The authors studied the data concerning 101 patients who had undergone erroneous laparotomy for suspected acute surgical disease; these accounted for 0.4% of all the patients who were operated on for emergency indications in the same period. Eleven patients died. The operation was undertaken for an erroneous diagnosis of
acute appendicitis
(32 patients), acute cholecystitis (18), perforating gastric ulcer (15), peritonitis of unknown etiology (14), acute intestinal obstruction (5),
strangulated hernia
(3), destructive pancreatitis (3), tumor of the large intestine complicated by obstruction (3), abdominal abscess (2), thrombosis of the mesenteric vessels (1), ovarian apoplexy (1), closed abdominal trauma with injury to the viscera (4 patients). Diseases simulating the clinical picture of "acute abdomen" but not requiring an emergency operation were as follows: female reproductive (20 patients), pancreatic (11), renal diseases (11), hepatitis, cirrhosis of the liver (10), cardiovascular (9), pulmonary diseases (5), mesoadenitis (5), Crohn's disease (3), chronic colitis (3), carcinomatosis of the peritoneum (3), herpes zoster (3), and other diseases and injuries (20 patients). The main causes of the diagnostic and tactical errors were objective difficulties in the differential diagnosis due to similar symptomatology, as well as errors in the examination of the patient and haste in making a decision to make an operation.

The purpose of this study was to estimate the Incidence of visceral surgical emergencies In children and to determine the main causes and their prognoses. A retrospective study was carried out among children ranging from 1 month to 15 years who underwent emergency visceral surgery in the pediatric surgery unit of the University Hospital Center in Brazzaville, Congo. A total of 185 of the 206 children (14.1%) admitted for emergency visceral surgery were included In the study. The study population was predominantly male (67%). Most patients (71.3%) were over 5 years of age. The main indications for visceral surgery were
acute appendicitis
(30.3%), peritonitis (28.1%),
strangulated hernia
(22.2%), abdominal contusion (7.6%), intussusception (6.4%), other causes of intestinal obstruction (2.7%), and abdominal wounds (2.7%). Peritonitis was due to ruptured appendix in 76.9% of cases. Strangulated hernias were inguinal in 70.7% of cases and wnbilical in 29.3%. Intussusception was idiopathic in all cases and usually observed in infants under 6 months of age (58.3%). Postoperative recovery was uneventful in 79.5% of cases and complicated in 16.2%. EIght deaths (4.3%) were recorded. For the patient that died, the delay between initial symptoms and admission was longer than 3 days in 87.5% of cases and the interval for surgical treatment was longer than 6 hours in all cases. Early diagnosis and prompt surgical treatment are favorable prognostic factors for the outcome of emergency visceral surgery.

Inguinal hernia with

known as Amyand's hernia is uncommon. It may clinically manifest as acute scrotum, inguinal lymphadenitis, or
strangulated hernia
. The presentation of Amyand's hernia with acute scrotum has been rarely described. Also, the manifestation of infarcted omentum in the inguinal hernia has been described in one case previously. However, the coexistence of perforated appendix with infarcted omentum in the hernia sac which manifests acute scrotum has not been described previously. Herein, we described a case of a 5-year-old boy, admitted with right tense, painful, and erythematous scrotum in the emergency room. The diagnosis of herniated appendicitis was performed preoperatively by ultrasound. Moreover, the ischemic omentum was confirmed during surgery.
