, glaucoma, iritis, diseases of the fundus and phtisis bulbi were each seen in more than 1 percent of the patients. Problems of treatment are described.

Between 1977 and the end of August 1980 150 anterior chamber lenses were implanted at the Basel Eye Clinic. Ninety six eyes were followed up for more than three months after intracapsular cataract extraction and implantation of a Binkhorst 4 loop lens. The indications for the operation are reviewed, the operative technique and postoperative management are briefly mentioned and the complications are discussed. One lens was removed because of hypopyon iritis and four lenses became subluxated but were all repositioned without resorting to surgery. The most worrying complication was cystoid macular edema, observed in 8% of cases and always associated with iritis. So far no patients have developed permanent corneal edema. It is important that lens implantation is only attempted when one is fully conversant with anterior segment surgery; having mastered the technique of implantation one should perfect one method before trying out another. The question of intracapsular versus extracapsular extraction and the type of lens used, provided one chooses a currently available model, is probably of secondary importance. Clearly defined indications and a detailed discussion with the patient are prerequisites. Despite this, an anterior chamber lens represents a very dubious benefit for certain patients.

A retrospective study of 106 eye which have undergone prophylactic circumferential cryopexy is presented. The complications involving the anterior segment were iritis, transient cycloplegia, as fixed dilated pupil, and cataract formation. Those involving the posterior segment were vitreous, subchoroidal and retinal haemorrhage, and preretinal fibrosis. The incidence of retinal detachment was 7.5% (8/106) after an average follow-up of 33 months. The importance of a confluent cryopexy choroidoretinal reaction is demonstrated. The indications are reviewed and a rationale for the technique of application is proposed.

Report of six patients with corneal lesions due to abuse of local anesthetics. Depending on the duration and frequency of application of local anesthetics and individual disposition, the following lesions occur: superficial punctate keratitis, corneal erosions, edema of epithelium and stroma, stromal infiltrates, stromal defects, iritis, cataract, secondary glaucoma. During the prolonged period of healing following a stromal affection, bullae of the epithelium and erosions occur at these sites repeatedly in some cases; vascularization and leukoma may also occur. The most important local anesthetics currently in use are mentioned. Local anesthetics for the eye should only be administered by the physician.
