A rare case report of leiomyosarcoma of Meckel's diverticulum, which caused torsion and strangulation of the diverticulum, is presented. The clinical picture was similar to
acute appendicitis
, which was the working diagnosis when the 42-yr-old Arab woman was sent to surgery. Although rare, leiomyosarcoma is the most common
tumor
of Meckel's diverticulum, and has to be considered in the differential diagnosis of right lower abdominal pain.

The data on 80 cases of ovarian dysgerminoma are presented. The average age of patients was 19.1. In 9 out of 18 menstruation--free patients dysgerminoma was associated with genital infantilism and intersexuality. Pain was the most frequent symptom (70%). In 82%, correct diagnosis was established during primary examination. Among the most frequent misdiagnoses were
acute appendicitis
or pregnancy. Stage I
tumor
was established in nearly 70%. Unilateral involvement was registered in 83.5% of cases. Metastatic spread was limited to peritoneal cavity in 19.2%, paraaortic lymph nodes were involved in 16.4%. Five-year survival rate was 71.4%. Such characteristics of
tumor
process as stage and size of
neoplasm
proved to be of the highest prognostic value. The choice of a treatment modality was found to produce an insignificant effect on the results of therapy.

Three cases of adenocarcinoma of the appendix are reported. All three patients presented with
acute appendicitis
and the tumors were diagnosed only on histologic examination of the excised appendix. The first patient subsequently had a right hemicolectomy and was proven to have a Dukes' B
tumor
. The second patient probably had a Dukes' B also, but no further surgery was performed because of advanced presenile dementia. Advanced disease was found in the third patient. Analysis of 145 cases reported over the last ten years suggests that, unless the
tumor
is in Dukes' A stage, right hemicolectomy should be carried out if the patient is fit for radical surgery. The overall prognosis appears to be the same as that for carcinoma of the colon.

A 66-year-old man was admitted because of right lower abdominal pain and was operated on for
acute appendicitis
on March 12, 1983. It was found that his appendix was not inflamed, but a bleeding
tumor
was encountered in the posterior segment of the liver. Bleeding was controlled by suturing the liver above the
tumor
. Postoperative celiac angiography revealed many daughter tumors scattered in, but restricted to, the right lobe of the liver. The patient was treated by transcatheter arterial embolization combined with transcatheter arterial infusion of lipiodol and Adriamycin on April 25. Subsequently, a curative operation (right hemihepatectomy) was performed on May 24. The patient is still alive without recurrence 6 months after the resection. In case of ruptured hepatocellular carcinoma, it appears to be better to perform a two-stage operation with careful estimation of functional reserve of the liver, and to conduct transcatheter arterial embolization between operations to prevent the growth of the
tumor
during that period.

Regarding the event of an adenocarcinoma of the colic type of the cecal appendix, operated on at the ABC Medical College Hospital, the authors summarize the subject stating that, among the appendiceal carcinoma, this particular one tends to spread, by the veins or lymphatics, besides spreading by contiguity. This carcinoma hardly presents a symptomology of its own. It appears fairly often as
acute appendicitis
. The authors also state the difficulty for a macroscopic diagnosis. Thus, its real nature is determined, in general, only after the histologic exam of the removed part. The right hemicolectomy is the most indicated surgery and best results are obtained in the first surgery or 30 days afterwards. About the reported case the patient was operated on with the pre and intra operatory diagnosis of appendicitis and the histologic exam, on top of confirming it, pointed out the presence of
neoplasm
. After the indication for reoperation, the prompt spreading of the
neoplastic disease
offered no means to perform any other surgery disclosing its malignant potential.

is an exceptional case of Crohn's disease. Local recurrence, severe abdominal pain, diarrhoea, retardation of growth and development, and risk of malignant change may be reasons for elective surgery. As development of recurrence after operation is frequent and the results of colectomy and proctocolectomy with ileostomy are not always satisfactory some caution to surgery in ulcerative colitis and Crohn's disease in childhood is advisable.

Carcinoid tumors of the appendix are common incidental findings, but appendiceal tumors with histologic features of both carcinoids and adenocarcinomas are rare, and their biologic behavior is still unclear. We studied 10 such cases among 45 appendiceal tumors seen at Yale-New Haven Hospital between 1960 and 1982. The patients, ranging in age from 23 to 65 yr, were all clinically symptomatic [
acute appendicitis
(5); abdominal mass (5)]. Right colectomy was performed in 5 patients; the other 5 underwent appendectomy only. In 1 case, metastasis to a lymph node was detected; 2 patients had ovarian metastases, and 4 patients had cecal invasion. One of the 10 patients died of widely disseminated
tumor
, 2 are living with persistent disease, and 5 remained free of disease from 1 to 5 yr after initial surgery. Two cases were lost to follow-up. We conclude that the histology of these lesions is distinctive, enabling their differentiation from ordinary carcinoids. Because these lesions behave in a clinically more aggressive fashion than the usual appendiceal carcinoids, but are less virulent than adenocarcinoma, we support their classification as adenocarcinoids.

Most patients presenting an inflammatory mass of the right iliac fossa have a medical history suggestive of an
acute appendicitis
the weeks before and further investigations will confirm the diagnosis of an appendicular abscess. During the last 2 years, we have investigated and treated seven adults presenting such a mass. In a diagnostic purpose, but in way to treat the suspected abscess in the acute phase with a percutaneous drainage too, we have performed a CT of the abdomen and of the pelvis in every case. Only 3 of 7 patients had in fact a true appendicular pathology. One patient had a cecal diverticulitis with severe peri-diverticulitis, 3 other patients had a malignant lesion developed from a small intraluminal
tumor
but with a wide extension outside the limit of the cecum. These examples show that careful investigations are mandatory in all cases of apparent inflammatory mass of the right iliac fossa.
