In the clinic 307 patients with suppurative lesions were observed, in 183 of them (the main group) staphylococcic anatoxin was administered with prophylactic and therapeutic purposes, 124 patients (the control group) were treated with antibiotics, including 231 patients with acute appendicitis, 23--with osteomyelitis, 48--with mastitis and 5 patients with various purulent complications of staphylococcic etiology. Among 231 patients with acute appendicitis 125 comprised the main group who were injected staphylococcic anatoxin, and 106--the control group. In the latter the incidence of the operative wound suppuration was observed thrice as frequently.

A case of right iliac osteomyelitis initially misdiagnosed and treated as acute appendicitis is reported. Deep-seated right lower quadrant pain persisted and a gluteal abscess appeared in the immediate post-operative period. The gluteal abscess was incised and it continued to discharge pus until appropriate diagnosis and treatment was instituted. Pain due to iliac osteomyelitis is deep-seated and may radiate to the thighs or lumbar region. Compression and distraction of the pelvis elicits pain in the affected ilium.

The author describes two case-histories of profoundly different diseases which imitated in a similar way the symptoms of acute appendicitis. In both instances the authors had to operate under pressure of the local finding, although in the case of osteomyelitis of the ilium this diagnosis was contemplated before operation. The author draws attention to some specific features of work in developing countries.

The study covers 125 patients with anaerobic surgical infection, aged 7 to 82 years. Of the total 27 cases are operated for acute cholecystitis, four--diffuse acute peritonitis, seventeen--acute appendicitis, and three--acute hematogenous osteomyelitis. In acute cholecystitis and acute appendicitis microbiological study is carried out of the content, organic wall and periorganic space. In acute cholecystitis patients anaerobic flora is found in 39.01 per cent, and gram-negative--in 44.9 per cent, and in those presenting acute appendicitis--in 28.3 per cent and 58 per cent, respectively. The clinical analysis results point to a severer clinical course in the patients presenting anaerobic flora. The letter becomes manifest as mono infection in 37.2 per cent. It is pointed out that in the presence of two or more signs, characteristic of anaerobic infection, namely: destructive early process, offensive odor and intoxication, anaerobic bacterial flora should be invariably considered. At each microbiological examination the results of staining according to Gram should be also demanded from the laboratory.

The misdiagnosis of acute appendicitis is not uncommon. Rarely does infection of the triradiate cartilage imitate this entity. This case highlights an uncommon presentation of acetabular osteomyelitis as acute appendicitis and the severity of its sequelae. Like septic arthritis of the hip, acute appendicitis overtreatment is acceptable in part because of the complications resulting from delayed diagnosis and treatment. However, this case demonstrates the need to consider pelvic osteomyelitis and peripelvic infection in the differential diagnosis of appendicitis.

There is a continuum between Athletic (Sports) Hernia, Osteitis Pubis, and Osteomyelitis Pubis. The pubis is the site of attachment of many "core" muscles. A lay term used to describe a hernia is "rupture". Athletic hernia denotes a tear. Chronic musculotendinous strain may cause inflammation (osteitis pubis). An inflammatory focus may become a nidus for infection (osteomyelitis pubis). The symptoms caused by these three entities blur one with the other and with those characterizing acute appendicitis. This is an important association for clinicians to know.
