A case history is reported of a spontaneous, nontraumatic, hemoperitoneum in an eleven year old girl. The onset, the accompanying symptoms and the clinical course, mimicked those of
acute appendicitis
. However the etiology was found to be the spontaneous rupture of a cavernous
hemangioma
of the omentum. Only one other similar case has been reported in the literature.

Hemangiomas
are the most common benign neoplasms affecting the liver. They occur at all ages. Most cases are asymptomatic and do not require any treatment. Rarely, hemangiomas can be pedunculated. If they undergo torsion and infarction, they become symptomatic. Herein; we report the case of a 31 year old male presenting with features of
acute appendicitis
: continuous right iliac fossa pain, rebound, guarding tenderness at McBurney' s point, nausea, anorexia, shifted white blood cell count and a Mantrels score of 6. At laparotomy a normal appendix was observed and a torsioned pedinculated liver
hemangioma
turned out to be the cause.

Retroperitoneal
hemangioma
(RH) is a rare and benign vascular malformation. RH may be detected incidentally or present with symptoms due to local invasion of adjacent structures. Management options include surgical resection, as well as serial observation with routine imaging. We describe a retroperitoneal
hemangioma
encompassing the renal vein that was discovered during diagnosis of
acute appendicitis
, and characterize diagnostic magnetic resonance imaging (MRI) findings seen with this condition.

Epithelial tumors of appendix are not so common, and mesenchymal tumors of the appendix are even less common. Capillary hemangioma of the appendix is an extremely rare event and to the best of our knowledge only 4 cases have been reported in the English literature so far. In this case report we want to explain our experience with an extremely rare occurrence of capillary
hemangioma
of appendix in a 17-year-old girl presented with right lower quadrant pain that was operated with the clinical impression of
acute appendicitis
. The patient has been operated as a routine appendectomy with a completely uneventful postoperative period.
