The characteristics of viral-bacterial lesion in the appendix of children with appendicitis are given by means of histologic, histochemical, immunomorphological and morphometric methods. Antigens of influenza viruses A, B, C, entero-, adeno- and paramyxoviruses were observed among which influenza C was found significantly more frequently (64.1%). Viral infection in the majority of cases (79.5%) was found in the association with opportunistic flora, more frequently with E. coli, and less R. aeruginosa and Klebsiella. Analysis of the immunomorphological shifts in the appendix indicated a peculiar dynamics of the antibody production in
acute appendicitis
: the enhancement of the IgG, IgD and
IgE
production and the decrease of the production of IgA and IgM. The viral-bacterial effect, the development of the immune responses with
IgE
hyperproduction and participation in this process of labrocytes and endocrine cells suggest the integration of the two pathogenetic mechanisms-infectious and allergic.

A 20-year-old African female was hospitalized several times for diffuse chronic abdominal pain. The following exclusions were made: Acute adnexitis (by laparoscopy),
acute appendicitis
(by appendectomy), gastric ulcerations (by esophagogastroduodenoscopy) as well as Crohn's disease and ulcerative colitis. However, once taking a closer microscopical look at the mucosa, that otherwise appeared colonoscopically to be normal, multiple eggs of schistosomiasis mansoni (S. mansoni) were found in the colon as well as the rectum. Thus, the diagnosis of an intestinal bilharziosis was finely established. In retrospect even the sample taken for the appendix could have indicated this diagnosis already earlier on. Both the antibodies (ELISA/IFAT) and the specific immunoglobulins (
IgE
) for S. mansoni proved significantly positive. Therapy of choice was a single oral dosage of praziquantel. Migration and tourism have considerably increased the range of tropical and infectious diseases that need to be included into differential diagnosis. This case report focuses on intestinal bilharziosis as a potential underlying cause of chronic abdominal pain in immigrants of endemically affected areas. Direct diagnosis is the most important diagnostic method. The adult worms are usually inaccessible, so the method of choice to assess both diagnosis and the degree of activity of a chronic infection is evidence of living eggs in the stool. Alternatively, in case of lack of direct evidence diagnosis can be established by endoscopy and rectal biopsy.

Anisakidosis is the infestation of man by the larval stage of the parasite Anisakis simplex. A history of ingestion of raw or undercooked fish up to 48-72 hours before the onset is highly orientative. Based on the location of the lesions, various types of human anisakidosis have been identified: gastric, intestinal and luminal forms constitute the majority of cases and symptoms often mimic a variety of acute conditions such as gastritis,
acute appendicitis
, diverticulitis, or ileitis. We present a case of a young woman admitted to our department with acute abdominal pain. A history of raw fish ingestion was recorded. The patient was submitted to explorative laparoscopy in which an oedematous, thickened segment of ileum was observed. Therefore a small laparotomy was performed, followed by resection of the thickened ileum. The histological examination revealed transmural inflammation with marked infiltration of eosinophils. High levels of total
IgE
and Anisakis simplex-specific
IgE
were recorded. Anisakidosis is an underestimated cause of acute abdomen, and should be taken into consideration as a differential diagnosis in these conditions. Nevertheless, in most cases, the correct approach could prove difficult, because surgery may be necessary in these settings, allowing the diagnosis only by histological and parasitological investigation of the resected specimen.

treated with ciprofloxacin, immediately developed multiple erythmatous papules. Reaction subsided after withholding ciprofloxacin and treatment with dexamethasone and chlorpheneramine maleate. It was developed again when treated with levofloxacin and subsided after withdrawal.

As all the fluoroquinolones have similar core structure, hypersensitivity to one may have cross sensitivity to other fluoroquinolones. It is advisable to avoid other fluoroquinolones and switch over to other group of antibiotics when hypersensitivity to one occurs.
