We report a case of a leiomyosarcoma arising in a Meckel's diverticulum in a man of 90 years, with multiple ileal diverticulosis. The clinical picture was similar to acute appendicitis and diagnosis was not made until resection of an abdominal mass with histological appearance of leiomyosarcoma. 59 cases of leiomyosarcoma of Meckel's diverticulum have previously been reported in the literature. No case until now has been reported in a patient of Caribbean origin neither in association with ileal diverticulosis. Although rare, leiomyosarcoma is the commonest sarcoma of Meckel's diverticulum, and with full resection of the tumor the prognosis is very good.

In connection with carcinoid tumor of the appendix as an incidental finding in appendectomies, we carried out revision in a period of 5 years, from 1983 through 1987, in the General Hospital of Venezuelan Institute of Social Security in San Felix, Bolivar State. We found two cases of carcinoid tumor of the appendix in 1433 appendixes that were examined by the Service of Pathology. Both cases were males patients who were admitted due to acute appendicitis. They represented 0.14% of the total appendectomies. We review on the subject and we find that the frequency is like to that published up to now. We agree with others authors in that simple appendectomy in the carcinoid tumor of the appendix is curative in tumors < 2 cm.

Thirty-two patients with primary adenocarcinoma of the appendix were studied in this collective review. Diagnosis was never suspected preoperatively. Immediate operation was performed in 21 patients with a diagnosis of acute appendicitis. At operation, tumor of the appendix was only identified in 50 percent of the 32 patients. One patient with an obstructing tumor and peritoneal involvement died postoperatively. Estimated survival rate was 46% at five years. Probability of survival was unrelated to the histologic tumor type but significantly correlated with the extent of tumor spread. Right hemicolectomy led to a significant increase in survival compared with appendectomy alone and to a significant decrease in risk of recurrence. In Dukes' B2 and C patients, differences in the survival curves were in favor of right hemicolectomy. In patients with pseudomyxoma peritonei, long-term survival was obtained by repeated laparotomy with resection of mucinous material. Our results indicate that, in good risk patients, right hemicolectomy performed as first or second operation, provides better results than appendectomy alone.

Eighteen cases of appendiceal carcinoid in children aged 4 to 15 years, 14 of them girls, are reviewed. Most (73%) of the tumors were located in the distal third of the appendix. Tumor size, measured in histologic sections, ranged from 0.1 to 1.2 cm. In three cases the tumor had invaded the mesoappendix. No metastases were found. Standard appendectomy was performed in all cases as the only surgical procedure. The preoperative clinical diagnosis in 15 cases was acute appendicitis, which was histologically confirmed in 53%. At follow-up 2.2 to 29.3 years postoperatively (mean 16.4 years), all the patients were alive and without suspected or verified recurrence of carcinoid.

Goblet cell carcinoid of the appendix is a relatively uncommon clinical condition. Although its histogenesis remains controversial, its histological characteristics are distinctive enough not to pose a serious problem for diagnosis. The clinical behaviour of the tumor appears to lie between the typical behaviour of an ordinary carcinoid and that of a well differentiated adenocarcinoma of the appendix. Clinical diagnosis of this condition is seldom made pre-operatively, most patients presenting with signs and symptoms of an acute appendicitis. In a number of patients the tumor is identified incidentally during an operation performed for some unrelated entity. Treatment of this tumor is generally by simple appendectomy, although in some patients a more radical procedure is indicated. Prognosis for the most part is quite favorable with the exception of those few patients who present with the more virulent form of the disease.

A 48-year-old man with a clinical diagnosis of acute appendicitis on exploration was found to have a seminoma in an undescended pelvic testis. The tumor had directly invaded the retrocecal appendix, causing acute appendicitis. Such a highly unusual occurrence has not been described in the English literature.

A 29-year-old male visited our hospital because of lower abdominal pain. Acute appendicitis was suspected, and surgery was performed. However, no abnormality was found in the vermiform appendix. Instead, atrophy of the greater omentum, numerous nodes of varying sizes, and a small amount of ascites were observed. On the basis of histological examination, a diagnosis of metastatic glandular cancer was made. Gastric fluoroscopy performed 2 months earlier in another hospital had revealed irregularity of the stomach wall and a large concave area on the side of the greater curvature. Photogastroscopy and CT demonstrated progressive Borrmann type III cancer at the corresponding site and metastasis to regional lymph nodes. Biopsy findings were similar to the histological findings. After 1 week, treatment with UFT (400 mg/day), MMC (10 mg/month) and LNT (1 mg/week) was initiated. After about 3 months, i.e., following administration of 40 g UFT, 30 mg MMC and 8 mg LNT, gastric fluoroscopy and photogastroscopy revealed complete disappearance of the tumor. No abnormality was found by laboratory studies. The patient has since returned to a normal life.

Primary appendiceal adenocarcinoma is rare, with fewer than 300 reported cases. This report reviews 23 cases of appendiceal adenocarcinoma at Butterworth Hospital from 1968 to 1985. Clinical presentation, operative findings, treatment, and length of survival were recorded. Common clinical presentations included acute appendicitis and progressive abdominal distention. This disease was frequently an incidental finding during unrelated elective surgery and was often associated with other primary malignancies. Surgical treatment included appendectomy, right hemicolectomy, and interval hemicolectomy after initial appendectomy. Improved survival was noted in patients who had localized and noninvasive disease. Prognosis was most closely related to tumor grade. No patient who had localized, well-differentiated tumor died from the disease or had evidence of tumor recurrence after surgical treatment. All patients with poorly differentiated tumors died from widespread carcinomatosis. Appendectomy appears to be sufficient treatment for well-differentiated, localized, mucus secreting adenocarcinomas of the appendix.

A 50-year-old man was admitted with a diagnosis of acute appendicitis on January, 5, 1984 and an appendectomy was performed on the same day. The acutely inflamed appendix measured 7 cm in length, and a gross tumor, was seen at the apex. Histologic examination of the gross tumor, which was 1.7 cm in diameter, revealed a neoplasm representing a goblet cell carcinoid. Silver impregnation by both the Grimelius and Masson-Fontana methods disclosed a positive reaction in some neoplastic cells. Electron microscopically, endocrine granules with both limiting membranes and ahlos, 100-210 nm in diameter, were present in the neoplastic cells, Because of the relatively large size of the tumor and neoplastic invasion to the mesoappedix in this case and, in addition, the possible malignant nature of carcinoid tumor, ileoascendocolectomy was performed as an additional operation.
