Twenty-nine cases of acute appendicitis during pregnancy are presented, and the recent literature is reviewed. Occurring once in 1,600 gestations, appendicitis carries a pregnancy loss of 10.8%. Difficulties in diagnosis and delays in treatment increase with advancing gestational age. Aggressive management may minimize the resultant maternal morbidity and fetal mortality.

An analysis of 944 cases of acute appendicitis treated at the University of Kentucky Medical Center from 1966 to 1973 is presented. There were 12 deaths in this series, a mortality rate of 1.3 per cent, and the clinical presentation, operative and autopsy findings are discussed. It is significant that seven of the 12 patients had other serious illnesses that influenced the decision for operation and that operation was delayed over 48 hours in 50 percent. It is stressed that the need for early exploration in the patient suspected of having appendicitis is even more imperative in the individual with concomitant disease. Adequate but immediate preoperative preparation with fluids and electrolytes antibiotics and stabilization of renal, pulmonary and cardiac status is mandatory.

Under study were the results of treatment in 10 056 patients with acute appendicitis, including 418 patients having its complicated forms. It is concluded that the extent of surgical intervention in complicated forms should be determined by the character of the complication, manifested in pathological changes of the peritoneal coat and patient's status. Appendectomy may be performed in most patients with appendicular infiltration. It is the authors' opinion that indications to peritoneal dialysis in peritonitis of appendicular origin should be largely restricted. The tactics employed by the authors enabled them to reduce the mortality in complicated forms of appendicitis up to 4.4%.

The authors have studied the motoric activity of the stomach in 40 patients with acute appendicitis and in 15 patients with other lesions. In appendicitis the strength and depth of gastric contractions were found to be mostly labile, while the number of waves of gastric contractions was relatively constant.

Twelve cases of endometriosis of the vermiform appendix are discribed and reviewed. The incidence of 0.80 per cent is higher than has been previously reported. None of the patients displayed symptoms of acute appendicitis, however, five (42 per cent) patients had symptoms suggestive of chronic or cyclic appendicitis. Laboratory tests were nondiagnostic of this lesion. There did not appear to be a correlation between the histologic location of the endometriosis and the symptoms of chronic or cyclic appendicitis. All cases of endometriosis of the appendix were discovered as a result of incidental appendectomy.

Thirty-four pregnant women with acute appendicitis presented at Parkland Memorial Hospital during a 15-year period. Abdominal pain, usually accompanied by nausea with or without vomiting, was the most common presenting symptom. Anorexia was less constant, and its occurrence decreased with advancing gestation. Physical findings usually included direct abdominal tenderness and, less often, rebound tenderness. Leukocytosis and/or a "left shift" were common laboratory findings, and the urinalysis was normal in most cases. Diagnosis was increasingly difficult as gestation progressed. This was reflected both by the increasing severity of the disease process found at surgery and by increasing fetal loss. If the diagnosis of appendicitis is suspected in the gravid patient, immediate surgical intervention is indicated to prevent the catastrophic complications associated with procrastination in diagnosis and treatment.

All cases of acute appendicitis operated upon with removal of the appendix in one hospital, and from a defined district in Norway almost exclusively served by this hospital, have been registered for the 30 year period 1943-72. While the population has decreased slightly over this period, the number of acute appendectomies has diminished to less than half. This holds true also in the age group below 20, and for different severity grades of appendicitis.

Chest and abdominal radiograms are most helpful in evaluating acute abdominal pain in children. When basilar pneumonia is found the likelihood of associated appendicitis is slim. Evidence of appendicolithiasis on abdominal films secures the diagnosis of acute appendicitis and should prompt the surgeon to early operative intervention, since the threat of appendiceal perforation is great. Barium enema is safe and informative when obscure clinical presentation or associated illness exists. The likelihood of appendicitis is great when the appendix fails to fill at the time of barium enema. Complete filling of the appendix excludes the possibility of acute appendicitis.

The authors report twenty three cases of intestinal yerseniosis revealed among 575 patients operated upon for acute appendicitis. The diagnosis was supported bacteriologically and serologically. The disease proceeded with a predominant involvement of the appendicular process. Various forms of acute appendicitis were observed. Common appendicitis was noted in 6 cases, phlegmonous--in 15, gangrenous--in 2.

Among 976 children with acute appendicitis 966 were operated upon, 169 of them (17.4%)-for common appendicitis, 401 (41.2%)-for phlegmonous appendicitis, 396 (40.4%)-for gangrenous appendicitis; 10 children were subjected to surgery due to the presence of appendicular infiltration. Preoperatively, in 137 (13.7%) patients peritonitis was observed: local-in 90, diffuse and progressive-in 47. There was noted a dependence of complications on the terms of the disease and patients' stay at the hospital prior to surgery. Following the operation in 98 (10.1%) patients different complications were observed: the postoperative wound suppuration (61), inflammatory processes in the abdomen (infiltrations, abscesses-in 33), intestinal obstruction (2), intra-abdominal hemorrhage (1), enteric fistula (1). There were no lethal issues. The preoperative complications were conditioned by gravity and advanced forms of the principal lesion, technical drawbacks in operation and treatment of purulent peritonitis.
