Samples of 15 Ss with right- and 20 Ss with left-hemisphere brain lesions secondary to war-related penetrating missile wounds were matched for age, education, and recency of injury and evaluated with the MMPI. Right-hemisphere lesioned Ss produced a composite profile with all scales within normal limits. In contrast, the composite profile of left-hemisphere lesioned Ss showed significant elevations on the Sc, D, and Hs clinical scales, suggesting increased psychopathological responses in such Ss. Although the general configurations of the composite MMPI profiles in the two samples were similar, significant differences in the elevation of both validity and clinical scales were obtained. The results tend to support previous findings of a depressive-catastrophic reaction in patients with lesions in the dominant hemisphere, while not supporting the euphoric-indifference response in nondominant-hemisphere lesioned Ss.

The inability to transfer classroom knowledge to clinical nursing practice is a common learning problem encountered by many nursing students. Manifestations of this problem may involve both the academic performance and personal development of the students: inability to solve problems in nursing situations; inflexibility and rigidity in the exercise of nursing care; fragmentation of nursing care; and apathy towards clinical practice. Because of the seriousness of this learning problem, a plan must be formulated to rectify it. This plan is developed with major emphasis on the teaching process, the student and the curriculum. One of the important roles of a teacher is to help students understand many widely useful relationships, principles or generalizations. In order to enhance transfer, nursing students must be given ample opportunities to apply the learned principles in a variety of nursing situations. Learning is a self-active process; an ideal transfer demands the students' conscious realization that transfer is possible. Students need to be committed to the belief that particular facts in the classroom study are pertinent in other situations. To promote transfer, the curriculum must be designed in such a way so that it has transfer value in terms of the students' goals and purposes. Furthermore, proper sequence of curricular activities is crucial if transfer of classroom knowledge to clinical practice is to occur.

A household survey conducted in the Albany Health Region in New York State in 1974 revealed that the use of contraception by married women was very similar to that found in a national survey in 1973. In both surveys the majority of married women were currently using a contraceptive method, and most were using the most effective methods. Results of the Albany Health Region survey show that women residing in lower socioeconomic areas, women with less education, and Catholic women were less likely to be using any method. Despite these differences, the majority of women in all these groups were practicing contraception. Private physicians were found to be the most important source of contraceptive methods. Overall, contraceptive protection could be increased by providing services to those women not currently using contraception who are unable to give good reasons for not being protected, and by shifting women using less effective methods toward more effective methods of contraception. The survey was conducted to obtain information on correlates of planned and unplanned fertility, family planning practices, and the sexual experience and contraceptive practice of unmarried women. This report describes the prevalence of use and the source of contraception for married women in the Albany Health Region Survey. The Albany Health Region Family Planning Survey was a household probability sample of all women 15-44 years of age living in the 18-county Albany Health Region area. Interviews were completed for 90% of the sampling of 2291 women. If nonusers are excluded, of all women using contraceptives, 1/3 were using oral contraceptives, 1/4 were contraceptively sterilized or their partners were, 1/8 were condom users, and 1/10 were using IUDS; these 4 methods account for 81% of all users compared with about 83% in 1973 U.S. estimates. Reflecting the greater proportion of Catholics, the rhythm method is used to a greater extent in the Albany Health Region, 9.1 vs. 4.1% of all users. Of the married women who are using contraceptives, 32% are not using the most effective methods - oral contraceptives, IUDs, and sterilization. Lack of access does not appear to be the reason for nonuse, but lack of motivation is apparently important because indifference and nonspecific reasons account for a large number of nonusers.

Sixteen cases of neonatal cold injury, five of them fatal, were seen in the winter of 1974-75. The affected infants, who weighed from 2.5 to 3 kg. had developed symptoms when the ambient termperature was below 10 C. Few of them were referred as cases of hypothermia. Refusal to eat was the most common complaint and less often edema and/or apathy. No correlation was found between death and ethnic origin, sex, duration of illness or minimum temperature. Admission weight, however, tended to be lower in the infants who died. The consistent finding of thrombocytopenia and the suspected bleeding phenomena suggested that disseminated intravascular coagulation may have been a factor in the unfavorable outcome of some of the cases. Trichothecin (T-cin), amphotericin B (AB), and 5-fluorocytosine (FC) were compared singly and in combination for capacities to inhibit growth of Cryptococcus neoformans in culture and to protect mice bearing infections with this yeast. The minimum inhibitory concentrations for T-cin, AB, and FC were found to be 0.5, 0.2, and 5.0 mug/ml, respectively. In vitro viability studies demonstrated a marked reduction in colony counts with the AB-FC combination and additive effects with the AB-T-cin and FC-T-cin combinations for a 3-day period. In mice infected intravenously with C. neoformans, the mean effective dose for AB was 0.38 mg/kg, and for FC it was 100 mg/kg for a 30-day treatment period. No mean effective dose could be ascertained when T-cin was tested at doses of 0.1 to 50 mg/kg. Despite this, marked beneficial effects were noted in vivo with the AB-T-cin combination, whereas additive effects and indifference were observed for AB-FC and FC-T-cin combinations, respectively. High-dose T-cin controls survived despite having received a cumulative dosage of more than twice the reported (LD(50)) mean lethal dose value.

The neuropsychiatric symptoms of old patients with disturbed cerebral metabolism or blood flow mostly lead to great individual difficulties and make those patients difficult to handle: in the family as well as in hospital such patients develop alienation, isolation and therefore adaptation to a social structure deteriorates with time. In the course of a test program for medicinal therapy of this syndrome we studied the efficacy of a vincamine containing formulation (Pervincamin forte retard or Pervincamin ampoules, respectively) on the symptoms of a chronical brain disturbance. The study was randomised double-blind. We found that under the influence of the vincamine formulation the subjective symptoms, such as lack of interest, apathy, aggressiveness, raging, psychomotoric retardation, lack of concentration, dysmnesia, decreased with a statistical significance (p less than or equal to 0.05). Also the subjective symptoms, reported by the patients, such as tinnitus and vertigo decreased significantly under the treatment with the vincamine preparation. Therefore some of the parameters important for resocialisation and revitalisation of old patients could be influenced in a favourable way. Basing on the good results of our investigations, the treatment of psychiatric disturbances in old patients with vincamine containing drugs seems to be justified.

Hospital house staff movements for collective bargaining have persisted for more than 35 years. The problems addressed seen equally persistent--inadequate pay, excessive hours, poor working conditions, and low standards of training and patient care. Interns and residents continue to face the organizational handicaps of transient membership, poor funding, and apathy, yet negotiated contracts indicate limited legal successes. Improved patient care conditions are less evident than improved house staff conditions.

In the late 1880s it became apparent in Louisiana that leprosy was endemic in the southern part of the state. Initially, the intention was to establish a leprosy hospital in the city of New Orleans, close to medical facilities, and where the bulk of the patients were to be found. The establishment, instead, of an isolated leper colony at the run-down plantation at Carville, 85 miles up-river, was the result of community indifference, misunderstanding of the nature of the disease, and expected depreciation of property values. Fear of the disease was a secondary matter. The practice of locating residential facilities for the chronically ill at long distances from the centers of physician practice and medical research continues to this day. Interestingly, the arguments that permit this to happen have not changed appreciably from those of a century ago.

A new antidepressant, amoxapine, which is a dibenzoxazepine deprivative, was compared with amitriptyline in a randomised double-blind trial. Forty-eight patients were included and 41 completed a 4-week treatment. Most of the patients were maintained on 150 mg daily. Assessments were made by the Hamilton Psychiatric Rating Scale for Depression (HAM-D), Nurses' Observation Scale for Inpatient Evaluation (NOSIE), Clinical Global Impression (CGI) scale and Patient's Self-Evaluation. The total HAM-D score was considerably reduced in the majority of the patients. Amitriptyline was the most effective with regard to symptoms included in the factor Sleep Disturbances and-secondary maybe-towards some items included in the factor Somatization. For the remaining items,including the items of the factors Anxiety/Depression and Apathy, the last score was lower in the amoxapine group than in those treated with amitriptyline. Among the unipolar cases the amoxapine treated patients were more satisfied with regard to efficacy (P = 6.3%). The frequency of side effects such as tremor and dizziness was considerably lower in the amoxapine group. In total, the side effects lasted longer in the amitriptyline group. We conclude that amoxapine seems to be an effective antidepressant with a low frequency of side effects.

42 strains of gram-negative bacteria, isolated from clinical material, resistant to trimethoprim (TM), sulphamethoxazole (SMZ) or both, have been tested for synergistic sensitivity to TM + SMZ. The combination showed no synergy against strains 'highly resistant' to SMZ (MIC greater than 1 mg/ml). Indifference was invariably observed. However, synergy was seen with strains 'moderately resistant' to SMZ (MIC 100--1,000 microgram/ml). Our results indicate that present techniques for the testing of urinary tract isolates for sensitivity to co-trimoxazole need revision.
